Damp-Heat 5 (Recovery Phase of Intestinal Typhoid)
Patient Ni, Male, 37 years old
First Visit
The damp-heat disease had just recovered after a month, but in the past two days, low-grade fever and dry mouth suddenly appeared again. The patient felt irritable and thirsty, with general fatigue. There was a feeling of fullness, blockage, and discomfort in the epigastrium, frequent nausea, poor appetite, red tongue with greasy coating, and both pulses were soggy, rapid, and weak. The condition resembled a relapse of damp-heat due to overexertion, with residual heat not yet cleared. Therefore, the low-grade fever was not severe, fatigue was present, the stomach did not desire food, and there was frequent nausea. The treatment method employed was clearing heat and generating fluids, benefiting qi and harmonizing the stomach.
Zhu Ye (Bamboo Leaf) 3g, Sheng Shi Gao (Raw Gypsum Fibrosum) 12g, Bei Sha Shen (Glehniae Radix) 15g, Ban Xia (Pinelliae Rhizoma Preparatum) 9g, Mai Men Dong (Ophiopogonis Tuber) 9g, Dan Dou Chi (Sojae Semen Preparatum) 9g, Shan Zhi (Gardeniae Fructus) 3g, Sheng Gan Cao (Licorice Root) 3g, 2 doses.
Second Visit
The low-grade fever did not recur; body temperature was 36.5°C. Thirst and irritability had ceased, and appetite gradually improved, but epigastric stuffiness remained. Since damp-heat had just resolved, residual heat lingered. Clearing heat from the Qi level was combined with slightly tonifying healthy qi, resolving damp-stagnation to open the stomach. Dietary adjustments were advised. Cold foods, sweets, and sticky foods were prohibited.
Zhu Ye (Bamboo Leaf) and Zhu Ru (Bambusa Textilis Caulis) 3g each, Sheng Shi Gao (Raw Gypsum Fibrosum) 9g, Sha Shen (Glehniae Radix or Adenophorae Radix) 9g, Xing Ren (Armeniacae Semen Amarum) [dose missing], Ban Xia (Pinelliae Rhizoma Preparatum) 9g, Dan Dou Chi (Sojae Semen Preparatum) 9g, Fu Ling (Poria) 9g, Bai Kou Ren (Amomi Fructus Rotundus) powder 0.3g taken separately by mixing, Ji Nei Jin (Galli Gigerii Endothelium Corneum) 9g, 2 doses.
Third Visit
After continuously taking medicines to clear heat and open the stomach, the low fever subsided, fatigue decreased, and epigastric blockage and stuffiness lightened. Diet and bowel/bladder functions returned to normal. Since damp-heat had just resolved and healthy qi had not yet recovered, rest for two weeks was required to prevent relapse due to overexertion.
[Commentary]: When damp-heat has just resolved, relapse caused by fatigue and labor leads to low fever, irritability and thirst, fatigue, and anorexia. This indicates that residual heat is not fully cleared and healthy qi is insufficient. Thus, the method of *Zhuye Shigao Tang* (Cinnamon Twig and Gypsum Decoction - Note: Text says Zhuye Shigao Tang, usually implies formula structure) is adopted to clear heat, generate fluids, benefit qi, and harmonize the stomach. Whenever warm diseases have just resolved, one must guard against relapse due to overexertion (Lao Fu) or improper diet (Shi Fu). Overexertion or dietary indiscretion, such as eating meat too early or in excess, can cause recurrent fever, which may be high or low and difficult to resolve. One must use methods to clear residual heat and harmonize stomach qi; when the stomach qi is harmonized, the disease heals. Therefore, in the second visit of this case, medicines to open the stomach and promote digestion were added to resolve dampness and stagnation, so that residual heat would not linger.
