The Master said: Cough originates in the Lung, but it does not exclusively belong to Lung diseases. The Five Zang organs and Six Fu organs, if affected by exogenous pathogens, can all lead to cough. The reason for this is that pathogenic qi rebels upward and must interfere with the Lung; as the organ governing Qi movement, when disturbed, it produces sound, which is cough. To know its source, one must examine the pulse and respiration. I will record these points item by item for you to pass on to future sages.
Lung cough presents with a short and choppy pulse. If the pulse is floating and choppy, it indicates wind pathogen invasion; if tight, short, and choppy, it indicates cold pathogen invasion; if rapid, short, and choppy, it indicates heat pathogen invasion; if urgent, short, and choppy, it indicates dryness pathogen invasion; if soggy, short, and choppy, it indicates dampness pathogen invasion. These are the causes of Lung cough. Its manifestations include wheezing with audible sounds, and in severe cases, hemoptysis (coughing up blood).
Heart cough presents with a large and scattered pulse. If the pulse is floating, large, and scattered, it indicates wind pathogen invasion; if tight, large, and scattered, it indicates cold pathogen invasion; if rapid, large, and scattered, it indicates heat pathogen invasion; if urgent, large, and scattered, it indicates dryness pathogen invasion; if soggy, large, and scattered, it indicates dampness pathogen invasion. These are the causes of Heart cough. Its manifestations include heart pain, a sensation of something stuck in the throat (globus hystericus), and in severe cases, sore throat and throat obstruction (throat bi syndrome).
Liver cough presents with a wiry and choppy pulse. If the pulse is floating, wiry, and choppy, it indicates wind pathogen invasion; if wiry, tight, and choppy, it indicates cold pathogen invasion; if wiry, rapid, and choppy, it indicates heat pathogen invasion; if wiry, urgent, and choppy, it indicates dryness pathogen invasion; if wiry, soggy, and choppy, it indicates dampness pathogen invasion. These are the causes of Liver cough. Its manifestations include pain under both hypochondria, and in severe cases, inability to turn the body; turning causes fullness under the lateral costal regions.
Spleen cough presents with a soggy and choppy pulse. If the pulse is floating, soggy, and choppy, it indicates wind pathogen invasion; if deep, soggy, and choppy, it indicates cold pathogen invasion; if rapid, soggy, and choppy, it indicates heat pathogen invasion; if urgent, soggy, and choppy, it indicates dryness pathogen invasion; if slow, soggy, and choppy, it indicates dampness pathogen invasion. These are the causes of Spleen cough. Its manifestations include pain under the right hypochondrium, radiating faintly to the back, and in severe cases, inability to move; moving aggravates the cough.
Kidney cough presents with a deep and soggy pulse. If the pulse is deep, wiry, and soggy, it indicates wind pathogen invasion; if deep, tight, and soggy, it indicates cold pathogen invasion; if deep, rapid, and soggy, it indicates heat pathogen invasion; if deep, urgent, and soggy, it indicates dryness pathogen invasion; if deep, sluggish, and soggy, it indicates dampness pathogen invasion. The Master said: Diseases include Fengshui (Wind-Water), Pishui (Skin-Water), Zhengshui (Regular-Water), Shishui (Stone-Water), and Huanghan (Yellow Sweat).
Fengshui presents with a naturally floating pulse, joint pain, and aversion to wind. Pishui presents with a floating pulse, edema that leaves indentation upon pressure, no aversion to wind, drum-like abdomen, no thirst; induce sweating. Zhengshui presents with a deep-slow pulse and wheezing. Shishui presents with a naturally deep pulse, abdominal fullness without wheezing; promote urination. Huanghan presents with a deep-slow pulse, fever, chest fullness, edema of limbs/face/head; if chronic, abscesses may form.
