Lymphatic tuberculosis (Luoli) must originate from the Shaoyang meridian. If taboos are not observed, it spreads to the Yangming meridian. Generally speaking, rich flavors in diet and accumulated stagnant qi constitute poison, wind, and heat. These three factors induce pathological changes; one must distinguish between excess and deficiency patterns. Excess patterns are easy to treat, while deficiency patterns can be tonified. This is because the Sanjiao (Triple Burner) features abundant fire and relatively scarce blood and qi. When women present with this condition, menstrual irregularities occur, accompanied by alternating chills and fever. If prolonged, it transforms into tidal fever, which is critical. Unless one practices sexual restraint and avoids rich foods, even a divine physician cannot cure it.
Luoli refers to tubercles. They may appear behind the ear, in front of the ear, below the ear, extending to the mandible and neck; or below the neck, extending to the supraclavicular fossa (Quepen). All these are called Luoli. Those on the chest and hypochondrium are called Machdao (a type of tubercle growing along the chest and ribs), representing diseases of the Hand-Shaoyang and Foot-Shaoyang meridians. Shaoyang governs ministerial fire. In women and children, if the Sanjiao fire is obstructed, and anger, worry, thought, fear, and anxiety accumulate, causing qi depression and rebellion, phlegm-fire accumulates in the meridians. Tubercles fail to dissipate, resulting in Luoli. Symptoms include fever and aversion to cold, restlessness, thirst, night sweats, alternating chills and fever, progressive emaciation, and increasing pathogenic heat. Such cases require careful management; how can one not be cautious?
It is also said: Continuous tubercles are called Luoli. Tubercles shaped like a knife blade, long as a clam shell, are called Machdao.
If the pulse is choppy (Se), treatment should focus on nourishing blood.
If the pulse is weak (Ruo), treatment should focus on tonifying qi.
If the pulse is floating and rapid (Fu Shu) with swelling and pain, dispel wind, clear heat, and cool blood.
If swollen and hard without ulceration, tonify qi and blood.
If caused by emotional depression, resolve stagnation and regulate qi and blood.
If non-healing after ulceration, it belongs to joint deficiency of qi and blood; strongly tonify.
If caused by consumptive disease (Xu Lao), tonify.
If there are nodules that do not heal, remove necrotic tissue first, then tonify.
If the pulse is excess (Shi) and healing/dissipation does not occur, use purgation.
To treat Luoli tubercles and various swellings/sores on the head and face, and swelling/pain in the chest and hypochondrium, this method must be applied initially.
The Classics state: "When fire pathogen prevails, treat with salty-cold methods." First, stir-fry one to two taels of salt, decoct in one large bowl of flowing water, let cool to warm, and drink in three to four doses. Shortly after, induce vomiting of saliva/phlegm (three sheng) using a chicken feather to probe the throat. Then administer formulas such as Yuzhu San (Jade Candle Powder) to cool blood and unblock meridians, Siwu Liangge San (Four Substances Cooling Diaphragm Powder), or Jiedu Tang (Detoxification Decoction).
Didan (Euphorbia kansui root) and Banmao (Mylabris) processed according to regulations can attack the toxicity of Luoli because they break the root of the tubercles, which are expelled via urine, appearing as powder-like flakes, paste, blood clots, or rotting flesh. This serves as verification. When taking these medicines, use Mutong (Akebia Caulis) and Huashi (Talcum) as guiding herbs; otherwise, toxic residue may remain in the Small Intestine and Bladder.
Xiakucao (Prunella Vulgaris) is stated in Ben Cao to effectively treat Luoli, dispersing stagnant qi, and benefiting the Jueyin blood vessels. Although classics do not mention its ability to relieve chills and fever, it is reliable for deficiency patterns. For excess patterns, assist with dispersing herbs, apply external moxibustion, and gradual improvement will occur. This herb blooms purple flowers in March-April and withers at Summer Solstice, implying it possesses pure yang qi and withers when receiving prosperity (yang). It is a key herb for Luoli.
Take two dou of very ripe, black mulberries (Sangshen), filter through cloth to obtain natural juice, and slowly simmer in a clay vessel over high heat until a thin paste forms. Take one spoonful daily dissolved in plain boiled water, taken well after meals, three times a day.
