Jing (Startle) and Ji (Palpitation) are different conditions. Yang Renzhai stated that his words were concise yet comprehensive in principle, so this point is raised first to clarify the beginning of the discussion. The palpitation described as "beating like a pestle" (Zhu Zhu) is a different meaning and naturally differs from what is discussed here.
General Discussion (Nomenclature): What governs the human body is the Heart (Xin), and what nourishes the Heart is Blood (Xue). Once Heart Blood becomes deficient, the Shen (Spirit) fails to reside securely; this is the origin of Jing and Ji. Can we fail to distinguish between Jing and Ji? Jing refers to terror/fear. Ji refers to Zheng Chong (palpitations with a sense of unease). If there is Heart deficiency with stagnant Phlegm (Tan), then hearing loud noises, seeing strange objects, encountering danger, or being confronted with peril causes the Shen to be disturbed, leading to a state of timidity and apprehension; this is Jing. If there is Heart deficiency with retained Water (Shui), then there is a gurgling sensation in the chest, and Qi moves vacuously. Since the pathogenic Water rises to oppress the Heart, and Heart Fire dislikes Water, the Heart feels insecure, leading to a state of dissatisfaction; this is Ji. For Jing, administer formulas that resolve Phlegm and calm Startle. For Ji, administer formulas that expel Water and eliminate Fluid retention (Yin). (Supporting Deficiency) merely involves nourishing Heart Blood and harmonizing Heart Qi. If one uses acrid-warm and drying herbs indiscriminately, it may cause Heart Fire to flare up spontaneously, leading to symptoms of Heat generating Wind. (From *Zhi Zhi*) [Note: *Shuo Wen* defines Ji as heart movement. This is the basis of Yang's theory.]
Ji is synonymous with Zheng Chong. Zheng Chong means that without external startle, the Heart beats restlessly on its own. Jing occurs due to sudden external stimulation. (From *Zhun Sheng*) [Note: The original text contains a differentiation between Jing and Kong, now placed at the end of the chapter for reference.]
*Sang Han Ming Li Lun* explains the character Ji as follows: Ji is Xin Song (heart palpitation/unease), beating and trembling, presenting as Zheng Zhong Song Song, unable to feel at ease. Thus, Ji is Zheng Chong, and dividing them into two separate categories today is erroneous. (Ibid.)
Jing Ji arises from Essence loss: *Yang Sheng Fang* states: Essence is stored in the Jade Chamber (genitals); excessive intercourse leads to Essence loss. Those who lose Essence become despondent and suffer from frequent Jing Ji. (From *Bing Yuan Lun*)
Origin of Jing Disease: Feng Jing (Wind Startle) occurs when the constitution is weak and Heart Qi is insufficient, allowing Wind Pathogens to invade. The Heart stores the Shen and governs Blood vessels. Insufficient Heart Qi leads to deficiency; deficiency leads to chaotic Blood flow; chaotic Blood causes Qi to congregate with Blood. With Qi and Blood mingling, and further invaded by Wind Pathogens, one becomes startled and unsettled, named Feng Jing. (From *Bing Yuan Lun*) [Note: There are also Feng Jing Xie Hou, Feng Jing Ji Hou, and Feng Jing Kong Hou, all sharing the same general purport. Feng Jing Xie Hou describes symptoms as alternating startle and joy, with恍惚 (trance-like confusion).]
Jing Ji is caused by Heart deficiency and Gallbladder cowardice. The Heart is the Sovereign Official, from whom Shen emerges. The Gallbladder is the Official of Uprightness, from whom Decision emerges. When Heart Qi is tranquil and Gallbladder Qi is not fearful, decision-making and thought are proper. However, if one experiences great shock from events, hears phantom sounds, sees strange sights, climbs heights, or traverses dangers, the Shen and Heart are disturbed. Qi and Saliva become stagnant, resulting in Jing Ji. If Jing Ji persists, various syndromes arise: shortness of breath and fatigue, body weariness and spontaneous sweating, edema of the four limbs, loss of taste, Heart deficiency with vexation and oppression, restlessness in sitting and lying down. These are all signs of Heart deficiency and Gallbladder cowardice. The treatment method is to calm the Heart to strengthen Gallbladder Qi; there will be no case that does not heal. (From *Ji Sheng*) [Note: Yan Shi has a separate treatise on Zheng Chong; this section focuses on Jing disease.]