Floating pulse indicates Wind;洪 (Hong/Rapid-Large) pulse indicates Qi. When Wind and Qi contend: Wind dominance causes Urticaria (itchy rash); Qi dominance causes Water retention (difficulty bending/straightening, massive edema); sweating leads to cure. Aversion to wind indicates Deficiency (Fengshui); no aversion to wind with free urination and upper Jiao Cold with excess saliva indicates Huanghan.
Deep-slipping pulse at Cun Kou indicates internal water qi, facial/eye swelling with heat, named Fengshui. Swollen eye lids like a silkworm waking, pulsating neck vessels, frequent cough, and pitting edema on hands/feet also indicate Fengshui.
Taiyang Disease with floating-tight pulse usually causes joint pain. If pain is absent but body is heavy/sore, non-thirsty, this is Fengshui; sweating cures it. Aversion to cold indicates extreme Deficiency caused by improper sweating. Thirst without aversion to wind indicates Pishui. Body swelling with coldness, resembling Zhou Bi, chest oppression, anorexia,集聚 pain, restlessness, insomnia indicates Huanghan. Joint pain, cough, wheezing, non-thirsty indicates Zhengshui; sweating cures it. However, if thirst, diarrhea, and frequent urination exist, do not induce sweating; only promote urination.
Heart Water disease: heavy body, Qi deficiency, inability to lie flat, irritability, genital swelling.
Liver Water disease: abdominal distension, inability to turn side-to-side, hypochondriac pain, slight fluid production, intermittent urination.
Lung Water disease: body swelling, difficult urination, frequent loose stools.
Spleen Water disease: abdominal distension, heavy limbs, lack of fluids, Qi deficiency, difficult urination.
Kidney Water disease: abdominal distension, umbilical swelling, lower back pain, inability to urinate, dampness in genitals like cow's nose sweat, cold feet, thin face.
For all water diseases: Edema below the waist requires promoting urination; edema above the waist requires inducing sweating.
Deep-slow pulse at Cun Kou: Deep indicates Water, Slow indicates Cold. Cold-Water contention with Spleen/Stomach Qi decline causes diarrhea and body swelling, named Shuifen (Water Division).
Weak Shaoyang pulse and fine Shaoyin pulse: Men have difficult urination; women have irregular menstruation, named Xuefen (Blood Division).
Women: Menstruation stops first, then water disease appears, named Xuefen (difficult to treat). Water disease appears first, then menstruation stops, named Shuifen (easy to treat; water removal restores menstruation).
Deep-rapid pulse at Cun Kou: Rapid indicates Yang Excess, Deep indicates Yin Knot. Weak-wiry Douyang pulse indicates no Stomach Qi and breathing difficulty. Deep-slipping Shaoyin pulse indicates interior Excess. Blood knot in the uterus, meridian blockage, named Xuefen.
Q: Patient suffers from water, swollen face/body/limbs, difficult urination, but complains of chest pain, Qi rushing to throat like roasted meat, cough/wheeze. Pulse? A: Deep-tight pulse at Cun Kou. Deep is Water, Tight is Cold. Contention knots at Guan Yuan. Initially mild, unnoticed in youth. After Yang decline, Ying-Wei conflict, Yang damage/Yin excess, cold knot moves, Kidney Qi rushes up, throat obstruction, hypochondriac pain. Doctor treated as Retention of Yin with harsh purgatives; pulse remained deep-tight, disease persisted. Repeated emesis caused Stomach Deficiency/irritability, dry throat/thirst, difficult urination, indigestion, edema. Then used Tingli to drain water; temporary relief. Overeating caused recurrence, chest/hypochondriac pain resembling Ben Tun (Rolling Pig). Water overflow causes cough/wheeze. Treat rebellious Qi first to stop it, then treat cough. Once cough stops, wheezing resolves. Treat new disease first; water disease follows later.
Water disease: Deep-small pulse belongs to Shaoyin as Shishui; Deep-slow pulse belongs to Shaoyin as Zhengshui; Floating with aversion to wind is Fengshui (Taiyang); Floating without aversion to wind is Pishui (Taiyang); Deficient swelling belongs to Qifen (Qi Division); sweating cures it.
