The *Classic of Internal Medicine* states: Changqin (Intestinal Polyp/Tumor) is caused by cold pathogen invading and lodging outside the intestines, struggling with Wei Qi (Defensive Qi), preventing Qi from nourishing properly. Due to this obstruction, a mass forms and adheres internally, evil Qi arises, and polyps grow. Initially, it is as large as a chicken egg, gradually enlarging until it resembles pregnancy. It feels firm upon pressure and does not move when pushed, while menstruation continues on schedule; these are its signs. This indicates a disease in the Qi level but not yet in the Blood level, hence menstruation remains uninterrupted. It should be treated with Muxiang Tongqi San (Aucklandia Qi-Regulating Powder), a formula of great acridity and heat (this involves stagnation of Qi in the Large Intestine, constituting a Qi-level disorder).
*Zhe Gong Lu* records: A daughter-in-law, Shen Shi, suffered from frequent depression and anger, leading to amenorrhea. Her abdomen gradually enlarged, and she was presumed pregnant. After more than ten months without delivery, various symptoms emerged, causing suspicion. The physician also suspected blood stasis requiring purgation, but due to her weak constitution, he opted for gradual dissolution using blood-moving and regulating herbs over a long period, which ultimately proved fatal. Later, reading Dr. Sheng Qidong’s case of treating the Crown Prince’s Consort, he deeply regretted his error. During the Yongle era, Consort Zhang of the Crown Prince had experienced amenorrhea for over ten months; physicians assumed it was pregnancy. As abdominal distension worsened, the Emperor ordered Dr. Qidong to examine her. His diagnosis matched the presentation exactly, and his prescription consisted of blood-breaking agents. After expelling several dou of blood, the illness subsided. The daughter-in-law’s condition perfectly matched this scenario. Had the physicians boldly employed purgative methods after confirming non-pregnancy beyond the tenth month, she could have been saved. Alas, lacking such courage and insight, how tragic! (This involves blood stasis in the uterus, constituting a Blood-level disorder).
All antenatal syndromes are caused by fetal Qi. Both Taidong (Fetal Movement) and Tailou (Fetal Leakage) involve vaginal bleeding, but Taidong is accompanied by abdominal pain, whereas Tailou is not. Therefore, Taidong requires Qi-regulation, while Tailou requires Heat-clearing. Ezu (Nausea and Vomiting of Pregnancy) refers to nausea obstructing food intake; obesity often stems from Phlegm, while emaciation often stems from Heat. Erchen Tang (Two-Cured Decoction) with modifications is suitable. Zifan (Fetal Irritability) refers to restlessness and mental confusion. Zixian (Fetal Convulsion/Eclampsia) refers to surging phlegm-saliva with convulsions, upward deviation of the eyes, and locked jaw. Zizhong (Fetal Edema) refers to puffy swelling of the face and limbs. Ziqi (Fetal Gas/Edema of Legs) refers to edema of both feet. Zilin (Fetal Strangury) refers to difficult and scanty urination. Zhuanbao (Displacement of the Bladder) refers to urinary retention. Zixuan (Fetal Suspension) refers to irregular fetal Qi rushing to the heart and chest, causing distension and pain. Generally, the Spleen governs the transformation and transportation of water and grain. When a woman is pregnant, the transformation of water and grain becomes inefficient, generating Dampness; Dampness generates Phlegm; Phlegm generates Heat; Heat generates Wind. Zizhong and Ziqi belong to Dampness; Ezu belongs to Phlegm; Zifan and Zilin belong to Heat; Zixian belongs to Wind; Zixuan belongs to Qi; Zhuanbao belongs to Deficiency. Dampness is treated by leaching/diuresis; Phlegm by resolving; Heat by clearing; Wind by calming; Qi by dispersing; Deficiency by tonifying. The primary goal is always to eliminate pathogens and preserve the fetus. The method for preserving the fetus is to nourish the Spleen and Stomach before the third month, and to strengthen the Waist and Kidneys, nourish Blood, and smooth Qi with Heat-clearing adjuncts after the fourth month; this is the general principle. However, regulation must further be based on the individual’s Yin-Yang excess or deficiency and Qi-Blood predominance.
