Ulcers in the female genitalia are mostly caused by damp-heat pouring downward, or fire generated from emotional stagnation, or the abuse of medications (Note: Ancient methods sometimes used acrid-warm stimulating herbs applied externally to enhance sexual desire), leading to invasion by heat-toxin. External manifestations include: protrusion from the vagina resembling a snake's head, termed Yin Ting (Yin Prolapse); mushroom-shaped growths, termed Yin Jun (Yin Fungus); or formations resembling rooster combs; presence of parasites with dampness and itching; internal ulceration with swelling, rotting, and pain, often discharging toxic fluids. Internal manifestations include: body fatigue and internal heat, irregular menstruation; loss of appetite and afternoon tidal fever; epigastric fullness and distension, discomfort in the waist and hypochondrium; dribbling dysuria, and red-and-white leukorrhea.
General treatment principles for this condition: If there is swelling, pain, and both internal and external ulceration, Shaoyao Jili Decoction (Peony and Tribulus Decoction) is best, or Siwu Tang (Four Substances Decoction) added with Gardenia, Moutan Cortex, Gentian Root, and Schizonepeta; or Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). If dampness and itching predominate, use Shaoyao Jili Decoction or Guipi Tang (Restore the Spleen Decoction) added with Bupleurum, Gardenia, and Moutan Cortex. For dribbling dysuria, use Longdan Xiegan Tang (Gentian Liver-Draining Decoction) added with Atractylodes Macrocephala and Moutan Cortex. For dribbling dysuria accompanied by excessive fire, pain, and distension, use Dali Qingyin (Great Purification Drink) or Chouxin Yin (Draw Down the Fuel Drink). For swelling with a sensation of heaviness and prolapse, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Gardenia and Moutan Cortex. For washing, use Baicao Jian (Hundred Herbs Decoction); for topical application, use Piaoxiao San (Cuttlebone Powder) or Wanchuang San (Complete Ulcer Healing Powder).
Shaoyao Jili Decoction (see *New Formulas Eight Arrays · Yin Array*) treats widespread damp-heat sores and rashes, as well as various sores with redness, swelling, heat, and pain in the lower body, with miraculous effects. Externally apply Piaoxiao San.
Ingredients: Gentian Root (Longdan Cao), Gardenia (Zhizi), Scutellaria Root (Huangqin), Akebia Stem (Mutong), Alisma Rhizome (Zexie) (each 1.5 qian); Peony Root (Shaoyao), Rehmannia Root (Shengdi) (each 2 qian); Tribulus Fruit (Bai Jili) (with spines, crushed, 5 qian to 1 liang).
Preparation: Add two bowls of water, decoct until reduced to eight-tenths. Take after meals. If fire is not severe, remove Gentian Root and Gardenia, and add Angelica Sinensis, Poria, and Coix Seed. If damp-toxin is severe, add Smilax Glabra Rhizome (Tufuling) 5 qian or 1–2 liang.

Piaoxiao San (Cuttlebone Powder) (see *New Formulas Eight Arrays · Yin Array*) treats sores caused by damp-heat with skin breakdown and dripping toxic fluids, or swelling, pain, and tenderness in the scrotum, feet, and thighs, as well as chancres and other sores in the lower body, with miraculous efficacy.
Ingredients: Cuttlebone (Haipiaoxiao) (no need to soak to remove saltiness), Human Bone Ash (Renzhongbai) (or Human Waste Yellow, or Sand, equal parts).
Preparation: Grind into fine powder. First, decoct Baicao Jian into a strong broth. While hot, fumigate and wash the area, then sprinkle this powder on the lesion. If the wound is dry, mix with Sesame Oil, rendered Pork Lard, or Honey Water for topical application. If swelling and pain are severe, adding a small amount of Borneol is better. If there is excessive pus and fluid from damp sores, add Lead Monoxide (Miduoseng) in equal parts, or Over Palace Powder (Guogongfen) is also acceptable, or Calcined Calamine (Zhi Luganshi) is even better.
Wanchuang San (Complete Ulcer Healing Powder) (see *New Formulas Eight Arrays · Yin Array*) treats various damp-rotten sores where flesh has flattened but fails to heal, and various sore toxins where internal tissue has flattened but the opening does not close. This formula is most excellent for such cases.
Ingredients: Talcum (Huashi) (calcined, 1 liang), Red Kaolin (Chishizhi) (calcined, 5 qian), Licorice Root (Fen Gancao) (3 qian).
Preparation: Grind into powder. For dry exudate, sprinkle directly; or mix with Sesame Oil for topical application. Adding 1 qian of Alum (Kufan) is extremely suitable for itching.
Shetui San (Snake Slough Powder) Treats female genital ulcers. First, fumigate and wash with Schizonepeta and Cnidium Fruit, pat dry, then apply medicine.
Ingredients: Snake Slough (Shetui) (one strip, burned to retain properties), Alum (Kufan), Lead Tetroxide (Huandan), Polygonum Aviculare (Lümi), Ligusticum Wallichii (Chuanxiong) (each 1 liang), Realgar (Liuhuang), Schizonepeta Spike (Jingjie Sui), Cnidium Fruit (Shechuangzi) (each 5 qian).
Preparation: Grind into fine powder. Mix with Sesame Oil for rubbing. If wet, sprinkle dry powder.
