论曰久痔者，以脏腑夙有风冷，加之饥饱不常，将摄乖宜，或缘忧思恚怒，致阴阳不和
，气血凝滞，故风毒乘虚，时作时歇，攻注肛肠，痔孔有脓与血间下，肿痒疼闷，故谓之久
痔。
治五痔 血日久，众药不瘥者。雷丸丸方
雷丸（三两） 紫芝 白芷 紫菀（各二两） 贯众（五两） 秦艽（去苗土） 浓朴

上二味细研，用乌鸡子一枚，打开去黄，以清拌药，再入鸡子壳中，以纸封，饭甑中蒸
熟。空心服尽一枚，如年深者，服十数枚全安。
治久痔及肠风下血疼痛。诸药不瘥者。金针散方
皂荚刺（赤红者不拘多少炙）
上一味，捣罗为散，每服三钱匕，水一盏，煎至七分，去滓温服。一方用破故纸打碎，
纸上炒，与皂荚刺等分为散，每服三钱匕，温酒调下。
治年深痔疾不瘥。独圣散方
黄土（如石者半两烧令紫色细研为末）
上一味，以腻粉一钱匕，药末二钱半拌匀，临卧温酒调下，虫出是效。
治久痔。比金散方
蜀葵叶（夏月收者焙干）
上一味，捣罗为散，每服二钱匕，温酒调下，五服效。
治肠风痔 ，一二十年不瘥，面色萎黄，饮食无味，及患脏腑伤积泄泻。暑月常泻不止
，及诸般淋沥。久患消渴，妇人月候不调，赤白带下，多年不瘥。凡是脏腑诸疾皆主之。石
燕散方
石燕（不拘多少净洗）
上一味，每日空心取一枚，于坚钵内，温水磨尽，分三服，晚食前再服，欲作散。捣研
为细末水飞。每服半钱至一钱匕，清饭饮调下，温水亦得，年久肠风等疾，须常服及一月，
勿歇即愈。
治冷痔疮久不瘥者，追风毒，去疼痛。天雄膏方
天雄（去皮脐一枚） 天南星（一枚） 天麻（半两） 丹砂（研一钱） 黄蜡（半两）
上五味，并生为末，先用生油少许，熔黄蜡，次入诸药，熬成膏，每使时，用旧帛摊药
贴疮甚验。
治痔 久不瘥者。熏痔立效方
蛇蜕（四两细剪令碎） 蝉蜕（四两细剪令碎） 白矾（生研一两） 皂荚（二梃为末）
上四味拌匀，分为六帖，每用一帖，瓷碗内如烧香法，盛入桶内，烧令烟，就上坐熏之
，烟尽即止。
治痔 久不瘥者。熏痔必效散方

上四味，捣研为末拌匀，每用三五钱匕，如烧香法安盆器内，用板盖上，开窍坐，就疮

蜂窠（烧灰秤半两研） 蚰蜒（一个大者研作膏） 水银粉（秤一钱研） 铅丹（秤一

上四味，将三味研者，和入膏内，更入麝香半钱匕，同研匀，每用少许，敷疮上。
治痔 久不瘥，除疮口。黑纸拈方
密陀僧（煨） 黄连（去须） 沥青（等分）
上三味，捣罗为末，用纸作拈子，以津唾蘸药末，拈入窍内，觉微微痒即住，不可尽拈
也。
治久痔不瘥。熏痔方
臭椿皮（曝干二两） 枳壳（去瓤三两） 蛇蜕（半两）
上三味，粗捣筛，用一小罐子盛，以浓纸一张，盖口系定，上剪一小眼子，文武火煨热
，气从眼子内出，便骑罐子熏之，如熏得热痛，以灯盏内油涂，如此熏三五度，立效。
治久患痔疮，疼痛不可忍。乌金散方
乌驴乳（屋上尘煤是也细研） 陈腊茶末（各一分） 腻粉（一字）
上三味，同研细敷痔上，干者以油调涂之，一两上即销。
治远年痔疾，有脓血出，不可行坐。麝香散方
麝香（半钱研） 田螺（一个烧灰研）
上二味，细研为末，先用葱汤洗，次用药敷之。
治痔 久不愈。 藤散方
藤子不以多少
上一味为散，先以蜜调少许，涂痔 疮上，次用温酒调下一钱匕，食前服。

