(From *Yijian Fang*) For abdominal pain with vomiting and desire for bowel movement where other medicines cause vomiting, swallowing Sugan Wan soaked in ginger juice is best. (From *Defang*)
In patients with deficiency-cold suffering abdominal pain, warming-tonifying medicines provide temporary relief with intermittent pain. Severe cases may pass clear water or white foam. This indicates deficiency with excess: either retained food in the intestines was not cleared, or weak spleen-stomach failed to transform ingested food, causing stagnation. Even one or two pieces of old stool can cause distress. Method: Decoct warming-tonifying medicine, filter, add one qian of Dahuang (1.5 qian if not very deficient), boil briefly, and take. Retained food will pass, healthy qi remains intact, and disease resolves. This is a proven method. (From *Weiqi Lun*)
In Jiwei year in Jinling, a servant's wife suffered heart-abdominal distension and pain. Physicians prescribed Mugwort, Agarwood, Areca, Betel Nut, Peony, Ginger, Cinnamon, etc., but symptoms worsened. I diagnosed her: pulses were wiry-tight yet harmonious, unlike typical pathological pulses, but her hands felt hot as fire. Thus, I identified it as excess pain. Others asked for treatment. I said: Do not treat stabbing heart-abdominal pain as deficiency-cold immediately. Two physicians argued: "If not cold, what? Heat generates wind, cold generates qi." I replied: Incorrect. *Nanjing* states: Deficiency causes itching, excess causes pain. Zhang Zhongjing states: For abdominal pain, use Guizhi Ji Shaoyao Tang; for severe pain, Guizhi Jia Dahuang Tang. The servant claimed his family constitution was weak. I said: There are distinguishing signs. Press the abdomen during pain. If pain stops, it is deficiency-cold. If pain worsens and cannot be touched, it is excess pain. We pressed; she screamed and could not tolerate touch. I concluded: This is undoubtedly excess-heat. Use Dachaihu Tang. Others disagreed. I proceeded anyway; she recovered after eight doses. (From *Furen Liangfang*)
Danxi treated a youth, slightly sallow since childhood, with severe summer abdominal pain. Physician gave Xiao Jianzhong Tang plus Dingxiang; three doses failed, followed by vomiting clear fluid. Left-hand pulses: deep-wiry-rapid-excess, Guan particularly so. Right-hand: deep-slippery-rapid-excess. Prescribed Dachaihu Tang plus Gancao for four doses to purge. Pain/vomiting reduced, but no appetite. Then Xiaochaihu Tang minus Renshen/Huangqin, plus Shaoyao/Chenpi/Huanglian/Gancao for twenty doses to cure. (From *Lei An*)

Given Dachengqi Tang plus Gui; two doses brought mild relief and pain cessation. Pain returned daily at Shen/You hours (3-7 PM), hard and untouchable. Previous meds provided temporary relief via mild purging. Added Tao Ren paste to previous formula; passed over a sheng of purple-black blood, pain stopped. Pulse improved but blade-like feeling persisted. Added Chuan Fuzi to previous formula; passed five stools, two sheng of purple-black blood like torn cotton, cured. Later, dietary injury caused evening periumbilical pain with harmonious pulse; cured with one dose of Xiao Jianzhong Tang. (From *Lei An*)

A scholar from Zhending, age 31, naturally weak, had qi accumulation in left hypochondrium, avoided cold food, pain upon cold exposure, vomiting clear water, vertigo, inability to open eyes, aversion to noise. Resting 1-2 days or taking acrid-hot medicines reduced illness. In early autumn Jiawu year, due to labor and cold food, illness flared: continuous abdominal pain, cold sweat, cold extremities, cold breath, sallow-dull complexion, unable to lie down, sat leaning on table, cough, throat obstruction. *Neijing* states: Cold lodging in small intestine mesentery/blood collaterals causes blood stagnation, qi/blood retention, forming masses overnight. Cold in stomach/intestines causes rebellious qi upward, hence pain and vomiting. Internal cold pain stops immediately with heat. I prescribed medicine, but he vomited it up. Desperate, I used half jin cooked mugwort on paper on abdomen, layered with split scallions, covered with paper, warmed with slow-fire iron. Changed when cool. If abdomen felt too hot, bound with cotton strips until cool. Initially, warmth reduced pain; great warmth stopped pain; slept at night. Next day, took targeted medicine, largely cured.
