When Lung Qi is deficient, nasal breathing is clear but breath is short; when Lung Qi is excess, there is panting with sound, chest fullness requiring仰ine breathing.
When Lung Qi is deficient, one dreams of white objects, sees people being beheaded with blood spilling everywhere; if it occurs during its governing season (Autumn), one dreams of war; when Lung Qi is excess, one dreams of fear and weeping.
When Jue Qi (rebellious cold qi) lodges in the Lung, one dreams of flying high, seeing metal, iron vessels, or strange objects.
When disease is in the Lung, symptoms improve in the late afternoon, worsen at noon, and are calm at midnight.
Disease originating in the Lung presents with panting and coughing; after three days it transmits to the Liver, causing hypochondriac pain and distending fullness; after one day it transmits to the Spleen, causing obstruction and inability to pass stool, body pain and heaviness; after five days it transmits to the Stomach, causing abdominal distension; if not resolved within ten days, death ensues. Death occurs at sunset in Winter and sunrise in Summer.
If the Lung pulse is firm and long, one should expect hemoptysis (spitting blood); if the pulse is soft and scattered, one should expect leaking sweat (lou han). The pathogenic factor has not dispersed until now.
If the Lung pulse is rapid on deep pressure and panting on superficial pressure, the patient suffers from shivering chills and fever, abdominal fullness, heat in the intestines, red urine, shoulder and back pain, and sweating above the waist. This results from sexual intercourse followed by exposure to wind while sweating.
When the White Pulse (Lung pulse) arrives, there is panting and the pulse is floating and large; the upper part is deficient and the lower part is excess, accompanied by fright, accumulated qi in the chest, panting and deficiency, named Lung Bi (obstruction). It involves chills and fever, resulting from sexual activity while drunk.
When the Lung is struck by Wind, there is dry mouth and panting, dizziness and heaviness of the body, vertigo and swelling.
When the Lung is struck by Cold, the patient spits turbid mucus.
Exposure to cold forms and consumption of cold drinks injure the Lung, because the two types of cold interact, injuring both interior and exterior, thus causing Qi to rebel and ascend. When the Lung is injured, labor and fatigue induce coughing and hemoptysis. The pulses may be fine, tight, floating, or rapid, all indicating hematemesis/hemoptysis; this arises from irritability and anger, causing Lung injury and Qi stagnation.
In Lung Zhang (distension), there is deficiency with fullness, panting, coughing, rebellious Qi leaning on breathing, eyes protruding like falling out, and a floating pulse. In Lung Shui (water disease), the body is heavy, urination is difficult, and stools are frequently loose like duck feces. When Liver Wood overacts on Lung Metal, it must result in deficiency. The pulse is soft and weak; the weak position is reversed at the Guan (middle) position, the soft position is reversed at the Dian (top/cun) position. Floating is reversed in the upper, weak is reversed in the lower. Floating indicates Yang, weakness indicates blood deficiency, so weakness definitely indicates deficiency. Floating and weak are distinct: floating indicates exiting outward, weak indicates entering inward. Floating indicates exiting but not entering, meaning there is Exterior but no Interior; weak indicates entering but not exiting, meaning there is no Exterior but Interior. Yang exiting to the extreme causes sweating returning only to the waist, which means no Exterior but Interior, hence named Jue Yang (Rebellious Yang). It should have been treated with sweating but was not. If the Dubyang pulse is floating and slow, and Shaoyang is slightly tight, slight indicates blood deficiency, tight indicates slight cold, this is Shu Ru (scrofula-like condition). The disease belongs to the Lung. The accumulation in the Lung is named Xi Ben, located under the right hypochondrium, covering an area as large as a cup. If unresolved for a long time, the patient experiences shivering chills and fever, rebellious Qi with panting and coughing, developing Lung Yong (abscess). Why does it occur on Jia/Yi days in Spring? Heart disease transmits to Lung, Lung should transmit to Liver, but Liver is in its Wang (prosperous) state in Spring, and the prosperous does not accept evil. Lung tries to return to Heart, but Heart refuses, so it stays and knots into an accumulation. Thus, Xi Ben is known to occur in Spring.
In Lung disease, the complexion is white, the body feels only cold without fever, there is frequent coughing, and the pulse is slight and slow; this is treatable. One should take Wuweizi Dabu Fei Tang (Five-Flavor Fruit Greatly Tonifying Lung Decoction) and Xie Fei San (Draining Lung Powder). In Spring, needle Shaoshang (LU11); in Summer, fish Ji (LU10); both using draining method. In Late Summer, needle Taiyuan (LU9); in Autumn, Jingqu (LU8); in Winter, Chize (LU5); all using tonifying method. Also, moxa Danzhong (CV17) 100 cones, and 25 cones beside the third thoracic vertebra on the back. Patients with Lung disease will surely have panting, coughing, rebellious Qi, shoulder breathing, back pain, sweating, spasms of the sacrum, yin, thigh, knee, thighs, buttocks, calves, and feet pain. Deficiency leads to shortness of breath, inability to complete inhalation, deafness, and dry throat. Select channels of Hand Taiyin, and blood points of Foot Taiyang lateral, Jueyin medial, and Shaoyin. When pathogen is in the Lung, skin hurts, chills and fever appear, Qi rebels upward, panting occurs, sweating happens, and cough moves the shoulders and back. Select Yingzhong (chest points), external transmission points, beside the third thoracic vertebra on the back; press with hand where it feels comfortable, then needle. Select Qufen (LI18) to vent the pathogen. The Hand Taiyin channel originates in the Middle Jiao, descends to connect with the Large Intestine, returns along the stomach opening, ascends through the diaphragm, and belongs to the Lung. From the Lung system, it emerges horizontally under the axilla, descends along the medial aspect of the arm, anterior to the Shaoyin Pericardium channel, descends to the elbow middle, follows the medial aspect of the forearm along the lower edge of the bone, enters the Cunkou, ascends to the Thenar eminence, follows the Yuji (LU10), and exits at the tip of the thumb. Its branch goes straight from behind the wrist to the inner edge of the index finger, exiting at its tip. When the channel is disturbed, disease manifests as Lung fullness, bloating with panting and coughing, pain in the Qufen, and severely, crossing hands and fainting, called Arm Jue. This channel governs diseases generated by the Lung: cough, rebellious Qi, panting with sound, vexation, chest fullness, pain along the anterior medial border of the arm, and heat in the palm. Excess Qi leads to shoulder and back pain, sweating with aversion to wind, frequent and scanty urination; Deficient Qi leads to shoulder and back pain, cold sensation, shortness of breath insufficient for respiration, change in urine color, and sudden incontinence. In Excess, the Cunkou pulse is three times larger than the Renying pulse; in Deficiency, the Cunkou pulse is smaller than the Renying pulse. The collateral of Hand Taiyin is named Lieque (LU7). It originates in the gap between the axilla (or wrist), diverges to the Yangming channel. Its branch runs parallel to the Taiyin channel, directly enters the palm, and scatters in the Yuji. In Excess, the thenar eminence bulges; in Deficiency, there are yawning and coughing, and urinary incontinence/frequency. Needle 1.5 cun from the wrist. In Lung disease, the body should have fever, cough, shortness of breath, and expectoration of pus and blood. The pulse should be short and choppy, but now it is floating and large; the complexion should be white, but is red; this is Fire overcoming Metal, a major reversal, ten deaths, untreatable.
