Bian Que said: When a person's pulse beats twice per breath, it is called the normal pulse (Ping Mai), and the body suffers no pain. When the pulse beats three times per breath, it is called the disease pulse. When the pulse beats four times per breath in a Bi syndrome patient, this indicates exhaustion of pulse qi (Tuo Mai Qi); if the eyes are blue-green, death will ensue. If the pulse beats five times or more per breath, death occurs and it is incurable. In cases of vocal abnormalities where the pulse comes with agitation, taking an extreme rapid pulse reaching five beats per breath, the condition may worsen to six or seven beats per breath.
Bian Que said: The harmonious Qi is neither slow nor fast, neither slippery nor choppy, neither existing nor absent, neither short nor long, neither bowing nor rising, neither longitudinal nor transverse; this is called the normal pulse. If the Kidney (or Tense pulse) presents thus, the body suffers no pain.
Bian Que said: A string-tight and rapid pulse indicates disease in the Liver. Symptoms include reduced appetite and aversion to food, abdominal tension with excessive talking, dizziness and eye pain, abdominal fullness, tendon contraction, epilepsy with rebellious Qi, hard accumulations in the lower abdomen, frequent spitting of blood, and dryness in the throat. The method for diagnosing disease involves observing complexion, listening to voice, and viewing the location of the illness. Are not the essential principles of pulse diagnosis subtle? A floating pulse resembling a rapid one without fever indicates Wind pathogen. If the pulse is floating and rapid with fever, it indicates Qi stagnation. If the pulse is洪大 (Hong Da, large and forceful), accompanied by pulsation in both breasts, further rapidity, and alternating chills and fever, this is Shang Han Bing (Cold Damage Disease). In chronic wasting diseases, if the pulse floats and overflows at the Cun Kou (radial artery site) with slight fever, this is Zhu Qi Bing (Zhu Qi Disease, often referring to infectious or chronic consumptive conditions). If coughing and high fever are added, with symptoms fluctuating between severe and mild, it is difficult to treat. Conversely, treatment without severe symptoms leads to easy recovery; absence of cough makes it easier to cure.
When a patient's Five Zang organs have exhausted their essence, Shen Ming (Spirit-Mind) cannot reside within, and the voice is hoarse, death occurs.
When a patient grasps at clothing seams and speaks deliriously, it is incurable.
When a patient's Yin and Yang Qi are both exhausted, clutching at empty air and speaking deliriously, death occurs.
When a patient speaks deliriously, is confused, or unable to speak, it is generally incurable; however, if it is a Re Bing (Heat Disease), it may be treated.
When a patient's Yin and Yang Qi are both exhausted and they lose their voice and cannot speak, death occurs in three and a half days.
If yellow luster appears in a patient's eyes, the disease is about to heal.
If a patient has a yellow face and blue-green eyes, they will not die; but if the blue-green color resembles dead grass, they will die.
If a patient has a yellow face and red eyes, they will not die; but if the red color resembles coagulated blood, they will die.
If a patient has a yellow face and white eyes, they will not die; but if the white color resembles dry bone, they will die.

If a patient has a black face and blue-green eyes, they will not die.
If a patient has a blue-green face and white eyes, they will die.
If a patient has a black face and white eyes, they will not die.
If a patient has a red face and blue-green eyes, they will die in six days.
If a patient has a yellow face and blue-green eyes, they will certainly die in nine days; this is called Luan Jing (Disordered Meridians). Drinking alcohol while exposed to wind allows evil to enter the Stomach meridian, causing Gallbladder Qi to leak recklessly, turning the eyes blue-green. Even with heavenly rescue, life cannot be restored.
If a patient has a red face and white eyes, they will die in ten days. Worry, anger, and excessive thinking deplete Heart Qi internally; if the complexion appears falsely good, prepare coffins urgently.
If a patient has a white face and black eyes, they will die. This signifies that glory has departed and blood vessels are empty and depleted.
If a patient has a black face and white eyes, they will die in eight days. Kidney Qi is injured internally, and the pathogenic factor remains accumulated.
If a patient has a blue-green face and yellow eyes, they will die in five days.
If a patient is bedridden, suffers from heart pain and shortness of breath due to Spleen exhaustion and internal injury, they may recover in one hundred days. If they can rise and pace around, then sit on the ground, or fall back leaning against the bed, those who can cure such conditions can be called divine physicians.
If a patient lacks luster in their face, appearing like earth, and cannot accept food and drink, they will die in four days.
If a patient's eyes lack luster and teeth are black, it is incurable.
If black color appears in a patient's ears, eyes, nose, and mouth and enters the mouth, death is certain.
If a patient's ears, eyes, and cheekbones are red, death will occur within five days.
If black qi emerges from the forehead, extending up to the hairline and down straight to the nasal bridge and above the cheekbones, death will also occur within five days.
If black qi emerges from Tian Zhong (the area between the eyebrows) and extends down to Nian Shang (tip of the nose) and above the cheekbones, death occurs.