Jing Ji belongs to Blood deficiency. Jing Ji occurring intermittently (Note: Zheng Chong is described as continuous, distinguishing them by contrast) should be treated with Zhu Sha An Shen Wan (Cinnabar Calming the Spirit Pill). (From *Dan Xi*)
Origin of Ji Disease: Zheng Chong arises from insufficiency of Heart Blood. The Heart governs Blood; Blood is the master of the Heart, and the Heart is the ruler of the form. When Blood is abundant, the Heart Ruler is at peace. Often, due to anxiety over wealth and honor, sorrow over poverty and low status, prolonged longing for loved ones, or unexpected disappointments, True Blood is consumed, and the Heart Emperor loses support, leading to Zheng Chong. If Zheng Chong persists, various syndromes arise: stiff tongue, trance-like confusion, propensity to worry and grief, sallow complexion. These are all signs of Heart disease. *Nan Jing* states: "Damage to the Heart requires regulating Ying (Nutritive Qi)." The method should focus on tonifying True Blood. If True Blood is abundant, the Heart Ruler has support, and healing is assured. Additionally, exposure to Wind, Cold, Summer-Heat, or Dampness, blocking the meridians, can cause Zheng Chong. Accumulation of the Five Drinks obstructing the Middle Jiao also causes Zheng Chong. Treatment should follow the specific syndrome. (From *Ji Sheng*)
Restlessness in the chest and heart is called Zheng Chong, commonly known as Xin Song, both caused by Heat. (From *Yuan Bing Shi*)
Zheng Chong belongs to Blood deficiency; Zheng Chong without fixed timing is mostly due to Blood scarcity. Movement triggered by thought indicates deficiency. Intermittent onset and cessation indicate Phlegm stirred by Fire. Thin individuals often suffer from Blood deficiency; obese individuals belong to Phlegm. Common cases are mostly Phlegm. True palpitation indicates Blood deficiency; use Si Wu Tang (Four Substances Decoction) or Zhu Sha An Shen Wan. If the cause is Startle, Startle causes the Shen to leave its residence; emptiness leads to Phlegm generation. (From *Dan Xi*)
The origin of Xin Ji (Heart Palpitation) falls into two categories: one is Deficiency, the other is Fluid retention (Yin). Qi deficiency arises from internal deficiency of Yang Qi, emptiness below the Heart, and internal movement of Fire causing Ji; Blood deficiency is similar. Cases of retained Yin involve Water stopping below the Heart. The Heart belongs to Fire and dislikes Water; since Water stops internally, the Heart feels insecure, thus causing Ji. (From *Zhun Sheng*)
Pulse Signs: The Fu Yang pulse is faint and floating. Floating indicates Stomach Qi deficiency; faint indicates inability to eat. This is the pulse of fear, caused by anxiety and pressure. In patients suffering from Jing disease, the pulse stops and then returns; their eyes do not move, and they cannot exhale. (From *Mai Jing*)
Various Treatment Methods: Qi stagnation generating Phlegm and spontaneous sweating: use Wen Dan Tang (Warm the Gallbladder Decoction) or *Jian Yi* Xiong Zhu Wan. Great shock from events, ominous dreams: use *Ji Sheng* Yuan Zhi Wan, Ning Zhi Gao, or Suan Zao Ren Tang. Loss of Essence from sexual activity causing despondency and Jing Ji: use Shi Si You Wan, taken before sleep on an empty stomach, or Da Tu Si Wan. (From *Yong Lei*) [Excerpt. Shi Si You Wan and Da Tu Si Wan are major formulas for dual tonification of Heart and Kidney.]
