This fistula occurs outside the anus, penetrating through at a distance of one cun, forming two holes that are opposite to each other left and right with precise alignment. Sometimes fluid drains from the left hole while the right hole is closed; sometimes fluid drains from the right hole while the left hole is closed. If subjected to physical exertion or fatigue, pus will discharge from the fistula openings. The treatment method involves externally inserting medicated threads, fumigating and washing to remove hard crusts (luojian), internally administering Zhongjiu Wan (Nine-Cent Pill), Jin Gou Wan (Golden Centipede Pill), and Huaijiao Wan (Huaijiao Pill), and finally applying Jiedu Gao (Detoxification Plaster) to conclude the treatment.
This condition initially presents as a single fistula. After several years, it spreads to the groin area, manifesting in varying numbers such as three, five, six, or seven. Initially, there is pain and pus discharge from one hole, which subsequently extends to involve multiple holes discharging pus. Hence, it is also known as "Gua Teng Lou" (Melon Vine Fistula). There is a hard scar at the fistula opening, resembling a pulled vine. Treatment must begin with the initial hole, followed by sequentially using threads to remove the hard crusts. Once the crusts are completely removed, wash the area with Xunxi Tang (Fumigation and Washing Decoction), then apply Jiawei Tianran San (Modified Natural Powder) to conclude. Internally administer Zhongjiu Wan concurrently with Huaijiao Wan.
This fistula resembles Quchi Zhi (Right-Angle Hemorrhoid) in shape, looking like a cow's nostril, hence its name. The treatment method is identical to that of Quchi Zhi.
This condition occurs around the anus, frequently discharging foul-smelling fluid with mild pain. The treatment involves internally administering Zhongjiu Wan and externally applying Qianchui Zhi (Thousand-Beaten Paper); the foul-smelling fluid will cease spontaneously.
This condition occurs above and below the bone at the Guwei acupoint, with redness, swelling, and pain affecting a segment of the lower body and the spinal column. Symptoms include tidal fever and heavy sensation in the body. Over a period of three to four days or six to seven days, copious pus continues to discharge from the fistula. During treatment, pressing on the red and swollen area at Guwei reveals an external hardness but internal emptiness, with fluctuation indicating pus accumulation. Apply Huarou Gao (Flesh-Melting Plaster) to this area. When the tissue turns black, puncture it with a needle to allow pus to drain. After draining the pus, insert medicated threads into the cavity three times to promote the drying of pus. Internally administer Zhongjiu Wan and Sanxiang Wan (Three-Fragrance Pill) for recovery.
Preparation Method for Drying-Pus Small Threads:
Take one Wu Beizi (Galla Chinensis) with a natural round cavity. Drill a small hole at one end. Grind 5 qian of Ming Fan (Alumen) and 1 qian of Bai Pi (Realgar) into powder. Fill the powder into the cavity of the Wu Beizi. Wrap it in several layers of paper, moisten it, and bury it in charcoal ash to roast (wei). When dry, remove the Wu Beizi, roast it again until scorched, and grind it into powder. Mix with rice flour paste to form strips, then dry them for future use.
