Congenital epilepsy in infants is caused by the mother drinking alcohol, experiencing anger or fright/falls during pregnancy, or being externally affected by wind pathogens that internally injure the fetus, resulting in illness immediately after birth. If within one month of birth there is high fever, eyes rolling upward, clenched fists, locked jaw with salivation, stiff back with spasms, frightened crying, retracted cheeks, open fontanelle, and bluish complexion with closed eyes, treatment should focus on dispersing wind and calming fright, resolving phlegm and regulating qi, along with applying external paste to the fontanelle. If the face is blue with clenched fists and spasms, use Gouteng San (Uncaria Decoction). Do not mistakenly treat it as Spleen Wind and recklessly apply warming herbs. If the area between the eyebrows is red, or if the fingerprint on the tiger's mouth curves inward, it is treatable; add Quanxie (Full Scorpion) to Gouteng San. If the area between the eyebrows is blue-black, it is untreatable. Generally, infants' zang-fu organs are fragile; heavy sedative agents containing silver powder (cinnabar/mercury compounds) should not be used lightly, as they may damage true qi and often lead to fatal outcomes.
The eyes are the orifices of the Liver. The Liver and Gallbladder belong to the Wind-Wood meridians and are associated with Ministerial Fire. When Liver blood is insufficient, internal wind and fire arise, causing the eyes to tremble. The Classics state: "Bending and straightening, shaking and moving" are manifestations of Wind. It is appropriate to use Siwu Tang (Four Substances Decoction) to nourish blood, and Chaihu (Bupleuri Radix), Shanzhi (Gardeniae Fructus) to clear the Liver. When Yin-blood is nourished internally, deficient wind will naturally subside. Additionally, use Liuwei Wan (Six-Ingredient Pill with Rehmannia) to nourish the root source. If after recovery there is palpitation and insomnia, or convulsions, teeth grinding, and eye trembling during sleep, this is due to blood deficiency failing to nourish the sinews and vessels. Use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with increased amounts of Danggui (Angelicae Sinensis Radix) and Baishao (Paeoniae Radix Alba). Whenever pathogenic qi is excessive, it belongs to a deficiency of Yuanqi (Primary Qi). Moreover, this syndrome involves both the Liver and Spleen, often representing the onset of Slow Convulsion (Man Jing); thus, careful examination is essential.
The lips are the flourishing aspect of the Spleen. the Foot-Yangming Stomach Meridian circles the lips and intersects at the Renzhong point. Therefore, those with Spleen-Stomach deficiency often present with this syndrome, not only post-illness. The Spleen governs saliva; when Spleen deficiency fails to control fluids, drooling often accompanies it. If mistakenly treated as Phlegm using drugs to expel phlegm, body fluids will become further depleted, failing to nourish the sinews, leading to limb convulsions and worsening condition. The treatment method is the same as for Slow Spleen Wind (Man Pi Feng): greatly tonify the Spleen and Stomach using Liujunzi Tang (Six Gentlemen Decoction) with Shengma (Cimicifugae Rhizoma) and Chaihu (Bupleuri Radix). Strictly prohibit using Qingpi (Aurantii Fructus Immaturus) and Longdancao (Gentianae Radix) which drain the Liver. If accompanied by slight limb convulsions, alternating chills and fever, diarrhea, vomiting, and sallow complexion, these indicate Spleen-Stomach injury. Add Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), and Baishao (Paeoniae Radix Alba). If Spleen Qi sinks and Liver Wood insults it, use Buzhong Yiqi Tang to raise Yang, adding Fuling (Poria), Banxia (Pinelliae Rhizoma), and Shaoyao (Paeoniae Radix) to restrain Liver and tonify Spleen. Do not use wind-dispersing or fright-calming drugs.
