Epileptic seizures are a severe condition in infants and children. Due to their young age, the blood vessels are not yet consolidated, and the bone qi is not fully gathered. The disease arises from invasion by wind pathogens, contact with startling stimuli, irregular breastfeeding, and the stagnation of qi mechanism in the meridians. Clinical manifestations include depression of spirit and qi, staring eyes, facial muscle traction and spasms, locked jaw, drooling, abdominal distension and tightness, twitching and convulsions of the limbs, appearing as if between life and death, sometimes vocalizing and sometimes silent, with some experiencing opisthotonos (backward arching of the neck and back) or rigidity of the waist and spine. However, the limbs remain soft, and the patient regains consciousness intermittently after an episode; this is termed "Xian" (epilepsy). If the whole body is rigid and the patient remains unconscious for the entire day, it is termed "Jing" (tetanus/stiffness syndrome).
Epilepsy is classified into five types corresponding to the five Zang organs:
1. Heart Epilepsy: Red face, staring eyes, protruding tongue, biting teeth, irritability in the heart region, shortness of breath, and rapid respiration.
2. Liver Epilepsy: Blue-green complexion of the face and lips, upward rolling of the eyes, clenched fists and contracture of the limbs, convulsive twisting and backward bending.
3. Kidney Epilepsy: Dark and dull complexion, trembling gaze directed at others, spitting clear foam, motionless like a corpse.
4. Lung Epilepsy: Withered complexion resembling dead bone, whites of the eyes looking upward, jumping and shaking head, spitting saliva.
5. Spleen Epilepsy: Sallow complexion, staring eyes, abdominal fullness with spontaneous diarrhea, inability to control the limbs.
These are the typical presentations of the five Zang epilepsies. The method of regulation and treatment primarily involves distinguishing and treating based on three causes—Startling, Wind, and Food—and two syndromes—Yin and Yang.
Wind Epilepsy: Caused by wind pathogens taking advantage of deficiency after sweating and opening of the pores. Initially, fingers may curl as if counting, accompanied by fever and phlegm generation.
Startling Epilepsy: Caused by shock, terror, or accumulation of startling qi from falls/trauma. Initially characterized by startled crying, mental confusion, and loss of spirit.
Food Epilepsy: Caused by fright during feeding leading to food stagnation. Initially characterized by vomiting milk, inability to feed, sour-smelling stool, formation of milk lumps, and initial chills followed by fever.
Differentiation of Yin and Yang Syndromes:
Yang Epilepsy: Initial fever, convulsions, and crying. Pulse is floating, complexion is lustrous, pathology is in the Six Fu organs and skin/muscle. This is relatively easy to cure.
Yin Epilepsy: Initial absence of fever, cold hands and feet, no convulsions or crying. Pulse is deep, complexion is dark and dull, pathology is in the Five Zang organs and bone marrow. This is the most difficult to cure.
Alternatively, supine position belongs to Yang, while prone or curled position belongs to Yin, which can also be used for reference. Generally, Yang syndromes should not use warming treatments, and Yin syndromes should not use cold treatments. For Wind Epilepsy, disperse wind first; for Startling Epilepsy, calm the mind/startle first; for Food Epilepsy, resolve food stagnation first, then administer antiepileptic formulas. Essentially, blood stasis in the heart orifice, wind evil in the heart, and accumulated startle forming epilepsy require treatment focused on unblocking the Heart Meridian, regulating Heart blood, directing Qi, and resolving phlegm.
If subsequent symptoms include heat and phlegm, refusal to feed, restless sleep, and frequent palpitations/startles, these are precursors to epilepsy. Immediately administer Zishuang Wan (White Frost Pill) to guide out the pathogen. Timely and appropriate dosage of Zishuang Wan reduces excessive pathogenic qi, preventing complications of wind-phlegm epilepsy. When symptoms are just emerging, blue veins often appear like threads behind the ear near the high bone. Upon seeing this, scratch it open immediately to induce bleeding and crying, which facilitates Qi circulation. Do not dry infants' clothes overnight outdoors, fearing contamination by feathers or feces from the "Wugu" bird (a toxic bird), which can cause epilepsy. Those complicated by strange pathogens exhibit variable complexions and shyness around people. Various epilepsies causing aphonia occur because the throat is the pathway for Qi; wind damages Qi, blocking this pathway, or blood stasis blocks the Heart orifice.
