At the initial stage, if there is slight aversion to cold and fever, normal diet, a localized swelling that is slightly red, and pain that does not adhere to the bone, it is a favorable condition (Shun). If the condition progresses with free movement, the swelling forms a sac-like structure, pain occurs at specific times, and pus forms easily, it is an auspicious sign (Ji). After ulceration, if the pus is thick, swelling subsides, pain decreases, the body feels light, consciousness is clear, sleep is stable, and there is no fever, it is auspicious. If after ulceration, vital energy (Yuan Qi) recovers easily, diet improves, flesh becomes firm, and pus dries up easily, it is auspicious.
At the initial stage, if there is aversion to cold and fever, diffuse swelling with white color, limb stiffness and tension, and pain adhering to the bone, it is a dangerous condition (Xian). If the condition progresses with difficulty in movement, diffuse swelling of the entire leg, absence of heat and redness, and failure to form pus, it is dangerous. After ulceration, if the pus is thin and watery, has a foul and fishy odor, swelling and pain do not subside, and the body emaciates day by day, it is a fatal condition (Si). If after ulceration, the spleen and stomach are weak, food lacks taste, thirst is unquenchable, lips are pale, and skin is dry, it is fatal.
At the initial stage, if there is fever and aversion to cold, bodily stiffness, swelling and pain in the legs and feet, and a floating and tight pulse, dispersing methods should be used. If the condition progresses with swelling and pain in the legs and feet, unchanged skin color, and diffuse swelling from top to bottom, cold-dispersing and meridian-warming methods should be applied. If swelling is caused by alternating cold and heat, with white and bright color, and indentation remains after pressing (like mud), spleen-fortifying and dampness-draining methods are suitable. If the body has no heat but aversion to cold, with a slow and choppy pulse, and swollen but cool legs, blood-nourishing and meridian-warming methods are indicated. If summer-heat pathogen invades the Three Yin meridians, with a surging and rapid pulse, swollen legs and feet, dry mouth, and dry stool, purgative methods should be used. After ulceration, if pus is thin and watery, diet is reduced, and the body emaciates, middle-jiao tonification and spleen-stomach strengthening are appropriate. If after ulceration, swelling and pain persist, pus flows continuously, and deficiency heat does not recede, warming the middle and nourishing qi and blood are required. After recovery, if tendons and bones are stiff with limited flexion and extension, nourishing qi and blood and promoting joint mobility are suitable.
A man suffered from incomplete dispersion of cold pathogen, resulting in thigh swelling and pain, with a floating, rapid, and forceless pulse. This indicates residual wind-cold and internal deficiency. Wuji San (Five Accumulations Powder) without Mahuang (Ephedrae Herba) plus Niuxi (Achyranthis Bidentatae Radix) was administered for two doses, reducing pain slightly; then, Neituo Huangqi Decoction brewed with wine was given for four doses, resolving all swelling. However, a localized swelling remained on the lateral side of the leg, indicating stagnant blood about to suppurate. Internal-promoting medicine was used until pus matured, followed by needle drainage, and finally Shiquan Dabu Tang (Decoction for Complete Great Tonification) was used to heal the wound.
A man suffered from leg pain triggered by labor and cold exposure. Each episode involved aversion to cold, fever, vomiting, chest fullness, and anorexia. This was due to anger and irregular diet damaging the spleen and stomach, leading to spleen qi deficiency. Consequently, dietary essence failed to transform phlegm and stagnation, causing damp-heat in the Middle Jiao to flow downward into the lower limbs, resulting in Hong Kong Foot (Gouty Arthritis/Chronic Ulcerative Leg Condition). Kaie Daoyin Wan (Pill for Opening Knots and Guiding Flow) was taken for two doses to expel retained phlegm; Banxia Zuojing Tang (Banxia Meridian-Assisting Decoction) was taken for several doses to stop vomiting and clear the diaphragm; finally, Fuzi Liuwu Tang (Fuzi Six-Ingredient Decoction) was taken for several doses, eliminating symptoms and preventing recurrence.
