Regarding the irritability and restlessness (Fan Zao, 烦躁) in Cold Damage (Shanghan, 伤寒), this arises from a deficiency of Yin Qi (Vital Energy of Yin nature) and an excess of Yang Qi (Vital Energy of Yang nature), resulting in heat accompanied by mental disturbance and fullness. In Shaoyin disease (Disease of the Lesser Yin), the patient experiences aversion to cold and occasional spontaneous mental irritation, desiring to remove their covers; this is slight irritability. If Yin Qi is deficient and the disease originates in the Yang meridians, it leads to irritability and restlessness. The manifestations include fever, mental irritation, thirst, and a desire to drink fluids. If the Stomach is dry, delirious speech (Chan Yu, 谵语) will certainly occur. There are also cases of irritability and restlessness originating from the Shaoyin meridian, which must be differentiated.
In Cold Damage, "Fan" (Irritability/Mental Disturbance) refers to a sense of mental unrest and agitation, while "Zao" (Restlessness/Physical Agitation) refers to physical impatience and tossing about. Combined, Fan Zao indicates a Heat syndrome. Separated, Fan and Zao have distinctions between Yin and Yang: Fan belongs to Yang, and Zao belongs to Yin. Fan represents milder Heat, while Zao represents severe Heat. Conditions such as painful irritability, chest fullness with irritability, thirst with irritability, and empty irritability are all categorized under Heat due to Fan. However, there are cases of Zao without Fan, characterized by sudden onset of restless oppression, known as Yin Excess repelling Yang (Yin Sheng Ge Yang, 阴盛格阳). Although extremely restless, wanting to lie in mud or water, if the patient drinks but cannot swallow or spits out the water, this is the case.
The term "Fan Zao" implies starting with mental irritation and gradually progressing to physical restlessness. The term "Zao Fan" implies starting with physical restlessness and later turning into mental irritation. The causes of Fan Zao vary: some are due to pathogenic factors in the Exterior, some in the Interior, some caused by Fire coercion (Huo Jie, 火劫), some due to Yang Deficiency, and some due to Yin Excess. These are all different.
The Classics state: "When sweating should be induced but is not, the person becomes irritable." In Taiyang Wind Strike (Taiyang Zhong Feng, 太阳中风), with a floating and tight pulse, no sweat, and irritability, Da Qing Long Tang (Major Blue Green Dragon Decoction) is the primary treatment. This is irritability due to pathogenic factors in the Exterior.
If a patient has not had a bowel movement for six or seven days, has pain around the umbilicus, irritability, and symptoms that occur at specific times, this indicates Dry Feces (Zao Shi, 燥屎) in the interior, representing irritability due to pathogenic factors in the Interior.
If Taiyang disease is treated with fire moxibustion/fumigation and no sweat is produced, the patient's heart becomes agitated. If Taiyang disease persists for two to three days and restlessness appears, and the back is ironed with heat causing profuse sweating, leading to intense Heat entering the Stomach and causing irritability and restlessness, this is irritability caused by Fire coercion.
If Shaoyang disease is mistakenly treated with mild sweating, causing restlessness and insomnia; or after purgation, followed by sweating, causing daytime irritability and insomnia but nighttime quietness, without vomiting, thirst, or exterior signs, with a deep and faint pulse and no significant fever, Gan Jiang Fu Zi Tang (Dried Ginger and Aconite Decoction) is indicated. Also, if after sweating, purgation is applied and the disease remains, causing irritability, Fu Ling Si Ni Tang (Poria Four Reversals Decoction) is indicated; this is irritability due to Yang Deficiency.
In Shaoyin disease, with vomiting, diarrhea, cold limbs, and extreme irritability wishing to die, Wu Zhu Yu Tang (Evodia Decoction) is indicated; this is irritability due to Yin Excess. These are common patterns and not necessarily fatal.
However, if all signs of Jie Xiong (Chest Binding) are present and irritability occurs, death ensues. If there is fever with difficulty in urination, cold limbs, and restlessness preventing lying down, death ensues. In Shaoyin disease, with vomiting, diarrhea, irritability, and cold limbs, death ensues. In Shaoyin disease, with cold limbs, aversion to cold and curling up, absent pulse, no irritability but only restlessness, death ensues. In Shaoyin disease, on the fifth or sixth day, with spontaneous diarrhea, followed by irritability and inability to sleep, death ensues. These are untreatable conditions. Alas, although irritability is a common ailment, it includes these fatal patterns; clinicians must examine carefully.
The Song states: "When irritability comes, Heat Qi rises; Yin is overcome by Yang in the Taiyang Meridian. Yang restlessness connects to stool and umbilical pain in the Kidney; slight Yang deficiency allows Yin restlessness to invade the Heart."
