Zi (a type of toxin), use detoxification methods, or Chan Gan Wan (Toad Liver Pill). If there is exterior pathogen but sweating is contraindicated, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Fangfeng (Saposhnikovia Root) and Baizhi (Angelica Dahurica); if there is interior syndrome but purgation is contraindicated, use Fengshe San (Hornet and Snake Powder). If swelling and pain are about to suppurate, use Tuoli San (Support the Interior Powder) or Neituo Shixuan San (Interior-Supporting Ten-Aperture Powder); if it cannot ulcerate, tonify with large doses of Renshen (Ginseng), [Huangqi (Astragalus)], Danggui (Angelica Sinensis), and Baizhu (Atractylodes Macrocephala), or combine Buzhong Yiqi Tang with Shengmai San (Generate the Pulse Powder) to prevent toxin sinking inward. Huoxin Qi (Opening the Heart Qi - referring to symptoms of toxin attacking the heart): when Ding Du (Ding Toxin) enters the heart, there is coma, dry mouth, irritability and oppression, mental confusion resembling drunkenness, and irregular vomiting; this is a critical condition. For excess patterns, use Wanbing Jiedu Dan (Ten-Thousand Disease Detoxifying Pill), taken with decoction of Huanglian (Coptis Rhizome) and Danggui (Angelica Sinensis); for deficiency patterns, use Gu Xionggui Tang (Ancient Ligusticum and Angelica Decoction) plus Fuling (Poria), Fushen (Poria with Hostwood), Yuanzhi (Polygala Root), and Lianrou (Lotus Seed Flesh) to clear. If toxin attacks upward to the heart with vomiting, use Huxin San (Heart-Protecting Powder). If vomiting occurs due to taking Saiming Dan (Life-Saving Pill), this powder is also suitable to resolve it. For mental confusion, restlessness, inability to sit or lie comfortably, irritability, thirst, body pain, and constipation, use Loulu Yinzi (Rhaponticum Decoction); for irritability and thirst, use Zhuye Huang[Tai] Tang (Bamboo Leaf and [Astragalus] Decoction). External treatment: for mild cases, use single Chansu (Toad Venom) powdered, kneaded with white flour and Huangdan (Lead Monoxide) into pills the size of wheat grains. Puncture the head of the Ding with a needle and insert one pill; effective. For severe cases, use Saijindan (Gold-Competing Pill); for critical cases, use Tiding Dingzi (Ding-Lifting Stick). Sudden death can be reversed by moxibustion.
Any sudden death is mostly due to Ding Du (Ding Toxin); urgently illuminate the whole body with a lamp. If small sores are found, urgently apply moxibustion and administer Saiming Dan (Life-Saving Pill); some may regain consciousness. In remote areas where medicines are hard to deliver, only moxibustion has the power to revive. If one relies solely on draining or dispersing methods, it is dangerous.
Duogu Ju (Multiple Bone Abscess) results from long-standing ulceration where Qi and Blood fail to nourish the affected area; over time, tissue rots and bone detaches. Treatment focuses on tonifying the Spleen and Stomach and strengthening Yuan Qi (Original Qi). Treat with Shiquan Dabu Tang (Comprehensive Great Tonifying Decoction) or Shenqi Wan (Kidney Qi Pill). Externally, apply Moxibustion with Aizi Bing (Aconite Cake) or Congyi Fa (Scallion-Ironing Method) to dispel cold pathogens and replenish Ying Qi (Nutritive Qi), then the bone will naturally detach and the sore heal. If Kidney Qi is deficient, the bone gradually swells; after dragging on for years, pus erupts with bone; the Scallion-Ironing Method should also be used here. If harsh reducing herbs are administered, Zheng Qi (True Qi) becomes more deficient and evil Qi more severe, leading to error. Some present with upper swelling and hardness, ulcerating only after a year, not healing for half a year, with internal heat, fatigue, and thirst; treat with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) and Shenqi Wan (Kidney Qi Pill). As Yuan Qi (Original Qi) recovers, a piece of bone is expelled, and continuing the previous medication leads to recovery. Some have a piece of bone falling from the dorsum of the foot; some from the dorsum of the hand.
