For any pregnancy where the fetal qi is unstable, the manifestations are not uniform, and the treatments differ. Generally, fetal instability must have a cause, whether it be deficiency (Xu), excess (Shi), cold (Han), or heat (Re), all of which can cause fetal pathology. Removing the pathogenic factor causing the fetal issue is the method for stabilizing the fetus. Therefore, formulas for stabilizing the fetus should not be applied rigidly, nor should one be bound by the number of months of gestation. Instead, treatment must follow the syndrome and the meridians, medicating according to the specific disease; this is the optimal approach. If one insists that Bai Zhu (Atractylodis Rhizoma) and Huang Qin (Scutellariae Radix) are the sacred medicines for stabilizing the fetus, sticking to their use will rarely avoid error!
When fetal instability is due to cold, symptoms may include acid regurgitation, vomiting of sour fluids, nausea, vomiting, abdominal distension and fullness, preference for warmth and aversion to cold, lower body coldness with diarrhea, a pulse that is mostly deep and fine, or the complete absence of fire/heat signs, yet the fetus remains unstable. These all belong to Yang deficiency and cold syndromes. By warming the Middle Jiao (Spleen and Stomach), the fetal qi will naturally stabilize. It is appropriate to use Wen Wei Yin (Stomach-Warming Decoction) or Li Yin Jian (Yin-Regulating Decoction) with modifications as the main treatment. One must also observe the patient's constitutional organ qi to determine the specific condition and prescribe accordingly.
Wen Wei Yin (See section on Blood Deficiency and Menstrual Disorder)
Li Yin Jian (See section on Blood Cold and Delayed Menses)
When fetal instability is due to heat, symptoms will likely include much irritability and fever, thirst or restlessness, turbidity in the upper and lower parts, threatened miscarriage bleeding, red urine, or a slippery and rapid pulse in all six positions. It is appropriate to use Liang Tai Yin (Fetal-Cooling Decoction) or Bao Yin Jian (Yin-Preserving Decoction) as the main treatment. If there is only heat without deficiency, such as Zhi Ke Tang (Immature Bitter Orange Decoction), Yi Mu Wan (Motherwort Pill), or Huang Qin San (Scutellaria Powder) can be selected. Clearing the fire will naturally stabilize the fetus.
Liang Tai Yin (See *New Formulas Eight Arrays · Cause Array*) treats conditions like internal heat of fetal qi causing instability.
Di Huang (Rehmanniae Radix) Shao Yao (Paeoniae Radix Alba) [each 2 qian] Huang Qin (Scutellariae Radix) Dang Gui (Angelicae Sinensis Radix) [each 1-2 qian] Gan Cao (Glycyrrhizae Radix et Rhizoma) [7 fen raw] Zhi Ke (Aurantii Fructus Immaturus) Shi Hu (Dendrobii Caulis) [each 1 qian] Fu Ling (Poria) [1.5 qian]
Use 1.5 zhong of water, decoct to 0.7 zhong, take warm long after meals. If heat is severe, add Huang Bo (Phellodendri Cortex) 1-2 qian.
Bao Yin Jian (See section on Heat in Blood and Early Menses)
Zhi Ke Tang (See *Ancient Formulas of Fumen Gui*) treats threatened miscarriage bleeding, or bleeding caused by activity, also aids digestion and harmonizes the middle, and treats morning sickness (E Zu).
Zhi Ke (Stir-fried) Huang Qin (Scutellariae Radix, honey-fried, each 0.5 liang) Bai Zhu (Atractylodis Rhizoma, stir-fried, 1 liang)
Grind into powder. Take 1 qian per dose, mixed with plain boiled water. If the previous four conditions are due to Spleen and Stomach weakness, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) plus Wu Wei Zi (Schisandrae Fructus) is appropriate. If due to Spleen and Stomach sinking, increase Sheng Ma (Cimicifugae Rhizoma) and Chai Hu (Bupleuri Radix) in the aforementioned formula. If due to afternoon fever and internal heat, Xiao Yao San (Free and Easy Wanderer Powder) is appropriate.