*Jianbian Fang* treats habitual miscarriage, or miscarriage occurring at three or four months. Two months prior, use eight liang of Duzhong (Eucommia Bark), soaked through in rice decoction, stir-fried to remove silk fibers. Mix with two liang of Xuduan (Dipsacus Root), wine-soaked and baked dry, ground into powder. Add five to six liang of Shanyao (Chinese Yam), ground into powder. Make pills the size of wu zi (smartweed seeds) using paste. Take fifty pills each time with empty-stomach rice drink.
Conception relies on the deficiency of one organ, the Liver. Its Liver Qi can only generate the fetus, having no other function. If Blood is insufficient to nourish the Liver, the Liver becomes deficient, leading to a craving for sour foods.
For antenatal fetal preservation, Baizhu (Atractylodes Rhizome) and Huangqin (Scutellaria Root) are excellent herbs. Tiao Qin (Striped Scutellaria) is the sacred herb for preserving the fetus. Laypeople do not understand this, fearing its cold nature, and instead use warm-hot herbs to nourish the fetus. They fail to realize that antenatally, Heat should be cleared so that Blood follows the meridians without reckless movement, thereby nourishing the fetus. Only in cases of deep, slow pulse and Spleen-Stomach weakness is it unsuitable.
Childbirth is like carrying a boat over a dam. Difficult labor mostly results from Qi and Blood deficiency. Some occur due to lack of caution during the ninth and tenth months; others due to Qi and Blood stagnation failing to circulate. Therefore, within the ninth month, one must smooth Qi and regulate the Middle Jiao by taking Dasheng San (Life-Accomplishing Powder) for over ten doses, making labor easier at term.
If a pregnant woman’s pulse is fine and regular, the fetal origin is complete, resembling the fragrance of ripe fruit; delivery will be easy. If the pulse is large, floating, and moderate, it indicates severe deficiency of Middle Qi due to nurturing the fetus, with Fire Qi scattering; delivery will be difficult. Thus, the antenatal pulse should be full and large, while the postnatal pulse should be deep and small. To distinguish male from female, observe the left and right pulses: rapid left pulse indicates male, rapid right pulse indicates female. Deep and solid on the left, large and floating on the right indicates female then male, allowing for prediction. Pulse deviating from the norm with six beats per respiration, deep, fine, and slippery, with intermittent pain reaching the waist, indicates immediate expulsion of the fetus. Half-miscarriage with leakage is indicated by Ge Mai (Cord-like Pulse); weak pulse indicates Blood exhaustion and imminent danger.
A woman with prolonged vomiting during pregnancy, refusing all medicines, was cured by a prescription from Zhang Lihou. Both Cun (Inch) pulses were full, large, and rapid, especially on the right. Metal constitution is naturally dry, now even drier. Left Guan (Bar) pulse is full and wiry, indicating the Liver storing Blood is currently devoid of Blood to store. Right Guan pulse is wiry, fine, and rapid, indicating Grain Qi generated over time, with Liver Deficient Fire insulting, hence wiry and rapid. The fineness indicates extreme depletion of Stomach Qi. Left Chi (Cubit) pulse is extremely fine, almost absent; Right Chi pulse is full, large, and rapid. These indicate fluid nourishment exhausted and dried up, with Shaoyin Deficient Fire floating upward, Jueyin Depressed Fire reaching up, and Shaoyang Hidden Fire ascending. The dried earth is scorched by the Three Fires; how can it moisten and nurture? The Classic says all vomiting and acid regurgitation belong to Heat; this is what it means. The following prescription is for emergency symptomatic treatment. After vomiting ceases, continue with Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) plus Maidong (Ophiopogon Tuber) and Ejiao (Donkey-Hide Gelatin) as pills for long-term conditioning to treat the root.

Do not increase the dose beyond seven pills just because they are small. For pregnant women with unstable fetus, take four to five times a day; once stable, resume normal frequency. If abdominal pain, lower back soreness, or distension/heaviness occurs, take immediately. If prone to miscarriage at three to five months, start taking it one month prior to ensure full-term pregnancy. Even if spotting occurs threatening miscarriage, urgent intake can preserve it. Strictly avoid anger and physical labor. Avoid fried, spicy, gas-producing, qi-blocking, fermented, cold water, and cold foods. Strictly avoid mental exertion.