Fanhuachu (Flower-Flipping Sore) occurs when a sore is about to heal but Yuan Qi (Original Qi) is weak, causing Liver Fire and Blood Dryness generating Wind. A piece of flesh flips out like a mushroom; sizes and lengths vary, some snake-like and several inches long. Apply Xionghuang Powder (Realgar Powder) externally. Internally, take herbs that tonify and nourish the Spleen and Stomach, such as Shiquan Dabu Tang (Comprehensive Great Tonifying Decoction), or Bawu Tang (Eight Substance Decoction) with doubled Renshen (Ginseng), [Huangqi (Astragalus)], Danggui (Angelica Sinensis), and Baizhu (Atractylodes Macrocephala). Bleeding indicates the Liver fails to store blood and the Spleen fails to control it; treat with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Wuweizi (Schisandra Berry) and Maidong (Ophiopogon Tuber), or Shenqi Wan (Kidney Qi Pill). For anger-fire, use Bawei Xiaoyao San (Eight-Ingredient Free and Easy Wanderer Powder). Using Wind-dispersing herbs accelerates death; excessive sweating inevitably leads to [Jing] (Spasm/Cramp), which is dangerous! Apply Licilu Gao (Veratrum Ointment) evenly externally. Licilu (Veratrum Nigrum) powdered alone, mixed with lard for application, changed once a week. Apply only when Yuan Qi (Original Qi) has gradually recovered and pus/toxin is nearly exhausted, so necrotic flesh falls off naturally; otherwise, it may fall off and grow back. If needles, knives, or cauterization are mistakenly used, the condition worsens, possibly with profuse bleeding, chills/fever, and vomiting; urgently tonify the Spleen and Stomach.
Liuzhu (Flowing Note) lumps are no ordinary matter, caused by internal injury and external contraction of damp-phlegm; contusions, sprains, and postpartum conditions lead to Qi stagnation and Blood stasis in the limbs' joints. "Liu" means moving; "Zhu" means staying. It presents as nodules or slow swelling, originating from pre-existing Phlegm-Fire or external contraction of Wind-Cold, where evil Qi flows and settles in phlegm-stagnant areas to manifest; or from internal injury due to suppressed anger causing sudden Phlegm-Fire eruption; or from internal injury due to sexual indulgence, where Yin deficiency allows Yang Qi to invade, reversing in the muscle layers; or from labor/dietary factors; or from contusions/sprains causing sudden Qi reversal and Blood coagulation; or from postpartum lochia not clearing, compounded by external contraction and stagnation. It often occurs in the limbs, or at joints of the chest, abdomen, waist, and buttocks. At onset, use the Scallion-Ironing Method; for excess patterns, use Shi Liuwei Liuqi Yin (Sixteen-Ingredient Flowing Qi Decoction) or Baidu San (Defeat Poison Powder); for phlegm-pain with constipation, use Gu Banxiao Wan (Ancient Half-Pepper Pill); for deficiency patterns, choose from Erchen Siwu Tang (Two-Aged Four-Substance Decoction), Tuoli Yiqi Tang (Support Interior and Augment Qi Decoction), Buhuanjin Zhengqi San (Non-Changing Golden Upright Qi Powder), Liujunzi Tang (Six Gentlemen Decoction) plus Chuanxiong (Ligusticum) and Danggui (Angelica Sinensis), or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Muxiang (Aucklandia) and Zhike (Bitter Orange). Allow it to ulcerate and dissipate spontaneously. If it ulcerates and fails to heal for a long time, even if exterior pathogens exist, focus on supporting the interior with Shiquan Dabu Tang (Comprehensive Great Tonifying Decoction), Renshen Yangrong Tang (Ginseng Nourishing the Spirit Decoction), Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), Tuoli Yiqing Tang (Support Interior and Suppress Green/Liver Decoction), Tuoli Yiqi Tang (Support Interior and Augment Qi Decoction), or Bawei Wan (Eight-Ingredient Pill), aided by Douchi Bing (Fermented Soybean Cake) and Hupo Gao (Amber Ointment) to dispel cold and replenish Yang Qi. When pus forms, puncture with fire needle; if sinus tracts exist, use medicated thread for debridement. Overuse of cold-cooling herbs renders it incurable.