Yi Mu Wan (See external formulas on Heat in Blood discussion)
Huang Qin San (i.e., Zi Qin San. See external formulas on Heat in Blood discussion)
When fetal instability is due to deficiency, it requires the most careful adjustment. However, there are congenital deficiencies and acquired deficiencies; the maintenance of the fetal origin depends entirely on these. Congenital deficiency stems from endowment; one should gradually tonify according to the bias of Yin and Yang, never seeking quick results, aiming for preservation. Acquired deficiency stems from lifestyle factors; excessive sexual activity, fatigue, improper diet, and emotional disturbances can all damage fetal qi. Treating this requires investigating the cause and adjusting based on the illness, while maintaining caution. Ultimately, it always relates to Qi and Blood deficiency, so Tai Yuan Yin (Fetal-Origin Decoction) should be the primary formula. If Heart and Spleen Qi deficiency is predominant above, Xiao Yao Yin (Free and Easy Wanderer Drink), Gui Pi Tang (Restore the Spleen Decoction), or Shou Pi Jian (Longevity Spleen Decoction) are appropriate. If Liver and Kidney insufficiency is predominant below, Zuo Gui Yin (Left Restoring Drink), You Gui Yin (Right Restoring Drink), or Gu Yin Jian (Solidify Yin Decoction) are appropriate. If both Qi and Blood are deficient, Wu Fu Yin (Five Blessings Drink), Ba Zhen Tang (Eight Treasures Decoction), or Shi Quan Da Bu Tang (Great Tonifying Decoction of Ten Ingredients) are appropriate. If Spleen and Kidney Qi deficiency is accompanied by turbid leukorrhea, Mi Yuan Jian (Secret Origin Decoction) or Tu Si Jian (Dodder Seed Decoction) are appropriate. If there is frequent nausea and vomiting, add Er Chen Tang (Two Aged Herbs Decoction) or similar formulas to harmonize, depending on the preceding syndromes and formulas. In treating deficiency syndromes, flexibility is key; one cannot speak of rigid adherence.
Tai Yuan Yin (See *New Formulas Eight Arrays · Cause Array*) treats women whose Chong and Ren vessels have lost control, resulting in unstable and insecure fetal origin; modify according to syndrome. Take 1-2 doses regularly every other day or every two to three days.
Ren Shen (Ginseng Radix et Rhizoma) [as needed] Dang Gui (Angelicae Sinensis Radix) Du Zhong (Eucommiae Cortex) Shao Yao (Paeoniae Radix Alba) [each 2 qian] Shu Di Huang (Rehmanniae Radix Preparata) [2-3 qian] Bai Zhu (Atractylodis Rhizoma) [1.5 qian] Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma, honey-fried) [1 qian] Chen Pi (Citri Reticulatae Pericarpium) [7 fen, omit if no stagnation]
Use 2 zhong of water, decoct to 0.7 zhong, take long after meals. If the Lower Yuan is insecure with much turbid discharge, add Shan Yao (Dioscoreae Rhizoma), Gu Bu Zhi (Psoraleae Fructus), and Wu Wei Zi (Schisandrae Fructus). If Qi deficiency is severe, double Bai Zhu and add Huang Qi (Astragali Radix); however, Bai Zhu has floating Qi and can block the stomach appetite; if there is chest/diaphragm fullness and discomfort, use with caution. If deficiency is accompanied by cold and much vomiting, add Pao Jiang (Zingiberis Rhizoma Preparatum) 7-8 fen or 1-2 qian. If deficiency is accompanied by heat, add Huang Qin (Scutellariae Radix) 1.5 qian, or add Di Huang (Rehmanniae Radix) 2 qian, removing Du Zhong. If Yin deficiency causes lower abdominal pain, add Gou Qi Zi (Lycii Fructus) 2 qian. If there is much anger and rebellious Qi, adding Xiang Fu (Cyperi Rhizoma) is acceptable, or Sha Ren (Amomi Fructus) is also excellent. If movement triggers bleeding due to contact, add Xu Duan (Dipsaci Radix) and E Jiao (Colla Corii Asini) 1-2 qian each. If vomiting persists, add Ban Xia (Pinelliae Rhizoma) 1-2 qian and Sheng Jiang (Zingiberis Rhizoma Recens) 3-5 slices.

