Wang Kentang treated a patient suffering from *fuguju* (deep-seated abscess). The pus had matured but could not be drained; upon ulceration, the pus entered the abdominal cavity. The patient was mentally confused and unable to take even gruel or medicine, leaving the physicians at a loss. Upon examination, the pulse was fine as a spider's thread, and the breath was faint. Wang said, "There is no danger." He used a needle to puncture the abdomen, draining a large amount of pus, which was clear and thin like crab foam. According to medical principles, penetration of the membrane is considered incurable. However, he prescribed a large dose of *Renshen* (Ginseng Root), [missing herb], *Fuzi* (Aconite Lateralis Radix Praeparata), along with herbs entering the Jueyin channel to promote circulation. The patient also took *Bawei Wan* (Eight-Ingredient Pill with Rehmannia). Appetite significantly improved, consuming over one *sheng* of rice and several pieces of meat per day, and the condition resolved within ten days. The reason for curability lies in the pulse of an ulcer: a solid/large pulse indicates death, while a fine/subtle pulse indicates life. Since the disease presentation matched the pulse, it was treatable. Puncturing the abdomen was necessary because undrained pus would inevitably attack internal organs; palpation confirmed the depth of the wound, ensuring the needle did not penetrate the membrane. *Bawei Wan* tonifies the Kidney; when Kidney Qi rises vigorously, the stomach opening opens, allowing food intake. When pus drainage is excessive, small doses of medicine are insufficient. Procrastination leads to other complications. Therefore, promoting stomach appetite and consuming meat to tonify the body allows muscles to regenerate quickly. This is the general method for treating ulcers.
A man suffered from bone-penetrating pain at the *Huantiao* (GB30) acupoint at the root of the thigh, with normal skin appearance. The pulse was rapid, stagnant, and slippery. This was identified as *fuguju*, with pus about to form. After six doses of *Tuoli* (supporting interior) medicine, swelling appeared with pain, and the pulse became slippery and rapid, indicating pus formation. Needling released about one bowl of pus. With additional tonic prescriptions, he recovered after more than a month.
Another man suffered from *fuguju*, characterized by local redness, swelling, hardness, heat, bone pain, and tendon spasms. The pulse was rapid and deep. He was cured using *Danggui Niangong Tang* (Angelica Ligustici Decoction for Pain Relief).
Zhang Jingyue treated a man in his thirties who was prone to overwork. He suddenly experienced soreness and pain at the *Huantiao* acupoint. Months later, the upper thigh gradually swelled. Zhang stated, "This is *fuguju*; it must be treated promptly." He prescribed two doses of *Huoming Yin* (Decoction for Saving Life), but there was no effect, and the swelling worsened. Due to the indiscriminate use of cooling fire and detoxifying herbs, the patient developed vomiting, nausea, fever, and loss of appetite, leading to a critical condition. Upon seeking further treatment, Zhang prescribed *Shen Nei Tuo San* (Powder for Supporting Interior with Ginseng) with a large addition of *Paojiang* (Prepared Ginger Rhizome). After several doses, vomiting stopped, appetite returned, and the swelling softened, indicating pus maturity. Needling released little pus. He then prescribed *Jiuwei Yigong Jian* (Nine-Ingredient Extraordinary Efficacy Decoction), resulting in massive ulceration with瓣状 (lobed) pus discharge. There were five or six ulcerations, and most of the leg muscle was lost, leaving only skin and bone. Post-ulceration, vomiting, fever, and anorexia recurred. Alternating between *Shiquan Dabu Tang* (Decoction for Complete Great Tonification) and *Jiuwei Yigong San*, the fever gradually subsided, and appetite slowly returned. However, the leg tendons were shortened; he could only lie supine with knees upright, legs pressed tightly together, unable to move without extreme pain. He believed he would become disabled. Using *Shiquan Dabu Tang* for over thirty doses and three *jin* of *Renshen* (Ginseng Root), muscle growth and tendon relaxation eventually returned to normal.
Another man, nearing thirty, neglected sexual restraint and suffered from *Huantiao* soreness and pain for over a month. Zhang said, "This is most alarming; fear of carbuncle/toxin development." The patient did not believe this. Others suggested that bone/joint pain is common, caused merely by wind-heat, and prescribed wind-dispersing and fire-clearing herbs. Half a year later, slight swelling occurred. Seeking treatment again, Zhang advised, "Promptly use supporting and tonifying methods to save the root; it is not too late." The patient refused, consulting a surgeon who asked, "How can one use warming and tonifying herbs before an abscess has ulcerated?" He again used cooling fire and detoxifying agents, leading to a critical state only after ulceration. Upon re-examination, the pulse and symptoms were depleted, and regret was futile.
