The Heart governs the Shen (Spirit). The Shen is the root of the concentrated essence of the five Zang organs. It acts as the Emperor, leading the four directions. In summer, it corresponds to seventy-two days and is located in the Li Palace of the South.
It belongs to Fire.
Therefore, Heart Qi connects with the tongue. When the tongue is harmonious, one can discern the five flavors. The Heart opens into the ears (Note: While traditionally associated with the tongue, some ancient texts link the Heart to the ears or suggest the tongue's function manifests through the ears). The Heart belongs to Fire, while the Kidney belongs to Water. Water and Fire mutually assist each other, and Heart Qi connects with the tongue. The tongue is not a direct orifice; its manifestation through an orifice is寄 seen in the ears. The left ear corresponds to Bing Fire, and the right ear to Ding Fire, circulating in the Yan Palace, emerging upwards from the lips and mouth, perceiving flavor glory in the ears. Externally, the Heart governs Blood; internally, it governs the Five Tones. The Heart weighs twelve liang, contains three hairs and seven holes, and holds three he of jingzhi (essence fluid). The name of the Shen is "Shen," governing the spirit, known as the residence of the Five Spirits. It responds to seasonal changes. Thus, it is said that the Heart stores the Mai (Vessels), and the Vessels house the Shen. In Qi, it manifests as swallowing; in fluids, as sweat. When Heart Qi is deficient, there is endless sorrow; when excessive, there is unceasing laughter. When Heart Qi is deficient, one dreams of saving fire or phallic objects; if it coincides with the season, one dreams of burning and scorching. When Heart Qi is excessive, one dreams of joy, laughter, fear, and dread. When Jue Qi (rebellious Qi) affects the Heart, one dreams of hills, mountains, smoke, and fire. Generally, the Heart's image is like Fire, combining with the Small Intestine to form a Fu organ. Its meridian is the Hand-Shaoyin, exterior-interior related to the Taiyang. Its pulse... (text missing here) corresponds to Spring and prospers in Summer. In summer, all things are grand and abundant; hanging branches and spreading leaves all droop downwards like hooks, hence named "Hook." The Heart pulse is Hong (Surging) and Da (Large) and Chang (Long). Hong indicates Wei Qi (Defensive Qi) is substantial; when substantial, Qi cannot exit easily. Da indicates Ying Qi (Nutritive Qi) is sprouting. Sprouting and Hong contend with each other, allowing for sweating, hence named "Chang" (Long). When Chang and Hong combine, they guide water and grain to irrigate the meridians, moistening the skin and fluids. The Taiyang pulse being Hong and Da represents the mother body. Fortunately aided by Wu and Ji Earth Qi, it firmly establishes the root. Yang Qi emerges upward, sweat appears on the head. The five internal organs become dry, and the Bao (Uterus/Bladder) becomes empty. If the physician again uses purgation, this constitutes double deficiency. A floating pulse indicates exterior syndrome without interior involvement. Yang pathogen enters and harms its mother organ.
Summer pulses resemble hooks. Summer pulses correspond to the Heart; the South belongs to Fire. This is why all things grow grandly. Therefore, the Qi comes strong and departs weak, hence called "Hook."
Those contrary to this have disease.
Disease is internal.
Heart pulse coming... (text missing here) numerous like connected pearls, like touching Langgan jade, is called Ping (Normal). Summer is based on Stomach Qi. Heart pulse coming rapidly connected, slightly curved within, is called Heart Disease. Coming first curved then stationary, like holding a belt hook, is called Heart Death. True Heart pulse arrives, hard and beating, like touching coix seeds, numerous. Complexion is red-black and lusterless; hair breaks and dies. Summer with slight Stomach Qi and Hook is Ping. More Hook less Stomach Qi is Heart Disease. Only Hook no Stomach Qi is Death. Stomach Qi with Shi (Stone) pulse is Winter disease. Severe Shi pulse is current disease, dying in Winter. Hand-Shaoyin Qi exhaustion leads to blocked vessels. Shaoyin is the Heart vessel. The Heart is the convergence of vessels. Blocked vessels mean Blood does not flow. Blood not flowing means hair lacks luster. Face black like lacquered charcoal indicates Blood died first. Ren day worsens, Gui day dies, because Water overcomes Fire. Signs of Heart death: superficial palpation feels solid, like beans hitting the hand; pressing makes it more restless indicates death. In Summer, Heart Fire prospers; pulse floating, large, and scattered is Ping. Contrarily obtaining Xian (Wiry) and Xi (Fine)... (text missing here) returning to Zi (Child), is Deficient Evil; although sick, easy to treat. Contrarily obtaining Da and Huan (Slow) is Spleen overcoming Heart, Child overcoming Mother. Contrarily obtaining Chen (Deep), Ru (Soggy), and Hua (Slippery) is Kidney overcoming Heart, Water overcoming Fire, acting as Thief Evil, great reversal, ten deaths untreatable. Lung overcoming Heart, Metal suppressing Fire, is Minor Evil; although sick, it heals quickly. Kidney overcoming Heart必然 causes Lin (strangury/difficulty urinating). Left hand Guan front Cun Kou Yin exhaustion means no Heart pulse. Bitter pain under the heart, heat in palms, frequent vomiting, oral ulcers. Puncture Hand-Shaoyang... (text missing here). If there is Shui Qi (Water Qi) under the heart, triggered by worry and resentment, puncture Hand-Jueyin (Pericardium), treating Yin. Heart pulse coming numerous like threaded pearls, slippery and smooth, two beats is Ping, three beats is Jing-Li disease, four beats Jing-Tuo (Essence depletion), five beats Death, six beats Life End. Hand-Shao... (text missing here) Heart pulse extremely urgent causes convulsions. Slightly urgent causes Heart pain radiating to back, inability to eat. Extremely slack causes manic laughter. Slightly slack causes Fu Liang (Hidden Beam) under the heart, moving up and down. Has... (text missing here) slightly slippery causes Xin Han (Heart Hernia) radiating to umbilicus, lower abdomen rumbling. Extremely choppy causes aphonia. Slightly choppy causes blood extravasation and limb coldness, tinnitus, epilepsy.
Heart pulse beating hard and long indicates tongue curling and inability to speak. Soft and scattered indicates Xiao Ke (Wasting-Thirst) resolving itself.
Red-colored pulse arriving, panting and hard, diagnosis says: accumulation of Qi in the middle, occasionally hindering food intake, named Xin Bi (Heart Obstruction), obtained from external illness, thinking and worrying causing Heart deficiency, thus... (text missing here)
Heart in sound is laughter, in change is worry, in emotion is joy. Joy harms the Heart. Essence and Qi merging into Heart causes joy. Heart deficiency causes sorrow; sorrow causes worry.
