虚寒腰痛，内用∶
八角茴香
研末，酒服下。
外以糯米炒热袋盛之，熨痛处。
麻黄（去节） 甘草（炙）各二两 独活 防风 桂心 栝蒌 干葛各三两 芍药四
两干地黄五两 生姜六两
上以水八升，酒二升，煎取三升，分三服。不瘥重作。
诃黎勒（炮去核）四颗 人参二分
上药捣末，以牛乳二升煮三、四沸，顿服之。分为二服亦得。
胁下偏痛发热，其脉紧弦，此寒也。当以温药下之，方用∶
大黄三两 细辛二两 附子（炮）三枚
上以水五升，煮取二升，分三服。若强盛人煮取三升半分为三服。服则如人行四、五里，

鲜地骨皮 生姜各四两
捣成泥，绢包囊上，虽极痒宜忍之。并以∶
连蒂老丝瓜（烧存性）
研末，每服三钱，热酒下。重者不过二、三服，即愈。
痛处如火，溲赤便艰，口干畏热，此热疝也。以∶
芙蓉叶 黄柏各三钱
为末，木鳖子磨醋调涂囊上，极效。
绕脐苦痛，发时则白汗出，手足厥冷，脉沉弦，此寒疝也。治用∶
大乌头十五枚 白蜜二斤
先以水三升煮乌头，取二升，去乌头。纳蜜煎令水气尽，得二升。强人服七合，弱人五
合。一服不瘥，明日更服，日止一服，不可再也。
病发时心部似被锥刀所刺，或四肢逆冷，或唇口变青。其原由阴气积于内，寒气不散，
上冲于心，遂致心痛，故名心疝。治用∶
芍药 桔梗 细辛 蜀椒 桂心 干姜各三分 附子一分
上末之，蜜和丸如梧子。服七丸，以酒下，日二服。

胁下偏侧疼痛且发热，脉象紧弦，这是寒邪所致。应当用温性药物攻下，方剂如下：
大黄三两 细辛二两 附子（炮制）三枚
以上药物用水五升，煮取二升药液，分三次服用。若是体质强壮的人，煮取三升半，分三次服用。服药后约行走四五里的时间，再进食
鲜地骨皮 生姜各四两
捣成泥状，用绢布包裹敷在患处，即使极痒也要忍耐。并配合使用：
带蒂老丝瓜（烧存性）
研成粉末，每次服用三钱，用热酒送下。病情重者不超过二、三次，即可痊愈。
疼痛部位如火灼，小便赤涩，大便困难，口干怕热，这是热疝。使用以下方剂：
芙蓉叶 黄柏各三钱
研成粉末，用木鳖子磨醋调匀涂抹于阴囊上，效果极佳。
围绕肚脐剧烈疼痛，发作时出冷汗，手足冰冷，脉象沉弦，这是寒疝。治疗使用以下方剂：
大乌头十五枚 白蜜二斤
先用水三升煮乌头，取二升药液，去掉乌头。加入蜂蜜煎熬至水分蒸发殆尽，得到二升药膏。体质强壮者服用七合，体弱者服用五合。一次服用后未痊愈，第二天继续服用，每天只服一次，不可再次服用。
发病时心部感觉像被锥刀刺扎，或四肢逆冷，或嘴唇口角变青。其病因是阴气积聚在内，寒气不散，向上冲犯心脏，导致心痛，所以名为心疝。治疗使用以下方剂：
芍药 桔梗 细辛 蜀椒 桂心 干姜各三分 附子一分
以上药物研成粉末，用蜂蜜调和制成梧桐子大小的丸药。每次服用七丸，用酒送下，每天服用两次。
For lumbar pain due to deficiency-cold, internal use:
Foeniculum Vulgare Fruit (Bajiao Huixiang)
Grind into powder, take with wine.
External use: Stir-fry glutinous rice until hot, place in a bag, and apply as a compress to the painful area.