Biao Ju (Bubonic Abscess) ranges from plum-sized to millet-sized, often on fingers, buttocks, and feet; color changes unpredictably, penetrating deep into muscles, involving sinews and bones with extreme pain. Biao Ju, also called She Zhang (Snake Miasma), is common in miasma-prone regions. It starts as a dot then exposes flesh, with edges black and hard like an ox's lip; small ones like millet or beans, severe ones like plums or peaches. Onset location is不定 (variable), arms, buttocks, mouth/teeth, or navel, often between fingers/toes. Colors change unpredictably among red, black, green, white. Roots penetrate deep, swelling travels along arms, toxic blood flows, penetrating sinews, rotting flesh exposing bones, massive bleeding, causing radiating pain and delirium. Pain entering the heart causes death; protrusion onto the external genitalia also causes death. Caused by惡风 (Evil Wind) accumulating toxin and Blood Heat, leading to irritability and oppression entering the heart/abdomen. Etiology: contraction of Evil Wind entering vessel channels, or miasma regions, post-Warm Pathogen/Malaria, or Snake venom contact. Ages 20-40: accumulated injury toxins enter the Stomach and accumulate; ages 40-60: Blood closure/stagnation, accumulated heat and Blood stasis. Treatment: promote detoxification and move Blood, using Gualou Gen (Trichosanthes Root) decocted in wine, plus Rushxiang (Frankincense), Moyao (Myrrh), Wulingzhi (Trogopterus Dung), and Zaocis (Hedgehog Spine) in equal parts to purge toxin, followed by formulas clearing Heart and moving Blood. For snake venom, Chi Zu Wu Gong (Red-footed Centipede) is best, followed by Xionghuang (Realgar) and Baizhi (Angelica Dahurica); or Lafan Wan (Wax-Alum Pill) taken with cold wine and Shexiang (Musk). Externally, decoct Jingjie (Schizonepeta), Baizhi (Angelica Dahurica), Chuanjiao (Sichuan Pepper), and Congbai (Scallion White) with salt; while warm, pour/scald from arm downwards, three times daily. Tighten binding above the spreading swelling of Biao Ju. Hand-originating spreads to Heart; foot-originating spreads to Kidney, both incurable. Look for small red veins; moxa three cones at the vein's location for cure. Classics state: amputate if on finger, cut if on flesh. Fear toxin entering Heart/Abdomen causing irritability, belching, coma, or green foul fluid from sore indicating extreme Kidney Deficiency, leading to rapid death. This sore easily attracts Wind. For every inch of open abscess, wind attraction increases; not necessarily wind entering inside, but Wind Evil attacking deficiency causes flesh rot through bone and evil blood flowing freely. Use Nanxing (Arisaema), Banxia (Pinellia), Baizhi Cao (Angelica Dahurica tip) to dispel wind, applying frequently. Other treatments follow general Abscess protocols.
Gu Fa (Stubborn Eruption) on hands/feet/palms/waist/buttocks, how deep is the toxin? No head or face, recurring after healing, pale red color warns of diarrhea invasion. All sores involve Qi-Blood conflict; those with head/face indicate Wind Evil acting internally without head/face. Gu lacks head/face, opens inward, lies flat on muscle, spreads and connects, occurring on hands/feet, palms, waist/legs, or buttock flexures. Onset: high fever, limb immobility, aversion to cold/headache, deficiency-thirst/sweating, vomiting, heavy limbs, worse thirst than other eruptions. If swelling subsides but re-emerges months later elsewhere as pale red flesh, it is incurable. Key: cultivate internal Qi to prevent loose stools; treatment follows Abscess logic. Apply Shenyin Gao (Miraculous Response Ointment) externally. Rice-water discharge from sore indicates death.
Small sores involve Wind-Toxin stagnation. Various minor sores result from Heart-Kidney non-interaction, irregular diet, retention in intestines/stomach, causing Wind-Heat-Cold-Damp toxins to conflict with Qi/Blood, stagnating in muscles. Summer heat prickles and frost cracks hands/feet. Summer heat causes Heart-Mind agitation, heat forces sweat causing sores, oozing pus/blood, red/raw like fire. Use Nanxing (Arisaema), Banxia (Pinellia), Huanglian (Coptis), Huangbai (Phellodendron) each 1 qian, Wubeizi (Chinese Gall) and Huangdan (Lead Monoxide) each 5 fen, powdered and dusted dry. Add Kufan (Alum calcined) and Xionghuang (Realgar) if itchy. Take Huanglian Ejiao Wan (Coptis and Donkey-hide Gelatin