The Heart governs Fire, and the Kidney governs Water. Heart Heat causes Fan (mental irritation); Kidney Heat causes Zao (physical restlessness). In Cold Damage Fan Zao, there are distinctions of Yin/Yang and Deficiency/Excess. If Yin is deficient and Yang is excessive, Heat is severe... [Text fragment] ...Gui Zhi Tang (Cinnamon Twig Decoction), Bai Zhu (Atractylodis Rhizoma), and Gan Cao (Licorice Root) are used, along with Black Bean Decoction to resolve it. After sweating, if there is irritability and insomnia with thirst, give small amounts of water. If urination is difficult, use Wu Ling San (Five-Ingredient Powder with Poria). After purgation, if there is no vomiting or thirst day or night, and the exterior sign shows a deep and faint pulse, use Gan Jiang Fu Zi Tang. If accompanied by Ao Nao (Oppressive Discomfort), use Zhi Zi Chi Tang (Gardenia and Fermented Soybean Decoction). If after sweating or purgation the disease persists with irritability, use Fu Ling Si Ni Tang. After vomiting, sweating, or purgation, if there is empty irritability, epigastric fullness, Qi rushing to the chest, dizziness, muscle twitching, and body tremors, [Text fragment] ...Gui Zhi, Bai Zhu, and Gan Cao are involved... After recovery, if the Stomach is weak and cannot handle food, irritability may result; reducing food intake brings relief, still using [Text fragment] ...Tang.
When Yin reaches its extreme, restlessness may occur. Or in Yin Toxin (Yin Du, 阴毒) treated with fire ironing, or prolonged use of hot herbs causing restlessness, cool herbs must not be mistakenly used; instead, hot agents like Si Ni Tang (Four Reversals Decoction) or Fan Yin Dan (Return to Yin Pill) are used. If there is slight fever, irritability, and a deep and faint pulse, this is called "Yin Syndrome resembling Yang." Yin causes restlessness, Heat causes cold limbs; things reverse when they reach extremes. Generally, the pulse is the key: rapid pulses indicate Heat, slow pulses indicate Cold. This is very accurate. For example, if there is fever, irritability, red face, and a deep and faint pulse, it is a Yin Syndrome. Slight fever indicates Interior Cold. Irritability indicates Yin Excess. Dai Yang (Face Reddening like Dying Yang) is due to lower deficiency... [Text fragment] ...If the physician does not check the pulse and mistakes the restlessness of deficient Yang rising to the diaphragm for Excess Heat, using cool herbs will consume Qi and cause serious illness. *Waitai Miyao* records: Yin Excess causing restlessness is called Yin Restlessness; the patient wants to sit in a well, and hot herbs should be used. Zhang Zhongjing's Shaoyin pattern with a red face is treated with Si Ni Tang plus Cong Bai (Green Onion White).
Cold, fine and deep rapid pulse, irritability without desire to drink, is called Yin Excess repelling Yang. In Cold Damage Yin Excess repelling Yang, the patient is cold, pulse is fine... [Text fragment] ...irritability without desire to drink is the sign. If they desire to drink, it is not this pattern. Those who do not desire to drink should take Xia Pi Bao Zhi San (Eliminate Yin Preserve Yang Powder). Soon, the restlessness will move outward and recover. This herb disperses Cold Qi, allowing Heat Qi to rise, and cure follows sweating. Yan Huo San (Flame Powder) and Dan Sha Wan (Cinnabar Pill) also treat this.
Ge Yang (Repelled Yang) pattern: patient is cold, pulse is fine, deep, and rapid, irritability without drinking. It is said: Yin Toxin generally stems from Kidney Qi deficiency and Cold. Spleen... [Text fragment] ...external contraction of Wind-Cold causes Yang Qi to fail to guard, leading to headache, heavy waist, abdominal pain, eye pain, body fatigue, cold limbs, or much irritability/thirst, vague spirit. If cool herbs are mistakenly taken, thirst worsens, restlessness intensifies. Patients with this condition should immediately take acrid-hot herbs. Red face, blue-black nails. If Yin Toxin has infected, the disease feels heavy and hot, the six pulses are palpable deep against the bone but disappear with light pressure... [Text fragment] ...Cong Yun Method (Green Onion Moxibustion) or Ai Moxa... if hands and feet remain cold after three days, it is untreatable. Shou Zhen said: If heat persists after purgation, or... [Text fragment] ...deep and urgent, causing Yang Qi to be constrained and unable to circulate to the surface and limbs, resulting in whole-body coldness and unbearable pain. Red pain, dizziness, vagueness, dry or painful throat, dry thirst, false sweat, vomiting, diarrhea, abdominal fullness, real irritability, oppression, panting... [Text fragment] ...extreme depth, with pulse deep, fine, and nearly absent, seems to misunderstand the principles of creation, and irritability transmits to Yin Toxin. Or failure to purge, extreme Heat leading to coma and impending death. Urgent purging would cause residual Yin to abruptly cease and Yang to die after calming. Not purging also leads to death. Patient... [Text fragment] ...How should the treatment be? Answer: Use Liang Ge San (Cool the Diaphragm Powder) or Huang Lian Jie Du Tang (Coptis Detoxification Decoction) to nourish Yin and retreat Yang. Only wish for the false Heat to gradually... [Text fragment] ...fullness, pulse gradually returns, until the pulse has some strength, then use San Yi Cheng Qi Tang (Three-One Bowel-Purging Decoction) to mildly purge. Or Jie Du plus Da Cheng Qi Tang... [Text fragment] ...understand this, thus mistaking it for Yin Syndrome, causing Yin-Yang treatment errors.
