All carbuncles, sores, ulcers, and toxic swellings belonging to the zygomatic region, cheeks, parotid area, chin, and nose are compiled in this section for easy reference and treatment.
Redness of the zygomatic and cheek areas appearing in the afternoon indicates rising deficiency fire (Xu Huo); it should not be treated as Taiyang Cold Damage (Shang Han).
Red face with a rapid but forceless pulse indicates Hidden Yin Disease (Fu Yin Bing), which is false heat.
Red face with a deep and fine pulse indicates Shaoyin Disease (Shao Yin Bing), characterized by external heat and internal cold, where excess Yin repels Yang outward; warming methods are appropriate, while misusing cooling herbs will lead to death.
Red face with a wiry and rapid pulse indicates Shaoyang Disease (Shao Yang Bing); Xiaochaihu Tang (Minor Bupleurum Decoction) is suitable for harmonizing.
General redness of the entire face indicates that the Yangming exterior syndrome has not yet resolved; muscle-releasing methods are indicated, while purging the interior is contraindicated.
Blue-green color of the face and lips indicates extreme Yin cold.
Blue-green face accompanied by tongue retraction and testicular contraction indicates Yin cold.
Blue-green face accompanied by lower abdominal pain indicates Cold Damage complicated by Kidney deficiency (Jia Yin Shang Han).
Yellowing of the face, eyes, and body accompanied by short and scanty urination indicates Damp-Heat (Shi Re).
Yellowing of the face, eyes, and body accompanied by distension, fullness, hardness, and pain in the lower abdomen with free urination indicates Blood Stasis Cold Damage (Xu Xue Shang Han).
Pale face lacking spirit or accompanied by excessive sweating indicates collapse due to blood loss (Tuo Xue).
Individuals with pale faces should not undergo heavy sweating therapies.
Individuals with dark complexions who develop deficiency patterns during Cold Damage should not receive strong tonification with Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu), as their Qi tends to be excess in nature.
The *Ling Shu* states: The condition of the hands and face reflects the state of the body. When heaven is cold, the earth cracks and ice forms, and limbs may become flaccid and drooping, yet the face remains uncovered. Why? It is said that the Twelve Meridians and 365 Luo-collaterals all direct their blood and Qi upward to the face and enter the sensory orifices. The essence and Yang Qi ascend to the eyes to form vision; divergent Qi ascends to the ears to form hearing; Zong Qi (Gathering Qi) emerges from the nose to form smell; turbid Qi originates from the Stomach and ascends to the lips and tongue to form taste. The fluids and Qi of these meridians all steam upward onto the face. Furthermore, the facial skin is thick and the flesh firm, so neither extreme heat nor extreme cold can overcome it.
Li Dongyuan stated: Irregular diet damages the Stomach; Stomach damage leads to shortness of breath and diminished spirit, generating intense heat. Sometimes, fire alone blazes upon the face. This is because the Yangming Meridian is rich in both Qi and Blood, and often carries Wind-Heat upward. Since all Yang meridians converge at the head and face, this causes the face to feel hot as if intoxicated. Treatment should first disperse Wind-Heat, or use Tiaowei Chengqi Tang (Regulate the Stomach and Order the Pills Decoction) added with Coptidis Rhizoma (Huanglian) and Rhinoceros Horn (Xijiao - note: usually substituted with Water Buffalo Horn in modern practice) to induce bowel movements two or three times, clearing the root heat and dispersing Wind-Heat. Alternatively, Shengma Tang (Cimicifugae Decoction) added with Huanglian is the primary formula.
Feng Luzhan stated: The six Yang meridities of the hands and feet all ascend to the head, but specifically the Yangming Stomach Meridian begins beside the nose, crosses at Yingxiang (LI20), enters the upper teeth, surrounds the lips, follows the Jiache point (ST6) up to the front of the ear, passes through Shangguan (ST7), thus governing the human face. If there is heat or stagnant fire in the Stomach, facial heat occurs; Shengma Tang added with Huanglian is used. If Wind-Heat is severe internally and attacks upward, causing swelling of the face and eyes, purple discoloration of the face and nose, or wind-induced prickling and hives, treatment should be based on the specific symptoms.
