Those with a sensation of vexation, fullness, and restlessness in the heart are suffering from *Zi Fan* (Fetal Vexation). This is attributed to Heart Qi deficiency with internal fire. The treatment should primarily use *Zhuye Tang* (Bamboo Leaf Decoction) from *Jian Yi Fang*.
**Zhuye Tang (Bamboo Leaf Decoction)**
*Bai Fuling* (Poria, 2 qian), *Fangfeng* (Saposhnikovia Root, 8 fen), *Maidong* (Ophiopogon Tuber), *Huangqin* (Scutellaria Root, 1 qian each), *Zhuye* (Bamboo Leaf, 9 slices).
If a pregnant woman suffers from excessive phlegm-fluid obstructing the airways, accompanied by occasional convulsions and loss of consciousness, it is named *Zi Xian* (Eclampsia of Pregnancy). The treatment should focus on clearing heat and transforming phlegm. This condition is mostly caused by Qi dysfunction, so the therapeutic method is similar to that for general epilepsy. [Marginal Note: Epilepsy, commonly known as "sheep epilepsy," is caused by the accumulation of phlegm and water in the Pericardium; it can affect both men and women and is difficult to eradicate completely. However, eclampsia in pregnancy is a different matter.] The dosage of herbs should be slightly smaller. If nausea is severe, *Erchen Tang* (Two-Aged Decoction) should be decocted to induce vomiting. After vomiting subsides, regulate Qi and transform phlegm. Simultaneously, use *Huangqin* (Scutellaria Root) and *Baizhu* (Atractylodes Macrocephala Rhizome) to stabilize the fetus. The ancient formula *Lingyangjiao San* (Antelope Horn Powder) has properties that are too pungent and dispersing; physicians should adapt its use according to the season and circumstances.
If a pregnant woman raises her arms high, causing the umbilical cord or vessels held in the fetal mouth to prolapse, resulting in crying, instruct the mother to bend her waist and touch the ground, as if picking up an object, to help the fetus re-enter the birth canal, which will stop the crying.
When a pregnant woman reaches seven or eight months, the fetus grows large, causing abdominal distension and fullness that compresses the uterus, leading to discomfort in sitting and lying down. It is appropriate to take *Shutai Yin* (Fetus-Binding Drink) for two or three doses.
**Shutai Yin (Fetus-Binding Drink)**
*Baizhu* (Atractylodes Macrocephala Rhizome), *Huangqin* (Scutellaria Root), *Zisu* (Perilla Leaf), *Zhiqiao* (Aurantium Fructus), *Dafupi* (Areca Peel, 1.5 qian each), *Zhigancao* (Prepared Licorice Root, 3 fen), *Daikesharen* (Cardamom Seed with Shell, 5 fen, guided by ginger).
After conception, if a woman continues to have menstrual bleeding monthly, it is called *Jijing* (Provoked Menses). This occurs due to excess blood. If a pregnant woman bleeds from the vagina without cause, or discharges yellow fluid or bean-soup-like fluid, but without pain in the lower back and abdomen, it is called *Tailou* (Fetal Leakage). If the fetus is injured and bleeding occurs, there must be pain in the lower back and abdomen. Pregnant women may also present with hematuria (blood in urine), where the abdomen is not painful. This differs from *Tailou*. Hematuria originates from the urethra, while leakage originates from the vagina. Although all three involve bleeding, their sources differ, and physicians must distinguish them carefully.
If *Jijing* presents without other accompanying symptoms, no medication is required. A strong fetus will consume the maternal blood as it grows, and menstruation will naturally cease. If *Tailou* involves bleeding, it is mostly due to Blood Heat; *Ejiao Tang* (Donkey-Hide Gelatin Decoction) should be used to clear heat and stop bleeding. If yellow fluid or bean-soup-like discharge is profuse, indicating a dried-up fetus that will inevitably abort, *Huangqi Tang* (likely referring to a Huangqin-based formula given context, or specifically *Huanglian*/*Huangbai* variants, but text says "Huang Tang") should be decocted and taken, or *Yinzhu Jiu* (Silver Flower and Ramie Wine) should be used. If hematuria occurs, it indicates Bladder Blood Heat; *Shengdi Siwu Tang* (Rehmannia Four Substances Decoction) added with *Xuyuren* (Carbonized Human Hair) and *Baimaogen* (Imperata Rhizome) should be used to cool the blood and stop bleeding.
