The manifestations of slow convulsion (Man Jing) include vomiting or diarrhea, phlegm rattling in the throat with slight panting, eyes open but spirit sluggish, sleeping with eyelids retracted exposing the eyeballs, sudden jumping and convulsions alternating with periods of calm, sometimes body heat, sometimes body coldness, sometimes heat in the four limbs, sometimes cold air from mouth and nose, pale white or pale cyan complexion, sometimes dark cyan between eyebrows and lips, and a pulse that is deep, slow, scattered, or slack. This is generally caused by excessive use of cold-cooling medicines during the treatment of rapid convulsion (Ji Jing), or by a too-rapid transformation and transmission of the disease. It may also result from unceasing vomiting and diarrhea, or from sudden severe vomiting and diarrhea due to Qi deficiency. In summer, if there is latent heat in the Spleen and Stomach causing severe vomiting and diarrhea, one should resolve the summerheat and clear heat, rather than solely focusing on securing Yang. It can be formed by viscera deficiency leading to watery diarrhea, by Qi collapse from chronic dysentery, by purging accumulated pathogens, by hematemesis or hematochezia, or by the transmission of Cold Damage (Shang Han) into a Yin syndrome. It may arise from prolonged cough developing into epilepsy, from unceasing epileptic seizures, from intestinal parasites rushing to the heart, or from testicular swelling, hernia, and abdominal pain. Additionally, excessive sweating leading to Spleen distress, irritability, thirst, edema of the four limbs, constipation of urine and stool, rapid-progressing malnutrition (Zou Ma Ji Gan), and other conditions can transmit into slow convulsion patterns. Only when vomiting, diarrhea, accumulation, and dysentery lead to deficiency does the condition change very rapidly. Whenever vomiting and diarrhea have just occurred, it constitutes slow convulsion, requiring warming the Middle and supporting the Interior. If convulsions are urgent, this indicates the initial transmission of slow convulsion, still retaining Yang manifestations; do not mistakenly treat it as rapid convulsion. The popular saying that "slow convulsions equal slow convulsion" is incorrect; adhering to this often leads to misidentifying slow spleen wind (Man Pi Feng) as slow convulsion. For slow convulsion, it is more severe in males when caused by diarrhea, and in females when caused by vomiting. Furthermore, there are five types of vomiting and five types of diarrhea, each with specific causes and treatments. Ancients said: "Patients fear convulsions but not diarrhea; doctors fear diarrhea but not convulsions." If diarrhea is unceasing, treat the diarrhea first; treating wind simultaneously will exacerbate the convulsions. For other syndromes, treat according to the original cause. If slow convulsion transmits from rapid convulsion, only cut off wind and regulate the Stomach, balancing Yin and Yang; do not use exclusively Yang-warming medicines, lest Yang becomes excessive and transforms back into rapid convulsion. Medication should be neither excessive nor insufficient. For rapid convulsion, due to blockage of Guan Ge (gate and partition), slight use of Niuhuang (Calculus Bovis) and Shexiang (Musk) to open orifices and settle convulsions is acceptable. For slow convulsion, characterized by Yin excess and Yang deficiency with emptiness in all meridians, one should not use opening-orifice methods or cool the Zang organs, as this easily transforms into slow spleen wind. In critical slow convulsion, if eyes are fixed and staring, blinking without looking left/right or rolling up, with cold limbs, profuse sweating, dark/blackish complexion and nails, and flaccid limbs, it is extremely severe. Slow convulsion presents with eyes half-open/half-closed, drowsy but not fully asleep, pulse floating or deep, body hot or cold, vomiting/diarrhea or none, accepting or rejecting milk, named Half-Yin