Epistaxis caused by cold damage refers to the symptom of bleeding from the nasal cavity. Epistaxis caused by miscellaneous diseases is attributed to heat in the interior. Epistaxis caused by cold damage is attributed to heat in the exterior. Why is this so?
The *Pathogenesis* states: The liver stores blood, and the lung governs qi, opening into the nose. When blood encounters heat, it scatters; following the upward movement of qi, it flows out from the nasal cavity, which is termed "nu" (epistaxis). Therefore, epistaxis from miscellaneous diseases is attributed to heat in the interior.
The *Classic* states: In cold damage, if the pulse is floating and tight, and sweating is not induced, epistaxis may result; Guizhi Tang (Aged Poria and Cinnamon Twig Decoction) is appropriate. If a patient with cold damage has not had a bowel movement for six or seven days, accompanied by headache and fever, Chengqi Tang (Gastrointestinal Releasing Decoction) should be administered. If the urine is clear, it indicates that the pathogen is not in the interior but remains in the exterior; sweating should be induced. Patients with headaches will likely experience epistaxis; Guizhi Tang (Aged Poria and Cinnamon Twig Decoction) is appropriate. From this, we see that epistaxis in cold damage is attributed to heat in the exterior.
The *Qianjin Yi Fang* states: There are three types of hematemesis. One is lung abscess, two is stomach injury, and three is internal epistaxis. Since hematemesis is called internal epistaxis, bleeding from the nasal cavity can be called external epistaxis. This is due to the reckless movement of blood in the meridians. When heat in the meridians is excessive and yang qi is congested and heavy, it forces blood to move recklessly, exiting through the nose as epistaxis.
The *Classic* states: The patient feels vexed and their eyes are dim; in severe cases, "xin nu" (severe epistaxis or mental confusion due to heat) occurs, and the condition resolves after epistaxis. This is because yang qi is congested and heavy.
It is also stated: Excessive yang leads to a desire for epistaxis, while yin deficiency leads to difficult urination. Here, epistaxis refers to the vexation caused by excessive yang in the meridians. In cold damage, if the pulse is floating, the nose is dry, the mouth is parched, and the patient only wants to rinse their mouth with water without swallowing, this indicates an impending epistaxis.
The *Classic* states: In Yangming disease, if the mouth is parched and the nose is dry, and the patient can eat, epistaxis will occur.
There are also cases where epistaxis results from forced sweating when it was not indicated. The *Classic* states: In Shaoyin disease, if there is only cold extremities without sweat, forcing sweating will surely disturb the blood. It is unknown from which channel the blood will exit; it may come from the mouth or nose, or from the eyes. This is termed "lower jue and upper exhaustion" (cold jue at the lower part and exhaustion of yin blood at the upper part), which is difficult to treat.
Patients with a history of epistaxis ("nu jia"), although having pathogenic heat in the meridians, should not undergo sweating therapy. The *Classic* states: Patients with a history of epistaxis should not undergo sweating. If sweating is induced, it will cause depression of the forehead, urgent pulse, fixed gaze, inability to close the eyes, and insomnia. The previously mentioned use of Guizhi Tang (Aged Poria and Cinnamon Twig Decoction) and Mahuang Tang (Ephedra Decoction) to treat epistaxis is not directly treating the epistaxis itself, but rather dispersing pathogenic qi in the meridians. If pathogenic qi cannot be dispersed and accumulates in the meridians, pressing on the blood, it leads to epistaxis. Thus, Guizhi Tang and Mahuang Tang are not specifically for treating epistaxis.
In Taiyang disease, if the pulse is floating and tight, there is fever, and the body has no sweat, spontaneous epistaxis leads to recovery. This is because the pathogen in the meridians dissipates with the epistaxis, leading to resolution. Therefore, it is known that after epistaxis, one should not use Guizhi Tang (Aged Poria and Cinnamon Twig Decoction) or Mahuang Tang (Ephedra Decoction) to disperse.
If an epistaxis patient has only head sweating, no body sweating, or sweating that does not reach the feet, the prognosis is fatal. Huangdi considers these conditions treatable with purgation. At the time of illness, careful examination and treatment will prevent errors.
The verse says: Spontaneous epistaxis in Taiyang disease indicates the illness is about to resolve; after epistaxis, carefully examine the pulse. If there is sweat and a floating pulse, Guizhi Tang may be used; if there is no sweat and a tight pulse, use Mahuang Tang. Here, "Guizhi" refers to Guizhi Tang (Aged Poria and Cinnamon Twig Decoction), and "Mahuang" refers to Mahuang Tang (Ephedra Decoction). In Taiyang disease, spontaneous epistaxis, or epistaxis occurring after taking various exterior-releasing medicines, indicates the illness is resolving. If there is sweat and a floating pulse, use Guizhi Tang again; if there is no sweat and a tight pulse, give Mahuang Tang again. If the pulse is faint, use Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction), or Huangqin Shaoyao Tang, or Maohua Tang.
If epistaxis is accompanied by thirst and desire to drink, but vomiting occurs immediately after drinking water, first administer Wuling San (Poria Decoction), then Zhuye Tang (Bamboo Leaf Decoction). In Yin patterns, there is no fever but there is bleeding; in Shaoyin disease, there are only cold extremities without sweat. Forcing sweating will surely disturb the blood, leading to death. Blood may exit from the anus, nose, or ears/eyes. This is termed "lower jue and upper exhaustion," which is difficult to treat.
The *Shengji Zonglu* discusses: Cold damage epistaxis is caused by excessive heat causing reckless blood movement. The heart governs blood, the liver stores it, the lung governs qi, and the nose vents it. When the heart and liver are injured by pathogenic heat, blood follows qi and exits through the nose. Ancients said "excessive yang leads to epistaxis," meaning excessive yang leads to excessive heat, and excessive heat leads to epistaxis. Thus, in Taiyang pattern of cold damage, epistaxis resolves the disease. As for Yin diseases, epistaxis is not suitable. Since Yin patterns lack heat, how can epistaxis occur? Therefore, in Shaoyin disease, with only cold extremities and no sweat, forcing sweating disturbs the blood, exiting from mouth, nose, ears, or eyes, termed "lower jue and upper exhaustion," which is difficult to treat.
The verse says: Yangming disease with dry mouth and headache, water not swallowed leads to epistaxis. Stagnant blood causes mania; the patient has no exterior cold or heat symptoms.
In Yangming disease, with body heat, headache, and epistaxis, if the pulse is faint, use Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction). If there are no exterior cold or heat symptoms, mania will definitely occur, indicating stagnant blood. For mild cases, use Rhinoceros Horn, Tao Ren, and Chengqi Tang; for severe cases, use Didang Tang (Ephedra, Myrobalan and Dogbane Decoction), until black material is expelled.
