Mr. Jin Wenxue Daquan's wife suffered from dysentery with red and white discharge during the hot summer months, defecating more than thirty times a day, accompanied by tenesmus (a feeling of incomplete evacuation), leg cramps, thirst, and lower abdominal pain. She had been pregnant for two months; fortunately, her waist did not hurt. Her right pulse was wiry (Xian), and her left pulse was slippery (Hua). Prescribed were Bai Shao Yao (Radix Paeoniae Alba) 3 qian, Huang Lian (Rhizoma Coptidis), Huang Qin (Scutellariae Radix), Dang Gui (Radix Angelicae Sinensis), Chen Pi (Pericarpium Citri Reticulatae), and Xiang Fu (Cyperi Rhizoma) each 1 qian, plus Gui Pi (Cinnamomi Cortex) and Mu Xiang (Aucklandiae Radix) each 5 fen, decocted in water and taken orally. Externally, Xiang Lian Wan (Fragrant Coptis Pill) was administered, which slightly relieved the tenesmus after ingestion. However, because she ate too early, she still defecated over ten times that night. The next morning upon examination, the wiry quality of the right pulse had decreased by half. The previous decoction was prepared, and she swallowed Nv Jin Dan (Female Golden Pill); after one dose, the dysentery reduced by more than half, and all six pulse positions became harmonious, though weak and soft. This indicated deficiency of Qi and Blood, so treatment should focus on tonification and nourishment using Ba Zhen Tang (Eight Treasures Decoction) added with Chen Pi, Xiang Fu, E Jiao (Colla Corii Asini), and Tiao Qin (Scutellariae Radix). After one dose, she recovered completely. In cases of pregnant women suffering from dysentery, if tenesmus is slightly relieved and abdominal pain subsides but diarrhea persists, the primary treatment must be to tonify Qi and Blood, assisted by Qi-regulating agents, to ensure the preservation of the fetus. This aligns with Danxi’s principle that "when deficiency returns to normal, dysentery stops naturally."
Literati Jin Yuanyan experienced afternoon fever, with dysenteric stools containing more red blood than white mucus, defecating more than seventy times a day, accompanied by a sensation of heaviness and prolapse due to tenesmus, and loss of appetite. His left pulse was fine and rapid (Xi Shu), and his right pulse was slippery (Hua), indicating Yin deficiency. Upon inquiry, it was learned that he had nocturnal emission two days prior, followed by the onset of dysentery, confirming Yin deficiency. However, the slippery pulse also suggests food stagnation. The method should be to tonify first and then purge. Thus, Xiao Jian Zhong Tang (Minor Construct the Middle Decoction) was prescribed for one dose: Bai Shao Yao 3 qian, Gui Zhi (Ramulus Cinnamomi) 7 fen, Fen Cao (Glycyrrhizae Radix et Rhizoma Praeparata), Jiu Lian (Coptidis Rhizoma processed with wine), and Jiu Qin (Scutellariae Radix processed with wine) each 8 fen, Dang Gui 1 qian, and Bing Lang (Arecae Semen) 5 fen, decocted in water and drunk. At midnight, upon re-examination, the pulse felt slightly stronger against the fingers. The prescription was changed to Zhi Qiao (Fructus Aurantii) 3 qian, Tao Ren (Semen Persicae) 1 qian, and Dang Gui 4 qian, decocted and taken with Mu Xin Bing Lang Wan (Aucklandia and Areca Pill) 1.5 qian swallowed. By dawn, he had three bowel movements, and feces were visible. The next noon, after eating rice and then consuming braised pork, his bowel movements became frequent again, and tenesmus returned as before. Shan Zha Zhi Zhu Wan (Hawthorn and Immature Bitter Orange Pill) was given once without effect. Upon re-examination, all six pulse positions were deficient, soft, and powerless, except for the slippery pulse at the right Guan position. This was due to eating meat and rice too early, causing Spleen Qi weakness and inability to digest. Treatment should strengthen Spleen Qi and lift Yang. Ren Shen (Ginseng Radix), Huang Qi (Astragali Radix) each 2 qian, Bai Zhu (Atractylodis Macrocephalae Rhizoma) 1 qian, Sheng Ma (Cimicifugae Rhizoma) 3 qian, Fang Feng (Saposhnikoviae Radix), Huo Xiang (Agastachis Herba), Pao Jiang (Zingiberis Rhizoma Praeparatum), and Fen Cao each 5 fen, Bai Shao Yao 1.5 qian, and Fu Ling (Poria) 8 fen were prescribed. After two consecutive doses, the dysentery decreased, and tenesmus eased. Due to excessive consumption of dog meat, which further injured Spleen Qi, Sha Ren (Amomi Fructus) and Shan Zha (Crataegi Fructus) were added to the previous formula, leading to recovery through regulation.
