Tubercles refer to hard, nodular accumulations caused by the retention of heat-fire due to phlegm obstruction, resembling the pit within a fruit. These nodules may appear in the neck, hypochondrium, hands and feet, nape of the neck, arms, or axillae. Their appearance resembles a swelling toxin (tumefaction), but they are neither red nor painful, do not suppurate, and do not require ulceration or discharge. Once the internal heat dissipates, the nodules will naturally resolve. The general treatment principle is to administer Er Chen Tang (Two-Aged Decoction, a basic formula for drying dampness and resolving phlegm) with Zhu Li (Bamboo Sap, used to clear heat and resolve phlegm), taken in large quantities for efficacy.
Plum-pit Qi refers to a sensation of obstruction in the throat, where the foreign body cannot be expectorated nor swallowed, feeling like a plum pit stuck there. This condition initially arises from excessive anger or joy, leading to accumulated heat and flourishing stagnation, causing stubborn phlegm to knot and resulting in this disease. Treatment should employ methods to guide out phlegm, open depression, clear heat, and regulate qi. Herbs such as Chen Pi (Citrus Peel), Ban Xia (Pinellia Rhizome), Chuan Xiong (Sichuan Lovage Rhizome), Xiang Fu (Cyperus Rhizome), Shan Zhi (Gardenia Fruit), Huang Qin (Scutellaria Root), Zhi Qiao (Aurantium Fruit), and Su Zi (Perilla Seed) may be selected. If old phlegm is knotted and unyielding, salty herbs that can soften hardness, such as Hai Shi (Floating Limestone), should be used.
One discussion addresses tubercles forming lumps in the throat, sized like walnuts, which are swollen, hard, and painful, accompanied by similar conditions in both axillae and stiffness/swelling in the neck preventing head rotation.

A man who was prone to anger suddenly developed slight swelling in the neck, which gradually enlarged to the size of a sheng (a unit of volume). Initially treated with phlegm-clearing and qi-regulating methods, he developed severe heat, thirst, and frequent turbid urination. I diagnosed Kidney Water deficiency and treated him with Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) combined with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction), resulting in recovery. In cases where the neck or hypochondrium swells to the size of a sheng or dou, or ulcerates like a pomegranate, regardless of size, the aforementioned methods should be applied. Large quantities must be taken until痊愈 (recovery) is achieved.
