Pulse is deficient, body is hot; this is heatstroke (Shangshu). Summer-heat enters through the mouth and teeth, corresponding to the Heart (Zang organ governing summer). There is extreme thirst, fatigue, aversion to cold on the back, spontaneous sweating, loose hair, mental restlessness, slight coldness of the four limbs, vomiting, and diarrhea. Yi Yuan San (Benefit Origin Powder) cures it. If Chai Hu (Bupleuri Radix) is added for fever, and Renshen (Ginseng Radix et Rhizoma) is used together to tonify Yuan Qi (Primary Qi); if vomiting occurs, Banxia (Pinelliae Rhizoma) fried with ginger juice should be added; if thirst is severe, Tianhuafen (Trichosanthis Radix) is most suitable. If urination is difficult or scanty and red, Zexie (Alismatis Rhizoma) and Shanzhi (Gardeniae Fructus) can be added. Whether there is spontaneous sweating or watery diarrhea, Shengma (Cimicifugae Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) are always appropriate. For headache, Chuanxiong (Ligustici Chuanxiong Rhizoma) and Shigao (Gypsum Fibrosum) are good; for phlegm and chest oppression, Gualou (Trichosanthis Fructus) and Zhishi (Aurantii Fructus Immaturus) follow suit. All these conditions can be treated by frequently taking Yi Yuan San, which has miraculous effects on heat, thirst, deficiency, and irritability. (The ancient Xiangru San (Artemisiae Scopariae Herba Decoction) originally contained only Xiangru (Moslae Herba), Houpo (Magnoliae Officinalis Cortex), and Doudou (Sojae Semen Praeparatum). Later, Huanglian (Coptidis Rhizoma) was added. Then Chenpi (Citri Reticulatae Pericarpium) and Fupingzi (Trogopterori Faeces) were added, named Bawei (Eight Ingredients). Adding Huanglian, Fuling (Poria), and Mugua (Chaenomelis Fructus) makes it Shiwei (Ten Ingredients).)
Huoxiang Zhengqi San (see Cholera section)
Xiaoshu Wan (Summer-Heat Relieving Pill): Banxia (Pinelliae Rhizoma, boiled in vinegar) one jin, Gancao (Glycyrrhizae Radix et Rhizoma, raw), Fuling (Poria) each eight liang. Boil in ginger juice to make a paste, form pills the size of paulownia seeds. Take fifty pills with hot water when suffering from heatstroke; the patient will revive immediately. It is especially effective for heatstroke causing fever and headache. Taking it regularly in summer stops thirst and promotes urination; other formulas generally cannot compare. For retained fluid and phlegm, take with ginger decoction.
Shui Hulu (Water Gourd - herbal preparation name)
Treats summer-toxin, relieves irritability and thirst.
Chuan Bai Yao Jian (Fermented Tea Cake) three liang, Maidong (Ophiopogonis Radix), Wumei (Mume Fructus flesh), Bai Mei (White Plum) flesh, Gange (Puerariae Radix), Gancao (Glycyrrhizae Radix et Rhizoma) each five qian, Renshen (Ginseng Radix et Rhizoma) two qian. Make pills with flour paste, size of euryale seed. One pill taken during travel in summer lasts a day.
Additional Remarks
Heatstroke resembles Taiyang disease (Cold Damage), as both may present with headache and fever. Quack doctors treat it as true Cold Damage, often leading to toxic rashes, jaundice, urinary retention, delirium, and death. If the pulse is tight, use Wuji San (Five Accumulations Powder) as the main formula (see Cold Damage section). If excessive consumption of cold food damages the Spleen and Stomach, simply use herbs that warm the Spleen, digest food, and promote urination.
Another conditioning method: During summer residence, constantly use Baizhu (Atractylodis Macrocephalae Rhizoma) 1.5 qian and Maidong (Ophiopogonis Radix, heart removed) 1 qian. Decoct and drink as tea. Or use Renshen (Ginseng Radix et Rhizoma), Maidong (Ophiopogonis Radix), and Wuweizi (Schisandrae Chinensis Fructus), named Shengmai San (Generate Pulse Powder). Or use Chen Sha Liuyi San (Cinnabar Six-One Powder), which can permanently prevent heatstroke.
One Formula: Use non-worm-eaten Zaojiao (Gleditsiae Sinensis Spina), any amount. Scrape off black skin, burn until smoke is almost gone. Cover with a basin on the ground without letting smoke escape. For every one liang of ash, add six qian of Gancao (Glycyrrhizae Radix et Rhizoma) powder, mix well. Take one qian, mixed with fresh cool water. For people with Qi deficiency, mix with warm grain water. For those unconscious, two doses are sufficient.
Zhuxia (Summer Complaint / Seasonal Weakness)
Yin deficiency and insufficient Yuan Qi (Primary Qi) loss; Qi deficiency and Blood weakness caused by Heat. Pulse is powerless, four limbs are fatigued, mental irritability, poor appetite, legs feel weak. Likes lying down, dry mouth, fails to gain flesh. Treatment uses Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). Renshen (Ginseng Radix et Rhizoma) and Gancao (Glycyrrhizae Radix et Rhizoma) solidify the root of Yuan Qi; Baizhu (Atractylodis Macrocephalae Rhizoma), Chenpi (Citri Reticulatae Pericarpium), and Wuweizi (Schisandrae Chinensis Fructus) are commonly used. Huangbai (Phellodendri Cortex), Shengma (Cimicifugae Rhizoma), Danggui (Angelicae Sinensis Radix) nourish Blood; Chai Hu (Bupleuri Radix), Fuling (Poria), Zhimu (Anemarrhenae Rhizoma), and Maidong (Ophiopogonis Radix) are used together. For night sweats, add Xiaomai (Tritici Fructus); for red urine, add Shanzhi (Gardeniae Fructus) and Mutong (Akebiae Caulis). For headache, add Chuanxiong (Ligustici Chuanxiong Rhizoma) and Baizhi (Angelicae Dahuricae Radix); for thirst, use Tianhuafen (Trichosanthis Radix) and Wumei (Mume Fructus). For loose stools, Shaoren (Amomi Fructus) is good; for cough, add Xingren (Prunus Armeniacae Semen) following the pattern; if diarrhea occurs, add Cangzhu (Atractylodis Rhizoma). Following these principles for regulation ensures no danger to the illness.
Buzhong Yiqi Tang (see Internal Injury section)
Additional Remarks
Zhuxia seems not to affect the major system, but one does not know that it is contracted due to Qi deficiency, and the illness further depletes Qi. If ignored and untreated, the body suffers. Everyone fears contracting it, and it happens year after year. Therefore, my friend specially composed a rhyme for it. I appreciate his intent and could not bear to omit it.