Half-Yang Combined Disease, similar to Shaoyang syndrome in Cold Damage. Treatment requires investigating the source; do not generalize as slow convulsion. If caused by vomiting/diarrhea, use Tang Shi Xing Pi San (Wake the Spleen Powder); other cases follow suit. Since slow convulsion belongs to Yin, assess the depth of Yin/Yang deficiency; avoid overly warm-drying agents. [Qian Yi] Slow convulsion occurs after serious illness, vomiting/diarrhea, or vomiting without diarrhea, transforming into Spleen/Stomach deficiency. Symptoms include whole-body coldness, cold breath from mouth/nose, intermittent limb twitching, drowsiness with exposed eyes, indicating absence of Yang. Treat with Gualou Tang (Trichosanthes Decoction). A child of Wang in Dongdu had vomiting/diarrhea; physicians used purgatives causing extreme deficiency, transforming into slow convulsion with drowsiness, exposed eyes, limb twitching, and cold body. Qian Yi diagnosed slow convulsion and prescribed Gualou Tang. As the child's Stomach Qi was substantial, eyes opened and body warmed. The father suspected urinary/fecal retention and asked for diuretics/purgatives. Physicians used Bazheng San (Eight-Herb Correcting Powder) without effect, body became cold again, and they asked Qian Yi to promote urination. Qian Yi stated promoting urination is inappropriate and would cause coldness. Indeed, the body became cold. Qian Yi explained that inability to eat indicates Stomach deficiency; promoting urination/stool would exhaust Spleen/Stomach, leading to coldness, closed eyes, and death. Fortunately, fetal Qi was strong. Qian Yi used Yihuang San (Yellow Protecting Powder) and Shijunzi Wan (Quince Pill) four doses, allowing slight intake. By noon, the child could eat. Promoting urination/stool damages Spleen/Stomach Yang; tonify Spleen, do not attack further. Later, the child lost speech; physicians treated aphasia. Qian Yi questioned how an aphasic patient could open eyes and eat, noting no lockjaw. Using Dihuang (Rehmannia) to tonify Kidney was wrong; cooling diuretics damaged Spleen/Kidney. Now Spleen is solid, Kidney deficient; tonify Kidney for safety. After half a month, speech returned; full recovery in one month. [Li Gao] Slow convulsion involves complete Yin syndromes and viscera pathology. Prolonged vomiting/diarrhea damages Spleen/Stomach; untreated, it becomes slow convulsion, termed Yu (muscle twitching), resembling convulsion but less intense. Due to Spleen/Stomach deficiency, stools are loose; remove wind from Spleen using Xuanfeng San (Disperse Wind Powder) first, then Yihuang San and Shijunzi Wan to stabilize, stopping diarrhea. Delayed treatment leads to Spleen/Stomach deficiency and Liver Wood overacting; this is slow convulsion, treat with warming tonification using Qianghuo Gao (Notopterygium Ointment). [Zhu Danxi] Yan Xiaozhong’s compilation of Qian’s formulas incorrectly claims Yihuang San tonifies Earth. Dingxiang (Clove) is acrid-hot and aids Fire, worsening Earth deficiency. Qingjupi (Green Tangerine Peel) drains Lung Metal; Dingxiang drains Lung/Large Intestine. When Spleen is excess, drain the Son (Metal); but here Spleen is deficient, so draining Son and aiding Fire doubly injures Earth, likely fatal. In wind-Wood excess with surging Right Guan pulse, palm/abdominal heat, such drugs are contraindicated. If Cold Water attacks Spleen Earth causing vomiting, abdominal pain, green/white dysentery, Yihuang San is excellent. I propose a formula: Huangqi (Astragalus) 2 qian, Renshen (Ginseng) 1 qian, Zhigancao (Honey-fried Licorice) 0.5 qian, Baishaoyao (White Peony Root) 1 qian. These four are sweet-warm, tonifying Yuan Qi; sweet drains fire. *Neijing* states: "For heat rampant within, drain with sweet, astringe with sour." Baishaoyao is sour-cold, draining fire and Liver, tonifying Lung Metal. This strengthens Metal/Earth positions; Metal strong, Fire weak, Wind-Wood cannot attack Earth, then drain wind pathogen. Yihuang San and