小儿急慢惊风症候,素问所谓阴阳痫者是也。急者,属阳,阳盛而阴亏;慢者,属阴,阴盛而阴亏。阳动而燥疾;

The clinical manifestations of infantile acute and chronic convulsions (Ji Man Jing Feng) correspond to what is referred to as "Yin-Yang Epilepsy" (Yin Yang Xian) in the *Su Wen* (Basic Questions). Acute convulsions belong to the Yang category, characterized by excess Yang and deficiency of Yin; chronic convulsions belong to the Yin category, characterized by excess Yin and deficiency of Yin. Yang conditions are active, restless, and rapid; Yin conditions are quiet, slow, and sluggish. Initially, both arise from internal deficiency of the Zang-Fu organs due to lack of proper regulation. Deficiency generates heat, and heat generates wind. Thus, wind originates from the Liver, phlegm from the Spleen, fright from the Heart, and heat affects the Lung. When the four pathological factors of Fright, Wind, Phlegm, and Heat are present, eight specific signs (Ba Hou) emerge. The eight signs include: eyes rolling up or deviating, head shaking, mouth open with tongue protruding, red lips and flushed cheeks, and spasmodic twitching with tremors and sweating. If the patient presents with a cyanotic complexion, cyanotic eye sockets, cyanotic lips, cyanotic stool, or cyanotic veins visible at the hairline near the Taiyang acupoint and Yintang area, and if the fingerprint on the Tiger's Mouth (Hu Kou) three palaces shows red-purple turning cyanotic, these are all signs of convulsions. One must ascertain the exterior and interior, examine cold and heat, assess deficiency and excess, distinguish between favorable and unfavorable prognoses, and treat according to the syndrome. Generally, medications for acute convulsions should be cooling; those for chronic convulsions should be warming. Additionally, observe gender differences: boys twitching look left, girls look right; boys' eyes roll upward, girls' downward; boys hold their thumbs outside when making a fist, girls inside; boys' hands pull to the left, girls to the right. These are all favorable syndromes that can be treated. Reversal indicates an unfavorable prognosis. In treating convulsions, first stop the wind; to treat wind, first facilitate calming fright; to treat fright, first clear heat; to clear heat, first resolve phlegm; to treat phlegm, first descend fire. If all four syndromes appear simultaneously, treat them concurrently.
Chronic Pi Wind (Man Pi Feng) often results from prolonged diarrhea, vomiting, or dysentery leading to loss of appetite and extreme depletion of Yuan Qi (Primordial Qi), transforming into this syndrome. Treatment must prioritize warming the Spleen and harmonizing the Stomach, supporting Yuan Qi, with dispelling wind and resolving phlegm as secondary measures. Another condition called Horse Pi Wind (Ma Pi Feng) is extremely malignant. It arises from failure to disperse wind-cold pathogenic factors, allowing cold qi to lodge in the Lung Meridian. Symptoms include cyanosis, shortness of breath, open mouth with flaring nostrils, chest and hypochondriac pulling with wheezing, resembling a horse panting after running, hence the name. Survival is rare.
Sudden strabismus indicates Fright invading the Liver; teeth grinding during sleep indicates Fright invading the Kidney; crying throughout the night until dawn indicates Fright invading the Small Intestine; wheezing and drinking water indicate Fright invading the Lung; red face and flushed cheeks indicate Fright invading the Heart; five-palm heat indicates Fright invading the Spleen; alternating cyanosis and pallor indicates Fright invading the Gallbladder; saw-like phlegm rale in the throat indicates Fright invading the Large Intestine; intermittent dry retching indicates Fright invading the Stomach; crying during sleep indicates Fright invading the San Jiao (Triple Burner).
Another interpretation of the Eight Signs is: Chu (twitching), Nuo (clenching), Che (pulling), Chan (trembling), Fan (reversal/extension), Yin (drawing), Cuan (bulging/protruding), Shi (gazing). (All are malignant signs.)
Chu refers to the extension and flexion of both hands; Nuo refers to the opening and closing of ten fingers; Che refers to a posture like wrestling; Chan refers to the head tilting abnormally; Fan refers to the body arching backward like a bow (opisthotonos); Yin refers to arms extended like drawing a bow; Cuan refers to staring eyes looking angry; Shi refers to exposed eyeballs that do not rotate.
