*Zhuang's Family Collection* records: When a child is unable to speak and cries without being able to distinguish the cause, one should palpate the abdomen with the hand. If there are hard areas, it indicates abdominal pain. The treatment method is to crush fresh ginger and extract the juice, warm it until lukewarm, mix with flour to form a paste, apply it on paper, and paste it onto the umbilicus (Shenque, CV8). The pain will stop immediately.
[Qian Yi] Abdominal distension is caused by deficiency of the Spleen and Stomach with rebellious Qi attacking. Excess patterns manifest as chest oppression, restlessness, wheezing, and fullness; purgation can be used, employing Zishuang Wan (Purple Frost Pill) or Baibing Zi (White Cake Pill) (This refers to wheezing before purgation, which indicates an excess pattern, hence purgation is suitable; if wheezing occurs after mistaken purgation, it indicates deficient Qi attaching to the Lung, and purgation is contraindicated. See "Pi Ji" for formulas). Non-wheezing cases indicate deficiency and should not be purged. Mistaken purgation causes Spleen-deficient Qi to rebel upwards and attach to the Lung. Since the Mother (Spleen) and Child (Lung) organs are both deficient, and the Lung governs the eyelids and cheeks while the Spleen governs the limbs, extreme deficiency of the Mother Qi leads to swelling of the eyelids and cheeks, and yellowing of the limbs. Treatment involves using Taqi Wan (Collapse Qi Pill) to gradually resolve the condition. If not cured, gradually increase the dosage. Do not use strongly warming and dispersing herbs like Dingxiang (Caryophylli Flos), Muxiang (Aucklandiae Radix), Jupi (Citri Reticulatae Pericarpium), or Doukou (Cardamomi Fructus). Why? Because the Spleen-deficient Qi has not yet escaped outward; thus, although there is abdominal distension, there is no wheezing. In this case, warming and dispersing herbs can be used to distribute and eliminate Qi from above and below, leading to recovery. If the Qi deficiency has already escaped outward and attached to the Lung, indicating internal weakness of the Spleen and Stomach, deficient Qi frequently enters the limbs and face. Children are prone to mixed deficiency and excess; when the Spleen is deficient, it cannot tolerate cold or heat. Cold medicines generate cold pathogens, while warm medicines generate heat. One must recognize this to avoid errors. Long-term Stomach deficiency-heat often leads to jaundice or excessive thirst. Spleen deficiency fails to control Kidney Qi, which ascends with Lung Qi to the limbs, causing eyelid swelling resembling water retention. Kidney Qi floats diffusely into the Lung, causing severe wheezing. This requires Taqi Wan. If the face remains pale after cure, it is because deficiency has not yet recovered (This is wheezing after purgation, so Taqi Wan is appropriate; if wheezing precedes purgation, purgation is indicated). Treating abdominal distension is like warfare: if the enemy has not left the forest, attacking with troops will surely succeed; if the enemy has left the forest, do not attack hastily, as hasty attack leads to loss. One should gradually subdue them with strategy, which is favorable (Enemy not leaving the forest means deficient Qi has not escaped, no wheezing, no eyelid/cheek swelling, so strongly warming/dispersing herbs like Dingxiang and Muxiang can distribute Qi from above and below to cure; enemy leaving the forest means deficient Qi has escaped, presenting as wheezing and swelling, requiring gradual resolution with Taqi Wan). For pediatric deficiency-type abdominal distension, first administer Taqi Wan. If ineffective, and there is food accumulation and fecal impaction in the abdomen, yellow urine, occasional mild wheezing, deep and solid pulse, frequent thirst, and ability to eat, purgation may be used. Since the Spleen was initially deficient and then developed accumulation, treatment should first tonify the Spleen, then purge, then tonify the Spleen again for recovery. Do not tonify the Lung, lest it generate deficient wheezing.
[Zhang Yuansu] Regarding excess and deficiency in abdominal distension, especially after prolonged vomiting and diarrhea: Deficiency presents with fine and weak pulses. The Lung governs eyelids and cheeks; the Spleen governs limbs. Pale yellow complexion, flaccid swelling of eyelids and cheeks, and cold hands and feet indicate the need to first take Taqi Wan, followed by Yigong San (Extra Efficacy Powder), Hezhong Wan (Harmonize Middle Pill), Sijunzi Tang (Four Gentlemen Decoction), Yihuang San (Yellow Benefit Powder), etc., using various warming medicines to nourish True Qi (See "Vomiting and Diarrhea" for these three formulas). Excess patterns present with surging and solid pulses, not necessarily following prolonged vomiting/diarrhea or dysentery. Abdominal distension with urgent wheezing, oppression, restlessness, phlegm-heat, or undigested food causing distension require purgation with Baibing Zi, Dahuang Wan (Rhubarb Pill), or Jiedu Wan (Detoxification Pill). Carefully examine urination and defecation; if both are blocked, promote urination first, then defecation. (See "Pi Ji" for Baibing Zi, "Sore Throat" for Jiedu Wan, "Cold Damage" for Dahuang Wan). Yun Qingzi stated: For children with internal heat binding, abdominal distension, high fever, red-yellow stools, and restless agitation, Xieqing Wan (Drain Green Pill) is suitable.
