Discourse states: In the context of Cold Damage (Shanghan) disease, cardiac and abdominal distension and fullness arise because the Qi of the Zang organs is disharmonious, allowing pathogenic factors to invade and take advantage of the deficiency. The struggle between healthy Qi and pathogenic factors leads to a sensation of oppression, distension, and fullness in the chest and abdomen. However, Zang organ conditions can be either Deficiency (Xu) or Excess (Shi), and pathogenic factors can be either Cold (Han) or Heat (Re). If, after vomiting or purging, the disease persists with internal and external heat signs, accompanied by cardiac and abdominal distension, fullness, and pain, this constitutes an Excess pattern. Conversely, if the patient has a pre-existing cold accumulation (cold phlegm or fluid retention), develops fever due to illness, takes cold-natured medicines, or drinks excessive water, causing fluid retention to bind in the epigastrium (Xiaxin) and agitate existing chronic stagnation, resulting in cardiac and abdominal distension and fullness, this constitutes a Deficiency pattern.
