潜，彻夜不寐，心悸忡怔，有不能支持之候，脉弦滑数，左寸关长直，治宜清心和胃，佐以平肝。

处方：元参、连翘心、纯钩藤、陈胆星、鲜竹沥、鲜细叶（天竺黄）、石菖蒲汁（同冲服）、羚角片、川郁金、石决明（青黛拌打）、牛黄清心丸、薄荷梗、丹皮、朱茯苓、青荷梗。
又（二里诸姓人，三十一岁，右侧患病）感受湿邪，动扰肝阳，突然发生厥逆。
处方照前法去薄荷、郁金、胆星、竹沥，加沉香、二陈汤、姜竹茹。

Wang (from Lucun, 56 years old, November 26th) Blood deficiency generates wind, causing hemi-cranial headache. Severe pain damages the eyes, leading to corneal opacity (cataract-like visual obstruction). Accompanied by tidal fever, bitter taste in the mouth, palpitations, dizziness, and poor sleep and appetite. The pulse is small, wiry, and rapid. Treatment should nourish Yin and subdue Yang.
Prescription: American Ginseng (Xi yang shen), Chrysanthemum Bud (Gan ju rui), Moutan Cortex (Dan pi), Rose Flower (Mei gui hua), Prepared Polygonum multiflorum Root (Zhi shou wu), Angelica sinensis Root (Gui shen), Abalone Shell (Shi jue ming), Mulberry Leaf (Dong sang ye, stir-fried), Vitex Fruit (Man jing zi), White Peony Root (Dong bai shao), Poria with Hosta Root (Zhu fu shen).
If caused by external wind, treat according to the method of Chuanxiong Chatiao San (Ligusticum Chattison Tea-Powder Dispersion).
For various winds causing tremors and dizziness, if phlegm is abundant and suitable for epilepsy or syncope, treat with Jue Fang (Syncope Formula).
Zhang (Patient name/alias) Phlegm disease has progressed to epilepsy and syncope; fire and wind originate from the Liver.
Prescription: Scrophularia Root (Yuan shen), Moutan Cortex (Dan pi), Pinellia Tuber (Song ban xia), Aged Tangerine Peel (Hua chen pi), Fresh Bamboo Sap (Xian zhu li), Fresh Talcum-like substance (Xian xi ye - likely Tian Zhu Huang / Bambusae Concha), Rhizoma Acori Tatarinowii Juice (Shi chang pu zhi, mix one spoonful), Antelope Horn Slice (Ling jiao pian), Uncaria Rhynchophylla (Chun gen teng), Curcuma Aromatica Rhizome (Chuan yu jin), Arisaema Cum Bile (Chen dan xing), Gastrodia Tuber (Ming tian ma), Poria with Hosta Root (Zhu fu shen), Abalone Shell (Shi jue ming, mixed with 5 fen Qing dai / Malachite Green and pounded).
Zhu You (from Shimo Road, 16 years old, June) Summer-heat, dampness, and wind pathogens brew phlegm and transform into heat. From the Lung and Stomach, they disturb Liver Yang. Phlegm rises with Qi, suddenly causing syncope, loss of consciousness, locked jaw, and finger spasms. The pulse is wiry, slippery, and rapid. Treatment clears the Heart, eliminates phlegm, calms the Liver, and opens the orifices.
Prescription: Scrophularia Root (Yuan shen), Forsythia Heart (Lian qiao xin), Uncaria Rhynchophylla (Chun gen), Arisaema Cum Bile (Chen dan xing), Fresh Bamboo Sap (Xian zhu li), Fresh Talcum-like substance (Xian xi ye), Rhizoma Acori Tatarinowii Juice (Shi chang pu zhi, mix together), Antelope Horn Slice (Ling jiao pian), Curcuma Aromatica Rhizome (Chuan yu jin), Abalone Shell (Shi jue ming, mixed with Qing dai / Malachite Green and pounded), Niuhuang Qingxin Wan (Calculus Bovis Clear Heart Pill), Mint Stem (Bo he geng), Moutan Cortex (Dan pi), Poria with Hosta Root (Zhu fu ling), Lotus Stem (Qing he geng).
Again (From Erli, surname Zhu, 31 years old, right side affected) Contracted damp pathogen, disturbing Liver Yang, suddenly causing syncope.
Prescription follows the previous method but removes Bo he (Mint), Yu Jin (Curcuma), Dan Xing (Arisaema Cum Bile), and Zhu Li (Bamboo Sap); adds Chen Xiang (Aquilaria Wood), Er Chen Tang (Two-Cured Decoction), and Jiang Zhu Ru (Ginger-processed Bamboo Shavings).
Certain Patient (Surname unknown) Heart substance is insufficient, while Heart function is excessive. Liver is the mother of Heart, managing the spirit mechanism. Liver Wood and Heart Fire mutually incite each other. Liver Yang floats upward and fails to submerge. Insomnia throughout the night, palpitations and fright, with a sensation of inability to sustain. The pulse is wiry, slippery, and rapid; the left Cun and Guan positions are long and straight. Treatment should clear the Heart and harmonize the Stomach, assisted by calming the Liver.