Damp-Heat 6 (Fever of Unknown Origin)
Patient Li, Female, 20 years old
First Visit
Chief Complaint: On August 5th, the patient felt aversion to cold and fever, with body temperature between 37°C and 39°C. Diagnosed with viral influenza at a certain hospital, she took exterior-releasing medicines, but the fever persisted. Due to continuous fever for 19 days, she was hospitalized. Systematic Western medical examinations diagnosed "Fever of Unknown Origin." She underwent fluid support therapy and treatments with Sulfamethoxazole/Trimethoprim, Penicillin, Kanamycin, and Isoniazid. Chinese herbal medicines used included Qingying Tang (Clearing Nutritive Level Decoction), Tiaowei Chengqi Tang (Regulating the Stomach and Directing Downward Decoction), Baihu Tang (White Tiger Decoction), Zixue (Purple Snow), Zhibao (Treasure of Perfection), and Qinjiao Biejia Tang (Gentian Macrocarpa and Turtle Shell Decoction), but the effect was poor. Body temperature remained around 38°C. On September 30th, Teacher Zhao Shaoqin was consulted.
Examination: Symptoms included fever, worse in the afternoon, sweating without relief of fever, dizziness with heaviness, thirst without desire to drink, chest tightness, anorexia, and general fatigue and lassitude.
Pattern Differentiation: Dampness obstructing and heat lurking; fever is severe in the afternoon, sweating does not relieve fever; damp-heat pouring downward, yellow urine. The disease is located in the Middle Jiao, diffusing up and down.
Treatment Method: Prescribe Acrid-Opening and Bitter-Descending methods, assisted by Aromatic and Light-Diuretic agents.
Prescription: Pei Lan Ye (Eupatorii Herba) 10g (add later), Huo Xiang (Agastaches Herba) 10g (add later), Xing Ren (Armeniacae Semen Amarum) 10g, Dan Dou Chi (Sojae Semen Preparatum) 10g, Ban Xia (Pinelliae Rhizoma Preparatum) 10g, Huang Qin (Scutellariae Radix) 10g, Mu Xiang (Aucklandiae Radix) 6g, Ma Wei Lian (Coptidis Rhizoma) 10g, Qian Hu (Peucedani Radix) 6g, Da Fu Pi (Arecae Pericarpium) 10g, Chao Mai Ya (Hordei Fructus Germinatus) 10g, Zhi Zi (Gardeniae Fructus) 6g.
3 doses, decocted in water. Avoid fishy, sweet, and greasy foods.
Second Visit, October 4th.
After taking the medicine, the fever intensity decreased slightly. Because damp-heat pathogens are difficult to eliminate quickly, the original prescription was continued for four more doses to aim for complete recovery.
Third Visit, October 7th.
Body temperature dropped to 37.1°C, only neck soreness remained. Continuing the original prescription for two doses resulted in the disappearance of all symptoms, and she was discharged cured on October 12th.
[Commentary]: This case involves damp-heat accumulating and stagnating in the Qi level of the Middle Jiao. Previous misuses of sweating, clearing nutritive level, purging, opening orifices, and nourishing yin methods, along with cold and cloying medicines, further aided dampness. Damp-heat congestion blocked qi mechanism; since dampness was not transformed, heat was not removed. This explains the "prolonged unremitting fever." Damp-heat disease is a combination of dampness and heat, truly difficult to resolve. If dampness is not removed, heat is hard to eliminate. Therefore, treatment should prioritize removing dampness. Methods such as aromatic transformation, acrid-opening and bitter-descending, and light diuresis should be used to first regulate qi mechanism and unblock the San Jiao (Triple Burner). In the prescription, Huo Xiang and Pei Lan, along with Qian Hu and Xing Ren, transform dampness aromatically and diffuse Lung qi. Since the Lung governs qi, when qi moves, dampness transforms; when dampness transforms, heat is easily cleared. Zhi Zi and Dou Chi clear and diffuse stagnant heat; long-standing damp-heat becomes stale air, and the combination of Zhi Zi and Dou Chi is excellent at venting stale air, thus having the function of diffusing Yang and relieving depression. Ban Xia, Huang Qin, and Lian (Coptidis Rhizoma) use acrid-opening and bitter-descending to clear heat and dry dampness, opening and draining damp-heat accumulation in the Middle Jiao. Mu Xiang, Da Fu Pi, and Mai Ya regulate qi stagnation, move water pathways, aid digestion, and facilitate the San Jiao. The San Jiao is the pathway of water and grain, the beginning and end of qi, the official of drainage, where water passages emerge. When the San Jiao is smooth, upper and lower diffusion occurs, and pathogens leave. Patent urination and defecation after medication indicate unblocked San Jiao, hence generalized sweating and fever resolution. In the first visit, three doses were given, slightly reducing fever but pulse/symptoms unchanged. Thus, in the second visit, the original formula was continued for 4 doses, resulting in sudden recovery. Only those with firm conviction in their mind can manage such cases. If one changes the prescription in the second visit seeking quick results, it risks failure. In short, damp-heat obstruction in damp-heat disease is not easily removed quickly; dampness must be slowly transformed. When quantitative change reaches qualitative change, a state of sudden clarity is achieved. Therefore, treating damp-heat requires firm conviction, avoiding frequent prescription changes. As long as pattern identification is accurate and principles are correct, following the law of medication will naturally lead to success. This case is an example.