A sixty-year-old man suffered from kidney deficiency due to excessive sexual activity, leading to tendon spasms, bi-syndrome contraction, weakness in the feet and knees, and inability to walk. External application of Qufeng Zhushi Gao (Wind-Expelling and Dampness-Removing Plaster) combined with internal administration of Sanyin Shengjun Wan (Sanyin Victory Steed Pill) resulted in recovery.
A man suffered from right leg swelling and pain for over two months. I stated: "The pus has formed long ago; why not incise it?" He replied: "Two senior physicians treated me together, both claiming internal resolution without pus. Is this pus real or not? I should ask them again." He added: "There must be no pus." However, the patient was comatose and critically ill. I forcibly incised him, draining approximately one basin of pus. Du Shen Tang (Single Ginseng Decoction) was administered consecutively for two doses, restoring consciousness. Subsequently, Shiquan Dabu Tang was used for conditioning for over a hundred days, leading to stability.
A man experienced genital coldness after intercourse, followed by formless pain in the thigh, worsening at night, and inability to walk. Physicians treated him with cold-dispersing, damp-draining, phlegm-resolving, and pain-relieving herbs, which exacerbated the pain. His pulse was fine, choppy, and forceless, indicating qi and blood insufficiency with cold pathogen invasion. Dafangfeng Tang (Great Saposhnikovia Decoction) was prescribed. Two doses significantly reduced pain; four more doses led to complete recovery.
A man slept facing the wind after sexual intercourse during summer, causing cold pathogen to invade the left leg, resulting in swelling and pain, alternating cold and heat, chest fullness, and anorexia. Bao'an Wanling Dan (Security Myriad Miracles Pill) dissolved in scallion decoction was taken, along with bathing to induce sweating, to disperse cold toxicity in the bone marrow. Later, Dafangfeng Tang without Qianghuo (Notopterygii Rhizoma et Radix) plus Honghua (Carthami Flos) and Pogu Zhi (Cibotii Rhizoma) was used to warm the kidney meridian and promote circulation. Swelling and pain gradually subsided, and blood vessels harmonized. Intermittent use of Sanyin Shengjun Wan for conditioning led to recovery in two months.
A student official suffered from leg pain similar to the previous case after cold exposure post-intercourse. I treated him with herbs to warm the kidney meridian, warm the meridians, and disperse cold. He considered this too slow and consulted an internal physician who prescribed surface-dispersing agents. Although external pathogens were dispersed, internal deficiency ensued. Xiao Chaihu Tang (Minor Bupleurum Decoction) and other medicines were used for conditioning, triggering deficiency heat, dry mouth and throat, and incessant irritability and thirst. Zhimu Shigao Tang (Anemarrhenae and Gypsum Decoction) was used to clear the upper jiao, while Fangfeng (Saposhnikoviae Radix), Mugua (Chaenomelis Fructus), and Weilingxian (Syneilesis Aconitifoliae Herba) were used to attack the lower jiao. Conflicting opinions led to indiscriminate medication, worsening the disease. Upon re-examination, the affected leg was more swollen and shrunken, indicating a fatal prognosis. Since the disease originated from cold invading the kidney meridian, only Fuzi (Aconiti Lateralis Radix Praeparata) and Pogu Zhi could open the joints; only Qianghuo, Fangfeng, and Niuxi could unblock stagnation; only Renshen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), and Zhi Gancao (Prepared Licorice Root) could transport spleen qi to the limbs; only Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Shudi (Rehmanniae Radix Preparata), and Honghua could nourish and invigorate blood. For diseases entering through deficiency, tonify deficiency first, then attack the pathogen. Treating this condition with surface-dispersing and muscle-releasing methods instead of warming and tonifying, applying the treatment for excess wind-cold, inevitably leads to death. The patient indeed died. Only after numerous physicians erred did he believe and take my prescription.