If a patient or a healthy person shows black or white color entering the eyes, nose, and mouth, death will occur within three days.
If a patient or a healthy person's face suddenly looks like horse liver color—blue-green from afar and black up close—death occurs.
If a patient has a black face, fixed staring eyes, and aversion to wind, death occurs.
If a patient has a black face and blue-green lips, death occurs.
If a patient has a blue-green face and black lips, death occurs.
If a patient has a black face, fullness under both hypochondria, and inability to turn over independently, death occurs.
If a patient's eyes roll and stare fixedly, and they breathe with shoulder elevation (Shan Xi), death occurs in one day.
If a patient has suffered from head and eye pain for a long time and suddenly loses vision, death occurs.
If a patient has Yin binding and Yang exhaustion, protruding eyeballs, and mental confusion, death occurs.
If a patient's Yin and Yang are exhausted and sunken, death occurs.
If a patient's brow connection collapses, death occurs in seven days.
If a patient's mouth opens like a fish's mouth and cannot close, with breath exiting frequently but not returning, death occurs.
If a patient's mouth is open, death occurs in three days.
If a patient's lips are blue-green and the philtrum is inverted, death occurs in three days.
If a patient's lips are everted and the philtrum is full, death occurs.
If a patient's lips and mouth suddenly become dry, it is incurable.
If a patient's lips are swollen and teeth are scorched, death occurs.
If a patient's Yin and Yang are both exhausted, teeth resemble cooked small beans, and the pulse is rapid, death occurs.
If a patient's teeth suddenly turn black, death occurs in thirteen days.
If a patient's tongue curls and testicles retract, death is certain.
If a patient sweats but the sweat does not flow, and the tongue curls and turns black, death occurs.
If a patient's hair stands straight, death occurs in fifteen days.
If a patient's hair resembles dry hemp and they are prone to anger, death occurs.
If a patient's hair and eyebrows stand upright, death occurs.
If a patient's fingernails are blue-green, death occurs.
If a patient's fingernails are white, they will not survive.
If the flesh under the fingernails of a patient's hands and feet is black, death occurs in eight days.
If a patient's Ying Wei (Nutritive and Defensive Qi) are exhausted and the face is edematous, death occurs.
If a patient has sudden swelling and a cyan-black face, death occurs.
If a patient's palms are swollen and lines disappear, death occurs.
If a patient's umbilicus is swollen and everted, death occurs.
If a patient's scrotum and penis are both swollen, death occurs.
If a patient's pulse ceases, mouth is open, and feet are swollen, death occurs in five days.
If a patient's dorsum pedis is swollen, accompanied by vomiting and heavy head, death occurs.
If a patient's dorsum pedis is swollen and knees are as large as buckets, death occurs in ten days.
If a patient lies down and passes stool without awareness, death occurs.
If a patient emits a corpse-like odor, it is incurable.
In Liver disease, if skin is black, death occurs on Geng-Xin days (Metal days).
In Heart disease, if eyes are black, death occurs on Ren-Gui days (Water days).
In Spleen disease, if lips are blue-green, death occurs on Jia-Yi days (Wood days).
In Lung disease, if cheeks are red and eyes are swollen, death occurs on Bing-Ding days (Fire days).
In Kidney disease, if face is swollen and lips are yellow, death occurs on Wu-Ji days (Earth days).
Blue-green should resemble the luster of dark jade, not the dullness of indigo.
Red should resemble vermilion wrapped in silk, not the dullness of hematite.
White should resemble goose feathers, not the starkness of salt.
Black should resemble layered lacquer, not the dryness of charcoal.
Yellow should resemble realgar wrapped in gauze, not the dullness of yellow earth.
Regarding eye color: Red indicates Heart disease, White indicates Lung disease, Black indicates Kidney disease, Yellow indicates Spleen disease, and Blue-green indicates Liver disease. If yellow is indescribable, the disease is in the chest center.
In diagnosing eye diseases: Red veins moving from top to bottom indicate Tai Yang Bing (Tai Yang Disease); from bottom to top indicate Yang Ming Bing (Yang Ming Disease); from outside to inside indicate Shao Yang Bing (Shao Yang Disease). In diagnosing cold-heat Luoli (scrofula), if there are red veins in the eyes extending from top to bottom to the pupil, seeing one vein means death in one year; one and a half veins, death in one and a half years; two veins, death in two years; two and a half veins, death in two and a half years; three veins, death in three years.
In diagnosing toothache from dental caries, pressing the Yang Ming pulse: if there is an excess pulse, heat is localized—right side heat on the right, left side heat on the left, upper heat above, lower heat below.
In diagnosing blood-related pulses: many red indicates much heat; many blue-green indicates much pain; many black indicates chronic Bi syndrome. If red, black, and blue-green are all present, it indicates alternating chills and fever with body pain. Slight yellow complexion, yellow dental plaque, and yellow nails indicate Huang Dan Bing (Jaundice Disease). Resting quietly, avoiding yellow/red foods, with a small and choppy pulse indicates poor appetite.