Lizhong Wan treat Spleen/Stomach cold-damp excess. For latent fire, fatigue insufficiency, or slow convulsion from heat drugs like Badou (Croton), these are fatal. Slow convulsion arises from prolonged diarrhea damaging Spleen/Stomach. Qian’s use of Qianghuo Gao is erroneous. Spleen deficiency results from Fire Evil attacking Earth (deficient evil from posterior). Fire excess strengthens Wood, which attacks Earth. Tonify Earth source via Heart Meridian with sweet-warm; drain Fire in Spleen with sweet-cold; tonify Metal in Spleen with sour-cool. This makes Metal strong/Fire weak in Spleen Earth, rendering Wind-Wood empty. [Chen Zimou] For crying with convulsions, unstable sleep, use Sijunzi Tang plus Quanxie (Scorpion) fried, Gouteng (Uncaria) fried, Fuzi (Aconite) fried, equal parts, decoct. For Spleen/Stomach weakness generating wind, use Sijunzi Tang plus Chao Banxia Qu (Fried Pinellia Ferment), Moshizi (Lapis Lazuli), equal parts, powder, add winter melon seeds, decoct. [Xue Ji] Frequent vomiting/diarrhea impending slow convulsion: Use Qian’s Baizhu San plus Shanyao (Dioscorea), Bian Dou (Lablab) fried, Rou Doukou (Nutmeg) flour-roasted, 1 qian each, add ginger slice, decoct. If slow convulsion established, add Xixin (Asarum), Tianma (Gastrodia) 1 qian each, Quanxie 3 (remove sting), Fuzi 0.8 qian flour-roasted. Convulsion with diarrhea: Renshen, Lingzhi (Poria), Shaoyao wine-fried, Baizhu, ginger decoct. Summer: add Huanglian (Coptis), Gancao, Zhuye (Bamboo Leaf). [Zeng Shirong] Ancient texts lack basis for treating slow convulsion, recording only Yin Epilepsy. Slow convulsion is Yin, static with slow spasms. Causes: external Wind-Cold, internal vomiting/diarrhea, post-serious illness, or iatrogenic error. Symptoms: dull eyes, dim spirit, moving limbs, drooling, slight warmth, upward/sideways eye gaze, clenched fists with spasms, leg movements. Male left/female right spasms are favorable; reverse is unfavorable. Cold breath or sunken fontanelle indicates extreme deficiency; deep powerless pulse, sleeping with eyes half-open indicates True Yang depletion and Yin dominance. Yin generates Cold (Water), Water generates Liver Wood (Wind), Wood attacks Spleen Earth. Stomach is Spleen’s palace, so Wind in Stomach causes Yu. Symptoms: shrugging shoulders, hanging arms, trembling. Treat with Qingzhou Baiwanzi (Qingzhou White Pills) and Suhexiang Wan (Styrax Pills) pounded with ginger juice, taken with rice water. Add Jin Ye Dan (Golden Liquid Pill) for extreme deficiency. Then Chonghe Yin with Qibao San, ginger decoct, to disperse wind/resolve vomiting/diarrhea. Intermittently use Weiling Tang to rescue exterior/interior. Unstopping vomiting: Dingtu Yin. Unstopping diarrhea: Liuzhu San or Sheng Tang to remove stomach wind/settle spasms/clear spirit. Wuling San guides rebellion, harmonizes Ying/Wei, balances Yin/Yang. Phlegm-heavy, white lips, ice-cold limbs, unconsciousness: extreme deficiency-slow. Use Guzhen Tang immediately to generate Stomach Qi. Once Qi returns, use Xingpi San and Chenxiang Yin for regulation. [Xue Ji] Slow convulsion symptoms: vomiting/diarrhea, phlegm rattle, panting, open eyes/sluggish spirit, drowsiness/exposed eyes, jumping/spasms alternating, body hot/cold, pale cyan/white face or cyan/red lips, slow/deep/slack pulse. Caused by congenital insufficiency, chronic Spleen deficiency, or frequent use of harsh/draining drugs. If Earth deficiency fails to generate Metal, Metal fails to control Wood, and Wood insults Earth, use Wuwei Yigong San plus Danggui (Angelica), Suanzaoren (Ziziphus), assisted by Gouteng Yinzi to tonify Earth/control Wood. If Spleen Earth deficiency-cold, use Liujunzi Tang plus Pa Jiang (Fried Ginger), Muxiang (Costus); if ineffective, urgently add Fuzi to restore Yang. Yin-blood is born from Spleen Earth; use Sijunzi Tang, Danggui, Suanzaoren. For Yuan Qi depletion causing coma, moxa Baihui point urgently; delaying until phlegm persists leads to Qi collapse and death.