Fu Shu (Pneumonia)
Patient Wang, Male, 87 years old
First Visit
Fever for seven days, cough and wheezing for five days, body temperature fluctuating between 38°C and 39.5°C. Diagnosed with pneumonia by Western medicine, treated with Gentamicin injections and Tetracycline orally, with insignificant effect. Traditional Chinese Medicine consultation was requested. The patient had persistent high fever, sweating with dry mouth, cough and wheezing unable to lie flat, large amount of yellow sticky sputum, no bowel movement for five days, scanty yellow urine, slight abdominal fullness without pain, red tongue with yellow greasy coating, slippery and rapid pulse. This belongs to warm pathogen entering the Lung, scorching fluids into phlegm, phlegm obstructing qi mechanism, Lung losing diffusion and descending functions, causing cough and wheezing. Lung and Large Intestine are interior-exterior related; if Lung qi does not descend, Fu qi does not flow, hence no bowel movement for several days. Treatment: Diffuse Lung, wash away phlegm, purge Fu, drain heat. Fortunately, although elderly, the patient's constitution is still robust, and healthy qi is sufficient, allowing temporary use of purging methods. Prescribe Xuanbai Chengqi Tang modified.
Xing Ren (Armeniacae Semen Amarum) 6g, Quan Gua Lou (Trichosanthis Fructus) 20g, Zhi Pa Ye (Loquat Leaf Preparatum) 15g, Sheng Shi Gao (Raw Gypsum Fibrosum) 15g, Dai Ge San (Indigo Naturalis and Clam Shell Powder) 10g (wrapped), Sheng Da Huang (Rhei Radix et Rhizoma) 6g (add later), 1 dose.
Second Visit
After medication, three bowel movements occurred, stools were foul-smelling, abdomen was no longer full, cough and wheezing lightened. Continue original prescription without Da Huang, 2 doses.
Third Visit
After medication, major symptoms reduced significantly, body temperature 37.8°C, cough and wheezing mild, able to lie flat and sleep. Red tongue with yellow-white coating, wiry, fine, small, slippery pulse. Prescribe clearing and肃ifying Lung qi, assisting with harmonizing stomach.
Xing Ren (Armeniacae Semen Amarum) 6g, Jie Geng (Platycodonis Radix) 6g, Gua Lou Pi (Trichosanthis Pericarpium) 10g, Qing Ban Xia (Pinelliae Rhizoma Preparatum) 10g, Jiao Gu Ya (Hordei Fructus Germinatus Carbonized) 10g, Sheng Gan Cao (Licorice Root) 6g, Sang Bai Pi (Mori Cortex) 6g, Lu Gen (Imperatae Rhizoma) 20g, 2 doses.
After medication, all symptoms resolved. Body temperature normal, X-ray showed no pathological changes in both lungs, discharged cured.