A wealthy youth in his thirties, addicted to alcohol and sex, suffered from the same condition. He was advised to take medicine early but stubbornly refused. When swelling and pus formation occurred, he was advised to undergo needling immediately but refused, trusting quacks who applied bitter-cold detoxifying plasters. Ignoring that formed pus cannot be "detoxified," the condition worsened over time until spontaneous ulceration exhausted the vital energy, making recovery impossible.
Xue Lizhai treated Official Meng Xiang, who had a carbuncle at the *Huantiao* acupoint. Both internal and external treatments were cooling and detoxifying. Due to extreme pain, needling released a large amount of pus and blood. The wound opened, appearing purple and dark. The right Guan pulse was floating and large, indicating renewed damage to Stomach Qi, failing to nourish the affected area. He used moxibustion with a fermented soybean cake (*Douchi Bing*) combined with *Liujunzi Tang* (Six-Gentlemen Decoction) plus *Huoxiang* (Patchouli Herb), *Sharen* (Amomum Fruit), and *Paojiang*. After several doses, Stomach Qi revived, appetite increased, the affected area warmed, and muscles gradually regenerated. He was cured with *Shiquan Dabu Tang*.
Da Jun Ducheng Qing suffered from *fuguju*, with internal pain like a awl piercing and unchanged external color. The condition could not dissipate. Fortunately, cold-depleting herbs were not used. The pain had damaged Stomach Qi, causing anorexia. *Liujunzi Tang* was used, slightly improving diet. Then, *Shiquan Dabu Tang* was administered for over twenty doses until pus formed. After needling drainage, the major tonic decoction continued with doubled *Renshen* (Ginseng Root), [missing herb], *Danggui* (Angelica Sinensis), and *Baizhu* (Atractylodes Macrocephala Rhizome), plus *Maidong* (Ophiopogon Tuber), *Wuweizi* (Schisandra Fruit), *Yuanzhi* (Polygala Root), and *Beimu* (Fritillaria Bulb). After several doses, pus secretion ceased, and the wound healed.
A scholar had slight swelling in the left leg with normal muscle color and reduced appetite. This indicated deficiency of True Qi and invasion of warm pathogens. Since all Qi derives from the Stomach, the method was to assist the Stomach and strengthen Qi. *Liujunzi Tang* plus *Huoxiang* (Patchouli Herb), *Muxiang* (Saussurea Root), and *Danggui* (Angelica Sinensis) was given for several doses, gradually improving diet. *Shiquan Dabu Tang* restored original Qi and cured the condition.
Another scholar had painful swelling in both legs with unchanged skin color, aversion to cold, fever, reduced appetite, and limb fatigue. This was due to Spleen Qi deficiency and downward pouring of damp-phlegm. *Buzhong Yiqi Tang* (Decoction for Supplementing the Middle and Augmenting Qi) plus *Banxia* (Pinellia Tuber), *Fuling* (Poria), and *Baishao* (White Peony Root) was administered for two doses, resolving chills/fever and swelling/pain. After another ten-plus doses, the Spleen and Stomach strengthened, and the physique became robust.
A man suffered chest pain due to heavy lifting and irregular diet. Mistaking it for toxin-related sores, he took *Dahuang* (Rhubarb Root) and other purgatives. This led to right thigh swelling with normal skin color, headache, aversion to cold, panting/thirst, fever, and a flooding, forceless pulse. This resulted from labor injuring Original Qi and medicines damaging Stomach Qi. *Buzhong Yiqi Tang* for four doses, followed by *Shiquan Dabu Tang* for several doses, cured him, aided by his youth.
Xue Lizhai treated a woman with *fuguju* that failed to heal for a long time, resulting in thin, weak legs. The pulse was slow and delayed. He prescribed *Shiquan Dabu Tang* plus *Niuxi* (Achyranthes Root) and *Duzhong* (Eucommia Bark), along with moxibustion using *Fuzi* (Aconite Lateralis Radix Praeparata) cakes. She recovered after over two months. Generally, post-ulceration with a stagnant, slow/delayed pulse indicates easy recovery due to existing Stomach Qi. A fine, deep, straight pulse indicates interior deficiency and potential transformation of symptoms; if accompanied by irritability and pain, it is not yet cured. A flooding, solid, coarse, or scattered pulse is difficult to treat due to deficiency of Righteous Qi and excess of Pathogenic Qi.