When fetal qi has excess stagnation of Qi, manifesting as morning sickness, distension/fullness, and instability, this occurs only in those whose constitution is not deficient but has much depression/stagnation. Investigate the cause and open/guide the stagnation; treating excess patterns is not difficult. For persistent vomiting, Er Chen Tang (Two Aged Herbs Decoction) plus Zhi Ke (Aurantii Fructus Immaturus) and Sha Ren (Amomi Fructus) is the main treatment, or Ren Shen Ju Pi Tang (Ginseng and Tangerina Peel Decoction) is also excellent. Wang Jiezhai said: Regulating pregnant women lies in clearing heat and nourishing blood. Bai Zhu (Atractylodis Rhizoma) tonifies the Spleen and is the Sovereign herb for stabilizing the fetus; Tiao Shi Huang Qin (Scutellariae Radix) is the sacred herb for stabilizing the fetus because it clears heat, making it suitable for addition in summer. This is one viewpoint. Although it seems reasonable, it actually has a major flaw, which must be辨析 (analyzed). The fetal qi of pregnancy follows the mother's organ qi. Generally, those with Yin deficiency have much heat; those with Yang deficiency have much cold. Cold becomes cold, heat becomes heat; this is called balanced Qi. Speaking of ten people, three have cold, three have heat, and four are balanced; this is the general rule. If one claims that conception inevitably increases internal heat, differing from normal people, then why is it that treating morning sickness requires Er Chen Tang (Two Aged Herbs Decoction), Liu Jun Zi Tang (Six Gentlemen Decoction), Sheng Jiang (Zingiberis Rhizoma Recens), and Ban Xia (Pinelliae Rhizoma) to be effective? Is it truly increasing heat? Therefore, treating heat suits Huang Qin (Scutellariae Radix), but it does not suit cold. Not only does it not suit cold, it does not suit balanced Qi either. Generally, among modern pregnant women, those with Excess Qi are few; those with Deficient Qi are many. Deficient Qi implies Yang deficiency; using Huang Qin (Scutellariae Radix) further damages them, causing immediate illness. Even if no immediate harm is felt, it secretly damages the fetal origin and depletes maternal Qi, leading to maternal emaciation/fatigue or children with many Spleen diseases, largely due to this. Unfortunately, modern people fail to understand the principle, regarding "Sacred Medicine" as a mandatory drug for all pregnancies, regardless of Yin/Yang strength. Using it for all stabilization cases causes considerable harm. As for Bai Zhu (Atractylodis Rhizoma), although good at stabilizing the fetus, if used improperly, its nature is drying and its Qi is closing. Thus, it cannot be used alone for Yin deficiency, and for Qi stagnation, it must be handled flexibly. Hence, the difficulty of medication lies in being flexible like pearls on a tray; one cannot play the zither with fixed bridges (be rigid).
Lizhai said: "If pregnancy involves insufficient Yuan Qi, fever, fatigue, or fetal instability, use Dang Gui San (Angelica Powder). Due to anger, add Zhi Ke (Aurantii Fructus Immaturus); for chest/diaphragm stuffiness, add Zi Geng (Perillae Caulis); if pain occurs, add Chai Hu (Bupleuri Radix).

Xu Xueshi said: Generally, pregnancy relies on suppressing Yang and assisting Yin. However, pre-natal medicines abhor mixing Yin and Yang, which leads to other diseases. Only Zhi Ke Tang (Immature Bitter Orange Decoction) suppresses Yang, and Si Wu Tang (Four Substances Decoction) assists Yin. However, Zhi Ke Tang (Immature Bitter Orange Decoction) is mostly cold in flavor; taking it alone may cause fetal cold and abdominal pain. More importantly, assist with Nei Bu Wan (Internal Nourishing Pill), so Yang does not become too strong, Yin does not become too weak, Yin and Yang harmonize, benefiting offspring. This was not discussed by predecessors.