[Xue Ji] Li Dongyuan said: Cold distension is common; heat distension is rare. Both are governed by the Spleen and Stomach. Deficiency should use Liujunzi Tang (Six Gentlemen Decoction). Wheezing with shortness of breath indicates Spleen-Lung Qi deficiency, treated with Yigong San. If wheezing and distension worsen after using draining/harsh medicines, it indicates further injury to Spleen-Lung Qi; add Banxia (Pinelliae Rhizoma) and Shengma (Cimicifugae Rhizoma) to the previous formula. If purged and no longer wheezing, pathogenic Qi is gone and Lung Qi is calm; no medicine is needed. Prolonged illness with difficulty urinating or limb edema indicates Spleen-Lung Qi deficiency failing to regulate water passages; treat with Jin Gui Ji Jian Shenqi Wan (Modified Kidney Qi Pill from Golden Cabinet). Cold extremities and sleeping with eyes open indicate Spleen-Stomach deficiency-cold; use Liujunzi Tang with Paojiang (Prepared Ginger). Warm extremities but sleeping with eyes open indicate Spleen-Stomach weakness; use Liujunzi Tang. Cyan complexion indicates Wood overacting on Earth; use Liujunzi Tang, Muxiang, and Chaihu (Bupleuri Radix). Also, regulate the nursing mother's diet to prevent excessive medication from injuring Stomach Qi. A child with abdominal distension, red face, phlegm-wheezing, constipation, high fever, and preference for cold drinks presented with excess syndrome. One dose of Zishuang Wan aggravated symptoms, turning the face white and increasing desire for hot drinks. I reasoned that pathogenic Qi receded but True Qi was injured, hence whiteness and preference for hot drinks. Large doses of Baizhu San (Atractylodis Macrocephalae Powder) were decocted and allowed to drink freely; sleep ensued, and recovery occurred next day. Another child with food-stagnation distension and chest fullness/phlegm was cured with Yigong San. Recurrent food-stagnation distension with pain, treated with purgatives, worsened pain/distension and added wheezing. This indicated Spleen-Stomach injury leading to Lung deficiency. Cured with Liujunzi Tang plus Jiegeng (Platycodonis Radix). A child with distension, aversion to food, fever, nausea, mimicking exterior syndrome, was diagnosed with food stagnation. Cured with Baohe Wan (Preserve Harmony Pill) for distension relief, followed by Yigong San. A child with food-stagnation distension treated with harsh drugs developed scanty urination; subsequent Wuling San (Five Ling Powder) reduced intake but caused anuria and limb edema. Treated morning with Shenqi Wan (Kidney Qi Pill) from *Jin Gui* minus Fuzi (Aconiti Lateralis Radix Preparata), evening with Buzhong Yiqi Tang (Middle-Boosting and Qi-Invigorating Decoction), resulting in peace. A child with distension, green-white stools, left abdominal mass, sallow complexion, red/rotten gums, and drooling was treated primarily with Yigong San to harmonize Middle Qi, assisted by Dayuwang Tang (Greater Wormwood Decoction) to clear Gi and heat. After a month, symptoms improved; continued Yigong San and Hemawang Wan (Toad Pill), external Awei Gao (Styrax Plaster). Mass resolved in two months; residual left hypochondriac pain cured with Sijunzi Tang and Jiwei Luhui Wan (Nine-Ingredient Aloe Pill). *Baiwen* states: Gi distension, Qi distension, Zhengji distension, Suodu distension, Wuige distension, Food distension, Roundworm distension, Deficiency-cold accumulation distension. For Gi-Qi accumulation distension, use Baotong Wan and Taqi Wan first. For Pi Pi Qi distension and Zhengji distension, use Sanleng (Sparganii Rhizoma) to dissipate masses. For Suodu distension, use Zhenzhu Tianma Wan (Pearl Gastrodia Pill) to unblock. For upper epigastric food-injury distension, use Sanleng, Taqi Wan, and Dajulian Wan (Major Zanthoxyli-Coptis Pill) to digest. For roundworm distension, use Xiachong Wan (Expel Parasite Pill) to dissolve. For deficiency-cold accumulation distension, use Chenxiang Jian (Agarwood Decoction) to warm. All conditions require harmonizing Stomach Qi, digesting, and promoting unblocking; swelling will resolve. Heat cases require Tingli (Descurainiae Semen) and Qianniu (Pharbitidis Semen). Tuiqi Wan (Push Qi Pill) is also usable.
Note: *Baiwen*'s differentiation is detailed but biased towards purgation. Without Qian Yi/Zhang Yuansu's flexible methods and Xue Ji's tonifying approaches, few avoid fatal errors. Be cautious.