For Jing Ji with many strange dreams and immediate awakening upon dreaming: use Wen Dan Tang plus Suan Zao Ren (Sour Jujube Seed) and Lian Rou (Lotus Seed Meat), each 1 qian, decocted with Gold and Silver, followed by Shi Si You Wan or Zhen Xin Dan (see *He Ji Ju Fang*), Yuan Zhi Wan (using Yuan Zhi, Fu Shen, Yi Zhi Ren), or Miao Xiang San mixed with wine. (From *Yao Jue*)
To treat Jing Ji, one must first use formulas to nourish the Heart and calm the Shen, followed by resolving Phlegm. Alternatively, if the stool is constipated and the pulse is excess, use Zhu Sha Gun Tan Wan to purge. If one dose is ineffective, a second dose will surely cure. (From *Yi Tong*)
The Liver produces planning; the Wandering Soul scatters and resides; it fears movement and is startled. Treatment focuses heavily on the Liver Meridian. The Gallbladder governs decision; when Will (Zhi) is not expressed, one is startled by events. Treatment focuses heavily on the Gallbladder Organ. Anger injuring the Liver or Startle Qi entering the Gallbladder can cause the Mother (Liver/Wood) to make the Child (Heart/Fire) deficient, leading to Heart Blood insufficiency. Or, excessive desires and endless thoughts scatter the Shen and disturb the Heart Ruler. This is why treatment sometimes originates from the Liver and Gallbladder. (Ibid.)
Stagnant Phlegm and retained Yin accumulating in the Pericardium and Stomach gate can cause Jing Ji and Zheng Chong. This cannot be treated generally as deficiency. Physicians must investigate pulse signs and prescribe accordingly to achieve success. (Ibid.)
Susceptibility to Startle belongs to Gallbladder Qi deficiency. Avoid Qi-breaking, ascending, dispersing, and drying-hot herbs. Instead, tonify Gallbladder Qi using sweet-warm, acrid-warm, and sour-neutral herbs. (From *Ben Cao Jing Shu*)
For Phlegm, resolve Phlegm and calm Startle; for Yin, expel Water and eliminate Fluid retention; for Blood deficiency, nourish Heart Blood; for Qi deficiency, harmonize Heart Qi; for Phlegm knots, descend them; for Qi stagnation, soothe them. If Yin Fire flares upward, treat the Kidney, and Heart palpitation will cease spontaneously. If startled by external objects suddenly, use heavy sedatives. (From *Hui Bu*)
If the cause of Jing is Liver and Gallbladder dual deficiency, and hundreds of medicines fail, one must tonify the Kidney. Ancients said the Liver has no deficiency and cannot be tonified; tonifying the Kidney is indeed tonifying the Liver. (From *Zun Sheng Shu*)
If Yin Fire rushes upward, causing persistent Zheng Chong, and severely, Fire flares upward causing dizziness, blurred vision, tooth loss, hair loss, fingers feeling enlarged, seeing strange objects, or sounds in the abdomen, this is Yin Fire pathology. Treat with Zi Yin Yi Huo Tang (Nourish Yin and Suppress Fire Decoction). For restless Heart, add Heart-nourishing agents. If long-term use of Fire-descending medicines fails, add Fu Zi (Aconite Root) for counter-treatment, or add Ren Shen (Ginseng). (From *Zhun Sheng*)
Differentiation of Treatment for Jing and Kong: Startle causes Qi chaos; Fear causes Qi sinking. Although Jing and Kong seem similar, one must know that Kong harms people more severely than Jing. Why? Jing is temporary, and temporary conditions recover quickly; Kong accumulates gradually, and gradual accumulation is hard to resolve. It can even lead to Heart palpitation and Shen damage, Essence cowardice and impotence, wasting away month by month until death. Only those with great courage and decisiveness can remove the root of this disease; relying solely on medicine is difficult. I have treated acute Startle, curing 8 or 9 out of 10; treating Fear, curing only 1 or 2 out of 10. (From *Jing Yue*)

Take 15 pills, swallowed with saliva, after meals. (From *Lan Shi Mi Cang*) Another Zhu Sha An Shen Wan removes Di Huang and Dang Gui from this formula. Dan Xi said: For those with Heart Qi deficiency and cowardice, experiencing Zheng Chong, vexation, forgetfulness, or mental confusion after losing the mind, use Chen Sha An Shen Wan. Another Zhu Sha Wan treats palpitation from overwork; remove Di Huang from the base formula, add Shao Yao (Peony), Ce Bai Ye (Biota Leaf), Chuan Xiong (Ligusticum), and Chen Pi (Tangerine Peel). *Xin Tong* Hu Po An Shen Wan (Amber Calming Spirit Pill) treats post-illness vexation and insomnia; add Hu Po (Amber) and Zhen Zhu (Pearl) to the base formula.
