The symptoms of Shanghan (Cold Damage) Jing disease (Spasm disease characterized by stiffness and convulsions) are: fever in the body, coldness in the feet, stiffness and rigidity of the neck and nape, aversion to cold, heat in the head and face, shaking of the head, sudden locking of the jaw (Trismus), and opisthotonos (backward arching of the back).
If this condition presents without aversion to cold, it is Rou Jing (Soft Spasm). Palpation reveals a deep and fine pulse; this confirms Rou Jing. Another discussion states that excessive sweating in Taiyang disease (Disease of the Bladder Meridian/Superficial Exterior Syndrome) can lead to Jing disease. The symptoms include head shaking, fever, stiffness and urgency of the neck, rigidity of the waist and body, or convulsions (Chirping and Uncoiling) and trismus. It is specifically called Gang Jing (Hard Spasm) because it is characterized by anhidrosis (absence of sweating) and aversion to cold.
The symptoms of Rou Jing are: head shaking, fever, stiffness and urgency of the neck, rigidity of the waist and body, or convulsions and trismus, which are identical to Gang Jing. However, it is called Rou Jing specifically because it presents with spontaneous sweating and no aversion to cold.
This disease is termed "Jing" (Spasm/Stiffness) because "Jing" refers to a category of diseases where ancient physicians specifically used the term "stiffness and rigidity" to describe it. Furthermore, regarding convulsions caused by Shanghan: how should they be understood? "Chi" (Convulsive contraction) refers to tense tendons and vessels; "Zong" (Relaxing extension) refers to relaxed tendons and vessels. Tension causes drawing and contraction; relaxation causes stretching and extension. When there is alternating contraction and extension, with continuous movement, it is named "Chi Zong" (Convulsions). Commonly known as "Chu" (Twitching), it indicates a deep pathological state. The *Neijing* (Classic of Internal Medicine) states: "When pathological conditions cause tendons and vessels to pull each other and become urgent, it is called Chi Zong." Here, "Qi" implies fitting together tightly, moving slowly but with a tight body; "Cong" implies letting go freely, like releasing restraint. Taking tension as Chi and relaxation as Zong makes the principle very clear.
Chi Zong is a type of wind disease; epilepsy also manifests with Chi Zong. Convulsions in Shanghan occur when pathogenic qi becomes extremely abundant. Excessive heat allows wind pathogens to invade the meridians and collaterals. Since wind is active in nature, it causes the limbs to undergo Chi Zong restlessly. Therefore, in Wind-Warmth diseases treated incorrectly with fire therapy, mild cases result in yellowing of the skin, while severe cases resemble惊痫 (Startle Epilepsy) with frequent Chi Zong. This indicates extreme heat.
In Shanghan, if Chi Zong occurs after the onset of high fever, it means the intense heat has passed, and the condition is often difficult to control. The *Neijing* states: "When the Qi of the Taiyang meridian terminates, one sees eyes rolling upward, opisthotonos, Chi Zong, and sweat emerging like pearls on a string, with the body stiff and immobile. This is Chi Zong, representing a critical stage of advanced disease."
There is also a condition where the four limbs tremble continuously, called "Zhi Xi," meaning the limbs move ceaselessly. It resembles Chi Zong but lacks strength and cannot extend. This is due to the exhaustion of fluids (Sweat/Yin). Although Chi Zeng is difficult to cure, if one uses formulas that dispel wind and clear heat to subdue the excessive heat, recovery is possible. If moxibustion is wrongly applied, or diaphoretic drugs are used indiscriminately, death will follow swiftly.
The Canon says: "One error may prolong life for a day; two errors will hasten the end of life."
Song Verse:
Originally, Jing disease belongs to the Bladder meridian.
With trismus resembling epilepsy and opisthotonos.
It is caused by wind injury and cold-damp invasion,
Thus divided into two syndromes: Soft and Hard.
No sweat indicates Hard Jing; use Gegen Tang (Kudzu Decoction).
Sweat indicates Soft Jing; use Guizhi Jia Gegen Tang (Cinnamon Twig Decoction plus Kudzu Root).
Both types of Jing can generally use Xiao Xuming Tang (Minor Continuation of Life Decoction).