[Commentary]: In autumn, contracting warm pathogen and lingering heat from summer leads to Fu Shu. Pathogen lurks in Lung, refining fluids into phlegm, Lung loses diffusion and descending, causing wheezing/cough inability to lie flat, abundant yellow sputum, persistent fever. Although main symptoms are in Lung, pathology relates to Fu qi obstruction. Five days of constipation is the key to treatment. Lung and Large Intestine are interior-exterior; pathogen congested in Lung requires purging Large Intestine. Here, Wu Juzhong's Xuanbai Chengqi Tang is selected. Although the patient is elderly, purging was used because his constitution is strong. After passing foul stools, heat drains with stool, so Da Huang is removed. Finally, stomach-harmonizing herbs are added, so despite advanced age, injury is avoided.
Autumn Dryness 1 (Upper Respiratory Tract Infection)
Patient Wang, Male, 56 years old
First Visit
Contracted warm-dryness pathogen externally, Lung meridian scorched. Dry mouth and thirst, fever, slight aversion to wind-cold, headache, red throat, dry and parched nose, hacking cough with little sputum, yellow urine, slightly dry stool. Tip and sides of tongue red, thin white dry coating, floating and rapid pulse, right side slightly larger, deep pressure wiry-fine-rapid. Warm pathogen dry-heat injures Yin level. Use clearing-moistening and diffusing-descending method to肃ify Lung and stop cough. Spicy, oily, and rich flavors are prohibited.
Bei Sha Shen (Glehniae Radix) 10g, Chao Zhi Pi (Gardeniae Pericarpium) 6g, Qian Hu (Peucedani Radix) 6g, Yu Zhu (Polygonati Odorati Rhizoma) 6g, Mao Gen (Imperatae Rhizoma) and Lu Gen (Imperatae Rhizoma) 15g each, Fresh Pear 1 (slice with skin, remove core, decoct), 3 doses.
Second Visit
After taking three doses, chills and fever subsided, dry throat and nose alleviated, dry cough lightened, bowel movement smooth. Pulse rapid tendency diminished. Original prescription taken for five more doses led to cure.
[Commentary]: Long sunny and rainless autumn leads to dryness prevalence. Those contracting it often suffer warm-dryness: dry cough without sputum, dry mouth/nose, dry/painful throat. This is dryness injuring Lung Yin, hijacking fluids. Treatment focuses on clearing-moistening to generate fluids, assisting with diffusing-subsiding to expel pathogen. Medicines are balanced, pure, and agile, worthy of study.
Autumn Dryness 2 (Bronchitis)
Patient Yuan, Male, 37 years old
First Visit
Season is autumn, long drought, fever and headache, body temperature 38.3°C, dry cough with little sputum, blood in sputum this morning, dry nose/throat, irritability with desire to drink, self-perceived fatigue and shortness of breath. Self-administered Ju Hong Wan 12 pills (4 pills daily for 3 days), condition worsened. Pattern: Autumn contraction of dry-heat, Lung fluids injured. Should use Sweet-Cold clearing-moistening method. But self-administered Ju Hong Wan, which is for drying dampness and transforming phlegm, suitable for elderly with damp-phlegm. Mistakenly taking dry-hot medicine further injures Yin. Dry cough damages Lung collaterals, hence thick sticky sputum with blood stains. Pulse fine-wiry-rapid, crimson dry-cracked tongue. Imitate Yu Chang's Qingzao Jiufei Fa (Clearing Dryness Saving Lung Method), moisten dryness and stop bleeding.
Bei Sha Shen (Glehniae Radix) 25g, Zhe Bei Mu (Fritillariae Thunbergii Bulbus) and Chuan Bei Mu (Fritillariae Cirrhosae Bulbus) 10g each, Wan Can Sha (Bombyx Batryticatus) 12g, Xing Ren (Armeniacae Semen Amarum) 10g, Dan Dou Chi (Sojae Semen Preparatum) 10g, Chao Zhi Pi (Gardeniae Pericarpium) 6g, Qian Hu (Peucedani Radix) 3g, Xian Mao Gen (Imperatae Rhizoma) 30g, Dai Ge San (Indigo Naturalis and Clam Shell Powder) 12g (wrapped), Fresh Pear 1 (slice with skin, remove core, decoct), 3 doses.
