Treats dysentery (dysentery) with pus and blood in the stool, and tenesmus (a feeling of incomplete defecation).
For initial onset of dysentery, Shaoyao Tang (Peony Decoction) is generally adopted. It contains Huangqin (Scutellariae Radix), Lianlian (Coptidis Rhizoma), Binglang (Arecae Semen), Gancao (Glycyrrhizae Radix et Rhizoma), Rougui (Cinnamomi Cortex), Gui (Angelicae Sinensis Radix), and Xiang (Aucklandiae Radix). [Shaoyao 3 qian, Huangqin, Lianlian, and Gui each 8 fen, Rougui 3 fen, Gancao, Binglang, and Xiang each 5 fen; decoct in water and take orally. If dysentery does not subside, add Dahuang.] One must understand that [regulating Qi and promoting blood circulation will naturally resolve tenesmus and pus/blood in the stool; if dysentery persists...]
Chen Xiuyuan said: This formula originally lacks profound meaning. It is merely based on the principle that "promoting blood circulation heals pus and blood in the stool, while regulating Qi eliminates tenesmus." In the formula, Danggui (Angelicae Sinensis Radix) and Baishao (Paeoniae Radix Alba) are used to harmonize blood; Muxiang (Aucklandiae Radix) and Binglang (Arecae Semen) are used to regulate Qi; Huangqin (Scutellariae Radix) and Lianlian (Coptidis Rhizoma) dry dampness and clear heat; Gancao (Glycyrrhizae Radix et Rhizoma) harmonizes the middle jiao and moderates the other herbs; and the warmth of Rougui (Cinnamomi Cortex) is added as a counter-agent (antidote/reversal method). Huangqin and Lianlian are necessarily restrained to prevent bias. Alternatively, the potent action of Dahuang (Rhei Radix et Rhizoma) is employed for the method of unblocking stagnation. For excess pain, vigorous purgation is required until it ceases. I also have methods for modification. Rougui (Cinnamomi Cortex) is red and enters the blood level; for red dysentery, it is taken as a counter-agent, and herbs such as Diyü (Sanguisorbae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), and Huaihua (Sophorae Flos) can also be added. Ganjiang (Zingiberis Rhizoma) is acrid and hot, entering the qi level; for white dysentery, it is taken as a counter-agent, and herbs such as Cangzhu (Atractylodis Rhizoma), Sharen (Amomi Fructus), and Fuling (Poria) can also be added. The formula has no profound meaning; Luo Dongyi's discussion of formulas seeks depth but ends up being shallow.
Treats disharmony of the zang-fu organs, where body fluids leak excessively into the bladder, resulting in frequent urination and constipation.
Use Piyue Wan (Splee Restriction Pill) to treat difficulty in defecation caused by dryness-heat. It contains Baishao (Paeoniae Radix Alba), Huomaren (Mallavi Semen), Zhishi (Aurantii Fructus Immaturus), Houpo (Magnoliae Officinalis Cortex), Xingren (Armeniacae Semen Amarum), and Huanglian (Coptidis Rhizoma - *Note: Text says Huanglian in poem but Dahuang in prescription details, context implies Dahuang/Rhubarb for constipation*). [Baishao, Huomaren, Xingren [deprived of skin and tip], Zhishi, and Houpo [stir-fried with ginger] each 5 liang 5 qian; Zheng Dahuang (Steamed Rhubarb) 10 liang; refine honey to make pills the size of tung tree seeds; take 20 pills with warm water; stop when bowel movement becomes smooth.] Moistening and sweet-slowing preserves body fluids; frequent urination with dry intestines.
Chen Xiuyuan said: Substances rich in oil can moisten dryness. Therefore, Huomaren (Mallavi Semen) is used as the sovereign herb, and Xingren (Armeniacae Semen Amarum) as the minister herb. To break accumulations, bitter substances are necessary; thus, the bitter-cold nature of Dahuang (Rhei Radix et Rhizoma) and the bitter-neutral nature of Baishao (Paeoniae Radix Alba) are used as assistant herbs. To move stagnation, one must follow the flow of Qi; therefore, Zhishi (Aurantii Fructus Immaturus) follows Qi to eliminate fullness/stagnation, and Houpo (Magnoliae Officinalis Cortex) follows Qi to drain distension as an assistant herb. Using honey to make pills takes advantage of its slow-moving property, preventing abrupt action.
