[Appendix Discussion]
Qibo stated: When a girl reaches seven years of age, the Qi of the Kidney becomes abundant; her teeth change and hair grows. At fourteen (two sevens), Tian Gui (substances that promote reproductive maturation) arrives; the Ren Mai (Conception Vessel) becomes unobstructed, and the Chong Mai (Penetrating Vessel) becomes vigorous; menstruation occurs at regular intervals.
These two vessels... are the root of menstruation. If pathogens affect the Heart, Lungs, back, hypochondrium, hands, and feet, symptoms will manifest similarly during menstruation. Extreme anger damages the Liver, leading to dizziness, hypochondriac pain, hematemesis, scrofula, and sores/ulcers. [If left untreated], it fails to heal, resulting in myriad complications that are indescribable. This is what is meant by "violating seasonal pathogens, initially minute as autumn down, but becoming heavy as a mountain upon contracting disease." Can one not be reverent?
The five flavors of food nourish tendons, marrow, bones, flesh, skin, and hair. Men belong to Yang; within Yang there must be Yin, and the number for Yin is eight. Thus, at eight years old (one eight)... spirit... sufficient ear... therefore... fetus how... disease. Yin... vaginal traction pain... year thinking male... if men have excessive intercourse, fluids dry up and the person becomes deficient; if women bear many children or nurse excessively, blood dries up and leads to death. Observing their essence and blood, one understands half of the matter.
Generally, treating disease first involves regulating its primary aspect. For men, regulate Qi; for women, regulate Blood. Qi and Blood are the spirit of humans; they must not be neglected in care. However, women... period refers to... wind pathogen... alternating cold and heat. Abdominal distension and pain, difficulty conceiving. Cold and heat in the uterus over time cause labor damage, inevitably accompanied by leukorrhea, frequent dribbling urination, and sudden metrorrhagia. The Classics state abdomen like... unobstructed and... thus its... little.
As for irregular menstruation in women, it arises from labor injuring Qi and Blood, causing constitutional deficiency where Wind-Cold pathogens take advantage. If Wind-Cold pathogens lodge in the interior of the uterus, damaging the Chong and Ren vessels... Heart's... then month... if cold... thus... time... Qi... The Classics say when heaven is hot and earth is warm, menstrual water boils over, hence causing fluctuating abundance and arriving before the expected date. If one knows Yin and Yang, regulates Qi and Blood, preventing mutual excess, peace brings fortune. *Shengji Zonglu* states: Irregular menstruation means the channels are either more or less, clear or turbid, coming early or arriving late. This is generally due to neglecting regulation, leading to deficiency and damage of the Chong and Ren vessels, causing Tian Qi to deviate from its normal degree. Thus, the *Neijing* says: When the Ren Mai is unobstructed and the Chong Mai is vigorous, menstruation comes on time. This implies it has a regular degree. Wei Shi's *Defang Fang* states: At fourteen, Tian Gui arrives; at forty-nine, Tian Gui exhausts. Early onset with cunning nature, late onset with dullness. Unimpeded flow allows Yin and Yang to harmonize, enabling conception. Girls should menstruate at fourteen; if not by twenty, life is like a candle in the wind, precarious morning to evening. Disease onset leads to death; rare survivors are one in a hundred. This also causes lifelong illness without a day of ease. However, quarterly menstruation is acceptable, as is annual menstruation, though not ideal. Those who never follow the proper path face difficult-to-treat diseases in old age. Verification says: Girls menstruate at fourteen; boys emit seminal essence at sixteen. These conform to the numbers of Yin and Yang, each reaching their time. Thus, male essence should be abundant; female menstruation should be regulated. The way to regulate menstruation lies in consuming Qi to move Blood. When Blood is abundant and Qi transforms smoothly, this is called "following." Following ensures no disease arises and fertility remains intact. Moreover, women's diseases, caused by four-season influences and Six Excesses/Seven Emotions, are treated largely the same as men's. Only conditions before/during pregnancy, postpartum, seven syndromes, eight masses, metrorrhagia, and leukorrhea differ. Therefore, my prescribed formulas investigate causes, mostly stemming from irregular menstruation generating various syndromes. Generally, women's health is secure when menstruation is on time. If delayed, one must examine cold, heat, deficiency, and excess to regulate. Early arrival indicates Blood Heat; method requires clearing. Late arrival indicates Blood Cold; method requires warming. Yet, one must check for external pathogens and distinguish cold from heat before administering medicine. Blood is a Yin substance, resembling the moon's waxing and waning, arriving on time, hence called "Moon Water." Women's Chong and Ren Qi and Blood, with unharmonized meridians, result in blood arriving when it shouldn't or not arriving when it should, intermittent, varying in amount, with different colors—this is irregular menstruation.

Yuyuliang Wan (aka Zishiying Wan, from *Yifang Dacheng*)
Discussion: Menstrual timing is a constant indicator, observing the fluctuation of whole-body Yin and Yang to know safety or danger. Hence, it must come monthly. Excess or deficit constitutes irregularity. Excess Yang causes early arrival; excess Yin causes late arrival. Fluctuating amounts, cessation, or unstoppable bleeding also stem from Yin-Yang imbalance and Cold-Heat pathogens. Detailed below: If timing fluctuates, early or late, with intermittent umbilical/abdominal pain and lackluster complexion, long-term neglect leads to deficiency, sterility, and other diseases. Treating women's diseases involves tonifying and warming the lower origin, but one must know all diseases stem from irregular menstruation and Yin-Yang conflict. If Yin attacks Yang, the uterus becomes cold, blood does not circulate. As the Classics say: Heaven cold, earth frozen, water turns to ice, hence scarcity and late arrival. If Yang attacks Yin, blood flows diffusely. As the Classics say: Heaven hot, earth warm, menstrual water boils over, hence abundance and early arrival. Know Yin and Yang, regulate Qi and Blood, and diseases will not arise.