Second Visit
Continuously taking clearing-dryness, moistening-Lung, stopping-cough medicine, hemoptysis stopped in two days, nasal/throat dryness lightened, slept well at night. Cough with sticky clumps of sputum, still fatigued, dry mouth/thirst. Fever/headache ceased, body temperature 37°C, wiry-fine pulses. Rapid tendency slowing, crimson tongue with moister coating. Dry-heat gradually decreasing, Yin injury slightly recovering. Still propose Sweet-Cold moistening-dryness method.
Sheng Sang Pi (Mori Cortex) 10g, Di Gu Pi (Lycii Cortex) 10g, Fei Yu Zhu (Polygonati Odorati Rhizoma) 10g, Mai Men Dong (Ophiopogonis Tuber) 10g, Nan Sha Shen (Adenophorae Radix) and Bei Sha Shen (Glehniae Radix) 15g each, Chuan Bei Mu (Fritillariae Cirrhosae Bulbus) 6g, Chao Shan Zhi (Gardeniae Fructus) 6g, Dai Ge San (Indigo Naturalis and Clam Shell Powder) 10g (wrapped), Xian Mao Gen (Imperatae Rhizoma) and Lu Gen (Imperatae Rhizoma) 30g each, Dong Gua Zi (Benincasae Semen) 30g, 3 doses.
Third Visit
After drinking during festival, cough and blood in sputum recurred, fever intensified, body temperature 38.1°C, red tongue dry mouth, heavy cough/chest pain. X-ray: "Increased lung markings bilaterally, bronchitis phenomenon," wiry-slippery-rapid pulses. Constitution inherently Yin-injured with heat excess, season long drought/rainless, dry-heat heavy. Cough/hemoptysis just healed, dry-heat just retreated, Yin injury not recovered. Festival drinking excessive, plus spicy/greedy food, aids heat and injures Yin. Again use Sweet-Cold nourishing-Yin, draining-heat, stopping-red. Diet caution: abstain from alcohol for 100 days to prevent worsening hemoptysis.
Xian Sheng Di (Rehmanniae Radix Recens) 30g, Su Zi (Perillae Fructus) 6g, Chuan Lian Zi (Meliae Fructus) 10g, Huang Qin (Scutellariae Radix) 10g, Da Ji (Great Thistle Root) and Xiao Ji (Lesser Thistle Root) 10g each, Chao Huai Mi (Sophorae Flavescentis Flos) 10g, Dai Ge San (Indigo Naturalis and Clam Shell Powder) 10g (wrapped), Xian Mao Gen (Imperatae Rhizoma) 30g, Bei Sha Shen (Glehniae Radix) 30g, Jiao San Xian (Charred Three Immortals) 10g each, Yunnan Baiyao 1 bottle (taken in 4 doses staggered), 2 doses.
Fourth Visit
After medication, no hemoptysis, cough light, fever gradually decreasing, body temperature 37.5°C, self-reported chest pain lightened. Red rough-cracked tongue, dry mouth/thirst, fine-wiry-small-slippery pulses, rapid image decreasing. Again use Sweet-Cold nourishing-Yin, moistening-dryness, stopping-red.
Xian Sheng Di (Rehmanniae Radix Recens) 30g, Chuan Lian Zi (Meliae Fructus) 10g, Su Zi (Perillae Fructus) 10g, Huang Qin (Scutellariae Radix) 10g, Bai Tou Weng (Pulsatillae Radix) 10g, Qian Cao (Rubiae Radix et Rhizoma) 10g, Dai Ge San (Indigo Naturalis and Clam Shell Powder) 10g (wrapped), Gan He Ye (Nelumbinis Folium) 10g, Ou Jie (Nelumbinis Rhizomatis Node) 10g, Bei Sha Shen (Glehniae Radix) 25g, Yunnan Baiyao 1 bottle (taken in 6 doses), 3 doses.
Fifth Visit
Recently no cough/blood in sputum, fever subsided, body temperature 36.9°C. X-ray bilateral lungs normal. Wiry-fine-small-slippery pulses, red tongue dry mouth, diet/bowel/bladder normal. Advised to stop medication and rest one week, avoid spicy/greasy and all stimulating foods.