Ephedrae Herba (Mahuang, removed nodes), Glycyrrhizae Radix et Rhizoma (Gancao, honey-fried) each two liang; Angelicae Pubescentis Radix (Duhuo), Saposhnikoviae Radix (Fangfeng), Cinnamomi Cortex (Guixin), Trichosanthis Radix (Gualou), Puerariae Radix (Gange), each three liang; Paeoniae Radix Alba (Shaoyao) four liang; Rehmanniae Radix Secunda Preparata (Sheng Dihuang) five liang; Zingiberis Rhizoma Recens (Shengjiang) six liang.
Take the above herbs, add eight sheng of water and two sheng of wine, decoct to obtain three sheng, divide into three doses. If not cured, prepare again.
Aconiti Lateralis Radix Preparata (Sheng Fuzi) [Note: Text says Sheng but context implies processed for external use or specific preparation]
Grind into powder, mix with water, apply to both soles (Yongquan acupoint area), immediate effect.
Terminalia Chebula Fruit (Helilei, roasted, pitted) four pieces; Ginseng Radix et Rhizoma (Renshen) two fen.
Pound the above herbs into powder, boil in two sheng of cow's milk for three to four boils, take all at once. Dividing into two doses is also acceptable.
Pain on one side of the hypochondrium accompanied by fever, pulse tight and string-taut, this is cold. It should be treated with warm purgatives. Formula:
Rhei Radix et Rhizoma (Dahuang) three liang; Asari Radix et Rhizoma (Xixin) two liang; Aconiti Lateralis Radix Preparata (Fuzi, roasted) three pieces.
Take the above herbs, add five sheng of water, decoct to obtain two sheng, divide into three doses. For robust individuals, decoct to obtain three and a half sheng, divide into three doses. After taking the medicine, wait for the time it takes to walk four or five li, then eat.
Fresh Lycii Cortex (Di Gu Pi), Zingiberis Rhizoma Recens (Sheng Jiang) each four liang.
Pound into paste, wrap in silk cloth, apply to the affected area. Endure even if extremely itchy. Also use:
Luffae Fructus Retinervus (Laosigua, with pedicel, carbonized):
Grind into powder, take three qian per dose, wash down with hot wine. For severe cases, no more than two or three doses are needed for cure.
Painful area feels like fire burning, dark urine, difficult defecation, dry mouth, aversion to heat, this is heat hernia. Use:
Hibisci Mutabilis Folium (Furong Ye), Phellodendri Cortex (Huangbai) each three qian.
Grind into powder, grind Momordicae Semen (Mubiezi) in vinegar to make a paste, apply to the scrotum, extremely effective.
Severe pain around the umbilicus, sweating cold sweat during attacks, cold limbs, pulse deep and string-taut, this is cold hernia. Treatment uses:
Aconiti Radix (Wutou) fifteen pieces; Mel (Baimi) two jin.
First, boil Wutou in three sheng of water, obtain two sheng, remove Wutou. Add honey and simmer until moisture evaporates completely, obtaining two sheng. Robust individuals take seven he, weak individuals take five he. If one dose does not cure, take again the next day. Only one dose per day, do not repeat excessively.
During an attack, the heart region feels as if pierced by awls or knives, or limbs are cold, or lips and mouth turn blue. The origin is accumulation of yin qi internally, cold qi failing to dissipate, rushing upward to the heart, thus causing heart pain, hence named Heart Hernia. Treatment uses:
Paeoniae Radix Alba (Shaoyao), Platycodonis Radix (Jiegeng), Asari Radix et Rhizoma (Xixin), Zanthoxyli Pericarpium (Shujiao), Cinnamomi Cortex (Guixin), Zingiberis Rhizoma (Ganjiang) each three fen; Aconiti Lateralis Radix Preparata (Fuzi) one fen.
Pound the above into powder, mix with honey to form pills the size of wu zi (tree seeds). Take seven pills, wash down with wine, twice daily.