安息香（一分）、阿魏（半分）、乳香（一钱，这三味药一处研磨）、丹砂（一分）、雄黄、龙脑麝香、巴豆（三粒，上一十味药），先将安息香等三味药放入瓷器内，用重汤煮，或者在饭甑上蒸一次，再放入干净的钵中，烂研成膏状，加入其他药物一起揉和，制成绿豆大小的丸剂。每次服用一丸，空腹服用。如果是肠风泻血、五痔漏脓血不止，或者生鼠乳（痔疮的一种），都用好茶送服；如果是突发心痛，用生姜汤送服；如果是食积陈曲引起的不适，用陈曲汤送服；如果是心腹各种气痛，用温酒送服；如果是妇人心痛、血气病，用当归酒送服；如果是水泻、冷水引起的各种痢疾，用饭饮送服；如果是疟疾，用桃心汤送服。
治疗十年以上的痔疮如鼠乳状，脓出下血严重的情况。白蔹散方：
白蔹（二两）、赤小豆（一两）、黄芪（锉碎二两）、芍药（三两）、黄芩（去黑心一两）、桂枝（去粗皮，上八味药），捣碎过筛成散剂。每次服用二钱匕，空腹用温酒调服，晚上再服一次，以痊愈为度。
治疗五痔连年不止，兼有痔漏的情况。龙骨丸方：
龙骨、螺蛳壳（炙令焦，各一两）、黄芪（锉碎）、当归（切焙）、枳壳（去瓤麸炒）、干姜（炮，各三钱，上八味药），捣碎过筛成粉末，用炼蜜制成梧桐子大小的丸剂。空腹用黄芪汤送服四十丸，晚上再服一次。
治疗五痔十年不愈。槐子丸方：
槐实（微炒一斤）、龙骨（三两）、白矾（烧成灰）、硫黄（研，各五两）、沙糖（十两）、大黄（锉碎，上七味药），除了槐子和沙糖外，其余捣碎过筛成粉末。先将糖和槐子放在瓷器中，在饭甑上蒸一顿饭的时间，用绵布绞取汁液，与前面的药末混合，制成梧桐子大小的丸剂，阴干。每次服用二十丸，空腹用温水送服，晚上再服一次。
治疗数十年五痔，下血如鸡肝，肛边结核如鼠乳，肛中疼痛的情况。大黄汤方：
大黄（锉炒五两）、滑石（三两半）、芒硝（研）、桑根白皮（锉）、黄芩（去黑心）、杏仁（汤浸去皮尖双仁，炒，上六味药），粗捣筛。每次服用六钱匕。用酒一盏、水一盏、枣二枚劈破，同煎至一盏，去渣，空腹温服，微微腹泻一两行为度，未腹泻则再服。

[Discussion] The so-called chronic hemorrhoids (Jiu Zhi) arise because the internal organs have long harbored wind-cold pathogens. This is compounded by irregular eating habits, improper care, or emotional distress such as worry and anger, leading to disharmony of Yin and Yang and stagnation of Qi and Blood. Consequently, wind-toxins take advantage of the deficiency, causing symptoms that flare up and subside intermittently. These toxins attack and penetrate the anal canal, resulting in pus and blood discharging from the hemorrhoidal openings, accompanied by swelling, itching, pain, and a sensation of fullness or oppression. Hence, it is termed chronic hemorrhoids.
For treating five types of hemorrhoids with prolonged bleeding, where various medications have failed to cure: Lei Wan Pill Formula.
Lei Wan (San Liang), Zi Zhi, Bai Zhi, Zi Wan (each two Liang), Guan Zhong (Wu Liang), Qin Jiao (remove seedlings and soil), Hou Po (remove rough bark) (each half Liang), Zhu Xuan Chang. The above twelve ingredients are crushed, sifted into powder, mixed with refined mutton fat, and formed into pills the size of sophora seeds. Take twenty pills each time, decocted in rice water on an empty stomach, and taken again in the evening.
For treating long-standing intestinal wind hemorrhoids and various other diseases: An Xi Xiang Wan Pill Formula.
An Xi Xiang (Yinxiang, one Fen), A Wei (Asafoetida, half Fen), Ru Xiang (Frankincense, one Qian; these three ingredients are ground together), Dan Sha (Cinnabar, one Fen), Xiong Huang (Realgar), Long Nao She Xiang (Borneol and Musk), Ba Dou (Croton Seed, three grains). The above ten ingredients: First, place An Xi Xiang and the other two ingredients in a porcelain vessel, boil in a double boiler, or steam over a rice steamer once. Then transfer to a clean mortar, grind into a paste, mix with the remaining drugs, and form pills the size of mung beans. Take one pill on an empty stomach. For intestinal wind with bloody diarrhea, unceasing pus and blood discharge from the five hemorrhoids, or formation of rat-like nodules (Shu Ru), take with good tea. For sudden heart pain, take with ginger soup. For food stagnation from aged yeast, take with Chen Qu soup. For various Qi pains in the heart and abdomen, take with warm wine. For women's heart pain and Blood-Qi disorders, take with Dang Gui wine. For watery diarrhea and various dysenteries caused by cold water, take with rice drink. For malaria, take with Tao Xin (peach heart) soup.
For treating hemorrhoids lasting ten years, resembling rat-like nodules, with severe pus discharge and bleeding: Bai Lian Powder Formula.
Bai Lian (Two Liang), Chi Xiao Dou (One Liang), Huang Qi (Astragalus Root, cut into pieces, Two Liang), Shao Yao (Peony Root, Three Liang), Huang Qin (Scutellaria Root, remove black heart, One Liang), Gui Zhi (Cinnamon Twig, remove bark, [weight implied]). The above eight ingredients are crushed and sifted into powder. Take two Qian Bi each time, mixed with warm wine on an empty stomach, and taken again in the evening. Continue until cured.
For treating the five hemorrhoids persisting for years, accompanied by fistulas: Long Gu Pill Formula.
Long Gu (Dragon Bone), Luo Pi (Snail Shell, calcined until scorched, each One Liang), Huang Qi (Astragalus Root, cut), Dang Gui (Angelica Sinensis, sliced and dried), Zhi Ke (Aurantium Fruit, remove pith and stir-fried with bran), Gan Jiang (Dried Ginger, processed, each three Qian). The above eight ingredients are crushed and sifted into powder, mixed with refined honey, and formed into pills the size of sophora seeds. Take forty pills on an empty stomach with Huang Qi decoction, and taken again in the evening.
For treating the five hemorrhoids not curing after ten years: Huai Zi Pill Formula.