A woman had pain at the *Huantiao* acupoint with normal skin color and a tight, rapid pulse, identified as *fuguju*. Before pus formation, she drank *Neituo Huangqi* (Supporting Interior Astragalus) decoction made with wine, plus *Qingpi* (Green Tangerina Peel), *Longdancao* (Gentiana Root), and *Zhizi* (Gardenia Fruit). After several doses, it dissipated.
A woman suffered from chronic *fuguju* with continuous pus discharge, skin itching, and flaccid limbs. Suspecting deficiency, she wanted tonification. Confused by others as a wind disease, she took wind-expelling herbs, leading to her death. Chen Wuzhe stated: The human body consists of skin/hair, blood vessels, fascia, muscles, and bone marrow, forming the shape, governed internally by the Heart, Liver, Spleen, Lungs, and Kidneys. Indulgence in emotions, anger, joy, work, and rest depletes the essence and blood of the Five Zang Organs, causing weakness in skin, blood, tendons, bones, and muscles, leading to inability to move. This results in *weibi* (flaccidity/paralysis), resembling *roufeng* (soft wind) and *xiangjiaojiao* (Hong Kong foot/gout). *Roufeng* and *xiangjiaojiao* are caused by external factors, whereas *weibi* is caused by insufficiency of the Five Zang Organs.
Cao Zhulu from Shanxi, over forty, suffered from *fuguju* in summer and recovered with *Tuoli* (supporting interior) medicine. In autumn, he had reduced appetite, phlegm-Qi congestion, and mouth/tongue ulcers. He was cured with *Bawei Wan* (Eight-Ingredient Pill with Rehmannia).
An elderly man had *fuguju* on the leg, swollen and hard, painful only upon deep pressure, dry mouth, weak pulse, localized swelling without ulceration, and reduced appetite. It was noted that hardness below swelling originates from sinews/bones; unchanged skin color originates from bone marrow. He was treated with *Renshen* (Ginseng Root) and [missing herb] to support the interior. After thirty-plus doses, pus matured but did not burst. Although medicinal power reached the site, needling was required. He refused, waiting ten days before needling, which discharged five to six bowls of clear pus. However, due to old age, insufficient Qi and Blood, and prolonged retention of toxicity, he ultimately died. Generally, if a sore does not resolve daily, support the interior; if pus exists, needle it; if necrotic flesh exists, remove it; if deficient, tonify. This achieves complete efficacy.
Wang Shangshe suffered from *fuguju*, fearing needling, leading to widespread ulceration of arms and knees. Pulse was rapid with thirst, irritability, occasional cough, and reduced appetite. Qi Shi stated: Irritability, occasional cough, severe abdominal pain/thirst, or uncontrolled diarrhea in sores are malignant signs. Post-pus drainage, a flooding/rapid pulse is difficult to treat; a slightly choppy/slow-delayed pulse is easy to treat. He was needled, releasing four to five bowls of pus. Immediately, a large dose of *Renshen* (Ginseng Root), [missing herb], *Danggui* (Angelica Sinensis), and *Baizhu* (Atractylodes Macrocephala Rhizome) was taken. The next day, the pulse stabilized. *Bazhen Tang* (Eight-Treasure Decoction) plus *Wuweizi* (Schisandra Fruit), *Maidong* (Ophiopogon Tuber), *Rougui* (Cinnamon Bark), and *Bailian* (Belamcanda Rhizome) was administered for thirty-plus doses. The pulse slowed, pus thickened, and he recovered in three months.
Wang Hongxu's formula for *fuguju*: Crush *Baijiezi* (White Mustard Seed) into powder, mix with wine for topical application. Alternatively, mix powders of *Daji* (Euphorbia Root) and *Gansui* (Kansui Root) with white honey for application. Internally, take *Yanghe Tang* (Yang-Nourishing Decoction), one dose daily. It may dissipate in four to five days. After dissipation, take *Zilong Wan* (Child Dragon Pill) or *Xiaojin Dan* (Little Gold Pellet) to prevent recurrence. Incision is strictly contraindicated; cutting leads to permanent leg shortening/disability.