[Commentary]: In golden autumn, dryness prevails. Contracting its qi causes Autumn Dryness. Lung is delicate organ, highest position, connects with heaven qi. If contracted by seasonal dryness, Lung is injured first. Thus, Autumn Dryness location is definitely Lung. Classic says: "Dryness dominates leading to dryness." Although dryness belongs to Autumn Metal, its nature is like fire, easily injuring Yin fluids. Hence, Autumn Dryness disease features dryness syndromes: dry cough little sputum, sticky bloody sputum, dry nose/throat. Treating Autumn Dryness should focus on clearing-moistening, as stated "Dryness requires moistening." This patient initially self-administered warm-dryness transforming-phlegm Ju Hong Wan, further injuring Yin and aiding dryness, worsening condition. Gradually healed after taking Sweet-Cold moistening agents. Regrettably, he did not observe taboos, drank alcohol and ate spicy food, causing recurrence. Repeated Sweet-Cold nourishing-Yin finally brought full peace. This case shows dietary taboos are crucial in warm disease treatment, almost equal to medication efficacy. Those not observing taboos fail treatment; those observing taboos and caring carefully achieve double result with half effort.
Thus, Teacher Zhao earnestly advises abstaining from alcohol for 100 days and avoiding spicy/stimulating foods. Students should pay attention here.
Winter Warmth 1 (Fever of Unknown Origin)
Patient Wang, Male, 50 years old
Patient had fever for 5-6 days, transferred from out of town to a certain hospital. Hospitalized for 4 days treating fever of unknown origin. Used Chinese medicines including Raw Gypsum (3 liang), Zhi Mu, Gua Lou, Lian Qiao, Sheng Di, Yuan Shen, Hua Fen, Mao Gen, Lu Gen, Sheng Mu Li, Xi Jiao, Ling Yang Fen, Angong Niuhuang Wan, Zixue Dan, etc. Taken in several doses with insignificant effect. Used Western medicines including Penicillin, Streptomycin, Kanamycin, Tetracycline, etc., also ineffective. Consultation requested.
Patient unconscious, fever persisting, eyes not opening, lips scorched dark, anterior teeth dry, thin crimson tongue, cracked without fluid, difficulty opening mouth, deep wiry-slippery-rapid pulse. Occurring in winter, this belongs to warm pathogen deeply entering Ying (Nutritive) level. Qi mechanism suppressed by cold medicines, San Jiao blocked, no path for ascending/descending, fluids do not reach. Trending towards internal closure and external collapse. Treatment: Regulate ascending/descending to benefit San Jiao, diffuse qi mechanism to seek turning qi.
Chan Yi (Bombyx Batryticatus) 1.5 qian, Xing Ren (Armeniacae Semen Amarum) 3 qian, Qian Hu (Peucedani Radix) 1 qian, Pei Lan (Eupatorii Herba) 3 qian, Chang Pu (Acori Tatarinowii Rhizoma) 3 qian, Lu Gen and Mao Gen (Imperatae Rhizoma) 1 liang each, Pian Jiang Huang (Curcumae Longae Rhizoma) 2 qian, Bai Kou Ren (Amomi Fructus Rotundus) 1 qian, Ban Xia (Pinelliae Rhizoma Preparatum) 3 qian, Tong Cao (Tetrapanacis Medulla) 0.5 qian.
2 doses. Fever subsided, body cooled, pulse quiet, spirit clear, generalized slight sweating, then cured.
[Commentary]: Winter contracting non-seasonal warm pathogen is called Winter Warmth. Initially should release Wei (Defensive) level, letting pathogen exit externally. Alas, doctors used large amounts of cold medicines, closing qi mechanism, forcing pathogen into Ying level, causing unconsciousness, critical signs appeared. Teacher Zhao used diffusing-unblocking qi mechanism to vent pathogen outward. After medication, fever subsided, spirit cleared, sweating occurred, then cured. If seeing coma immediately assuming pathogen trapped in Heart Pericardium and using San Bao (Three Treasures) type medicines, likely repeating mistakes, treatment failing. It suffices to show "Venting Heat Turning Qi" is essential method for treating warm diseases, cautious use of cold medicines is key to preventing pathogen trapping. Understanding this principle means grasping the three flavors of treating warmth.