Pill) regularly to clear Heart. Heat-sweat maceration causing sores with unceasing itch/pain: use Huang[Tai] (Astragalus), Danggui (Angelica Sinensis), Fangfeng (Saposhnikovia), Jingjie Sui (Schizonepeta Tip), Digupi (Dogwood Bark), Mutong (Akebia) each 2 qian, Bai Fan (Alum) 1 liang, powdered. One liang medicine per three bowls water, boil 5-6 times, strain, wash lesion while slightly hot, dry and avoid wind, immediate effect. Mild cases: snow water and Ge Fen (Clam Shell Powder). Prickly heat acne arises from sweat meeting damp; mild like scattered millet, wash with Qinghao (Artemisia Annua) decoction or Zao Ye (Jujube Leaf); severe macerated by sweat, use Lv Dou (Mung Bean) and Huashi (Talc) each 5 qian, powdered, applied with cotton; if rubbed into sores, add Huangbai (Phellodendron) and Zao Ye (Jujube Leaf) each 5 qian, Pian Nao (Borneol)少许. Winter deficiency-cold causing stiff skin sores with numbness: internally take Shengma Heqi Yin (Cimicifuga Harmonizing Qi Decoction) minus Dahuang (Rhubarb); externally mix Muxiang (Aucklandia), Binglang (Areca Nut), Liuhuang (Sulfur), Wuyu (Evodia), Jianghuang (Turmeric), Shexiang (Musk) in powder, apply with Sesame Oil. Frostbite itches then pains, swells, breaks, bleeds, weeps yellow fluid: use Male Partridge Brain 1, mashed with equal Huangla (Yellow Wax) and half Qingyou (Clear Oil), simmered into ointment, strained and applied; effective for chronic cases. Another formula: Sheng Fuzi (Raw Aconite) powdered, mixed with flour. Hand/foot fissures painful: boil 5 qian Qingyou (Clear Oil), add Huangla (Yellow Wax), melt, add Shufen (Powdered Lead) and Wubei powder少许, boil until purple. Soak hands/feet in hot water, dry over fire, apply medicine, cover with paper. Pain stops immediately, withstands water immersion. Tung oil ointment also works. Hand/foot cracks: soak in boiling water, soften skin, dry, then apply Liquing (Bitumen) 2 liang and Huangla (Yellow Wax) 1 liang melted together; or Wubeizi (Chinese Gall) powder mixed with Ox Marrow, stored in porcelain jar buried 7 days, apply to cracks. Yellow Wax Ointment or Yunmu Gao (Mica Ointment) also good.
Wo Chuang (Snail Sore), Invasive Sore, White Snake Wrap. Wo Chuang grows between hands/feet, opposite like new Zhu Yu (Cornel fruit), itchy/painful/fissured, scratching yields yellow fluid, holes like snails, worms generate over time. Use Xingren (Apricot Kernel), Rushxiang (Frankincense) each 3 qian, Liuhuang (Sulfur), Qingfen (Calomel) each 1.5 qian, powdered. Mix with 3 qian Sesame Oil and 5 qian Huangla (Yellow Wax) melted, stir into ointment, remove fire toxicity, store in porcelain. Another method: soil from swallow nest where eggs are laid, powdered and dusted. Wash with Baizhi (Angelica Dahurica) and Dafupi (Areca Peel) decoction first. Zou Pi Chuang (Skin-Traversing Sore) fills cheeks/neck, emerges like bean/plum, itchy with discharge, spreads to ear内外 (inside/outside) wetly like invasive sore. Wash with Sangjisheng (Mistletoe), Sanggenpi (Mulberry Root Bark) handfuls, Baizhi (Angelica Dahurica), Huanglian (Coptis)少许 decoction, wipe dry after evil blood exits. Then burn Zaojiao (Soap Pod) and Mazhutuo (Bamboo Husk) to char, mix with Huangbai (Phellodendron), Huanglian (Coptis), Zhangye (Camphor Leaf), Baizhi (Angelica Dahurica) equal parts, apply with Sesame Oil; miraculous. Avoid vinegar. Hand Sore: apply Zaojiao (Soap Pod), Kufan (Alum Calcined), Qingfen (Calomel), Huangbai (Phellodendron), Huanglian (Coptis) powder. Infant胎Chuang (Fetal Sore): red cake sore on head, wash with Sheng Ai (Mugwort), Baizhi (Angelica Dahurica), Dafupi (Areca Peel), Congbai (Scallion White) decoction, dry, apply Sheng Lan Ye (Isatis Leaf), Sheng Ai Ye (Mugwort Leaf) mashed with honey; treats malignant sores too. Jinyin Chuang (Invasive Sore) starts small, itches then pains, weeping invades moistens flesh, spreads bodywide. Mouth-to-limbs is mild; limbs-to-mouth is severe. Use Kudigen (Kudzu Root) dried, charred, powdered, mixed with Pig Fat; if wet, dust dry; wash with Kushen (Sophora) and Dafupi (Areca Peel) decoction first. Bai She Chan Chuang (White Snake Wrap Sore) has head/tail like snake. Onset: moxa through Garlic at 7 cun; apply Xionghuang (Realgar) powder mixed with vinegar; drink vinegar-mixed preparation. Or Wanbing Jiedu Dan (Ten-Thousand Disease Detoxifying Pill), Lafan Wan (Wax-Alum Pill), external/internal.
