After the malaria syndrome has manifested two or three times, this formula can be used to arrest it, often yielding immediate results.
*Changshan* (Fire-wine fried) [Dichroae Radix] *Caoguoren* (Shelled) [Tsaoko Fructus] *Banxiaqu* (Ginger-juice fried) [Pinelliae Rhizoma Praeparatum] *Xiangfum* (Wine-fried) [Cyperi Rhizoma] *Qingpi* (Albedo removed, Vinegar-fried), each four taels; *Zhen Liushenqu* (True Six-God Qu) twelve taels.
Grind into powder, use rice drink to boil and make paste for pills, size of a bullet, coated with *Zhusha* (Cinnabar). For mild cases, one pill; for severe cases, two pills. Take with decoction of five or six red dates, swallowed in the morning on an empty stomach, facing east.
*Liu Junzi Tang* (Six Gentlemen Decoction), *Buzhong Yiqi Tang* (Tonify the Middle and Augment the Qi Decoction) (Both seen under the category of Stroke).
When malaria persists and transforms into deficiency, the aforementioned two formulas should be primarily used. However, true deficiency cases often involve cold pathogens, requiring the addition of herbs such as *Rougui* (Cinnamon Bark), *Fuzi* (Prepared Aconite Root), *Paojiang* (Baked Ginger), and *Sharen* (Amomum Fruit) to warm and tonify.
Question: How to differentiate between edema (*Shuizhong*) and distension/tympanites (*Guzhang*)? Answer: If swelling starts in the eye sockets and feet, followed by abdominal enlargement, it is water retention (edema); if abdominal enlargement occurs first, followed by swelling of the four limbs, it is distension (tympanites). However, edema may also present with accompanying distension, and distension may also present with accompanying water retention. Treatment must be determined based on the sequence of onset and the predominance of symptoms. Now, they are divided into two separate categories.
Edema syndromes are differentiated by exterior/interior, cold/heat, and Kidney/Stomach involvement. Generally, swelling of the four limbs without abdominal swelling indicates an exterior syndrome. Swelling of both the four limbs and the abdomen indicates an interior syndrome. Restlessness, thirst, dry mouth, dark urine, constipation, and preference for cool foods indicate involvement of the Yangming channel, representing a heat syndrome. Absence of restlessness and thirst, normal bowel movements, and preference for hot foods indicate Yin-water, representing a cold syndrome. Wheezing preceding swelling indicates water accumulation in the Kidney channel. Swelling preceding wheezing, or swelling without wheezing, indicates water retention in the Stomach channel. The Classics state: "The Kidney is the gate of the Stomach." When the gate closes, water accumulates; however, Stomach disease can also cause the gate to close autonomously. For treating Stomach-related issues, modify *Wujiapi Yin* (Acanthopanacis Cortex Drink). For treating Kidney-related issues, modify *Shenqi Wan* (Kidney Qi Pill). One might ask: Books state that "wheezing preceding swelling indicates lung pathology," why is this so? The answer is: Although wheezing involves the Lung, its root lies in the Kidney. The Classics state: "All flaccidity disorders, wheezing, and vomiting belong to the lower jiao." If wheezing is caused by external contraction, it may specifically relate to pathogenic invasion of the Lung channel; if caused by internal injury, it invariably stems from the Kidney. Practitioners should examine this carefully.