Soup/Fire Burns, Fatty Warts, Lacquer Thorns. Soup/Fire burns: endure pain initially, urgently roast near fire, do NOT use cold compresses to trap heat rotting into bone/sinew. Later use Hanshuishi (Calcite) 7 liang, Huangbai (Phellodendron), Huanglian (Coptis), Huangqin (Scutellaria), Shanzhi (Gardenia), Dahuang (Rhubarb), Chishizhi (Red Halloysite) each 1 liang, severe cases add Bingpian (Borneol)少许, powdered, mixed with Wine or Duck Egg White for application; Zhenwang Dan (Array King Pill) also good. Infant fatty sore: Songxiang (Rosin) powdered, rolled in paper, ignited in Sesame Oil, drip oil to apply; or Pig Trotter ash mixed with Sesame Oil. Wart sore: like fish-scale nevus or Thousand-Day Sore, often on hands/feet, also called Regret Sore. Moxa early stage for miraculous shedding of others. Lacquer sore: poisoning from raw lacquer, facial itch/swelling, eye redness, scratching raises [Zhen] (Rash), severe cases body-wide bean/almond sized pustules painful. Apply raw Crab Yellow broadly; or Lacquer Tea powdered with Sesame Oil; or Willow Leaf (Winter bark) decoction wash. Bamboo/Wood splinters stuck: apply Single Glutinous Rice Paste, or Head Grease, or Qicao (Caterpillar) mashed paste; or Ivory powder dusted.
Fractures: Ask about bleeding first. Fractures damage body via falls/hits/crushes/sprains, Qi/Blood stagnation without skin break, or metal blade wounds with bleeding. External bone/tendon injury treatable; internal organ/membrane damage or injury to Yin organs/ears incurable. Unbleeding: attack; Bleeding: defend. Unbleeding: use Sumu (Dalbergia) to remove stasis, Huanglian (Coptis) to descend fire, Baizhu (Atractylodes) to harmonize middle, decocted with Child Urine and Wine. Upper lesions: Leek juice with porridge; lower lesions: purge. Cold blood coagulates, do not drink cold water or blood enters Heart causing death. Remove stasis: Nanming San (Hard Cry Powder), Huaruishi San (Orpiment Stone Powder). Smooth Qi: Muxiang Yunqi San (Aucklandia Smoothing Qi Powder) plus Child Urine, Hongqu (Red Yeast Rice) or Red Wine. Bleeding: urgently use Zhenwang Dan (Array King Pill) to stop bleeding, first take tonifying/supporting herbs, then remove stasis; severe deficiency avoids purging. Blood deficiency: Siwu Tang (Four Substance Decoction) plus Chuanshanjia (Pangolin Scale); Qi deficiency: Sumu (Dalbergia), Renshen (Ginseng), [Huangqi (Astragalus)], Danggui (Angelica Sinensis), Chenpi (Tangerine Peel), Gancao (Licorice) for half month, until pulse gathers, then take Ziran Tong (Natural Copper) powder with decoction on empty stomach. Contraindicated if bone not fractured. Severe constitutional deficiency: Ziheche Dan (Placenta Pill) minus Shexiang (Musk). Injury treatment excels in tonifying Qi/Blood; if cold-exposed, first warm.
Abdominal/Hypochondriac distension/pain with chills/fever. Fracture theory focuses on Blood, unlike Six Evils/Seven Emotions having Qi/Blood distinctions. Liver governs Blood; regardless of meridian injured, evil blood returns to Liver, flows to hypochondrium, stagnates in abdomen causing distension/pain, or chills/fever. Excess: purge; Deficiency: Dangguixu San (Angelica Tail Powder), Fuyuan Huoxue Tang (Restore Origin Invigorating Blood Decoction), or Shiquan Dabu Tang (Comprehensive Great Tonifying Decoction) plus Xiangfu (Cyperus), Chenpi (Tangerine Peel), Beimu (Fritillaria) equal parts decocted. Vomit/Blood exit door: most dreaded. If wound suddenly dries, toxin attacks abdomen, mental confusion/oppression, vomiting, or excessive bleeding with continuous hematemesis, difficult to treat. Initial vomiting: Pingwei San (Peace the Stomach Powder) internally, Ginger Juice externally. Broken Wind edema also suitable. Initial hematemesis: Sumu (Dalbergia) decoction mixing Gu Wufu Tang (Ancient Aconitus Decoction) or Gu Bangshuang San (Ancient Clam Frost Powder). If evil blood enters intestines/stomach, passing turbid bloody stool: Baicaoshuang (Herb Ash) powder with Wine. External kidney injury with hematuria/obstruction: Wuling San (Five Ling Powder).
