随火炽而飞越。登高而呼者,火腾于上以呼救援也;弃衣而走者,憎衣之添热也;
Travelers and peddlers, galloping under the scorching sun, contract summer-heat qi (Summer-heat Qi), and suddenly collapse. People think it is heatstroke (Heatstroke), but who knows it is Zhong Ye (Heat Closure). "Ye" refers to heat, and summer-heat is also heat; why distinguish them? The heat of summer-heat enters from the outside, whereas the heat of Ye emanates from the inside. Travelers and peddlers endure hard labor and travel, causing internal heat to want to escape, while external summer-heat suppresses it. Thus, they suddenly collapse because summer-heat is on the outside and internal heat is closed off. If one treats only the summer-heat without promoting the release of internal heat, the qi will remain closed inside, and the heat will not dissipate. The treatment method should be to disperse the internal heat and assist with flavors that relieve summer-heat. Use the formula Jiu Ye Dan (Rescue Ye Decoction):
Qing Hao (Artemisiae Annuae Herba, 5 qian) Fu Shen (Poria with Hostwood, 3 qian) Bai Zhu (Atractylodis Macrocephalae Rhizoma, 3 qian) Xiang Ru (Moslae Herba, 1 qian) Zhi Mu (Anemarrhenae Rhizoma, 1 qian) Gan Ge (Puerariae Radix, 1 qian)
Gan Cao (Glycyrrhizae Radix et Rhizoma, 5 fen) Decoct in water and take orally.
After one dose, qi flows freely; after two doses, heat dissipates; a third dose is unnecessary.
This formula uses Qing Hao to calm the fire in the stomach and resolve summer-heat qi, hence serving as the Sovereign herb. Xiang Ru resolves summer-heat, and Gan Ge disperses heat, hence serving as Assistant herbs. Furthermore, fearing that extreme internal heat, if only dispersed without cooling, might flare upwards, Zhi Mu is added to cool it. Bai Zhu and Fu Ling are used to benefit the waist and umbilicus and unblock the Bladder, ensuring that the qi of fire and heat all flow downwards towards the Small Intestine to exit completely. Since the fire moves downwards, it naturally does not rebel and rush upwards, and both external summer-heat and internal heat dissolve into nothingness.
For this condition, Jie Shu San (Relieve Summer-heat Powder) is also effective.
Xiang Ru Moshi (Folium Perillae, 3 qian each) Gan Cao Huang Lian (Coptidis Rhizoma, 1 qian each) Bai Zhu (1 liang) Bai Bian Dou (Dolichoris Semen, 2 qian) Bai Dou Kou (Amomi Fructus Rotundus, 1 grain)
Decoct in water and take orally. One dose leads to recovery.
Young men from wealthy families often eat melons and fruits to chill their stomachs. Suddenly contracting summer-heat qi, they collapse suddenly; this is true heatstroke. Generally, people from wealthy backgrounds have originally weak constitutions, and with excessive desires, they inevitably develop internal cold. Adding melons and fruits increases this coldness. When internal cold reaches its extreme, external heat easily penetrates deeply. In individuals with Yin deficiency, summer-heat qi takes advantage of the deficiency to enter. The treatment should not prioritize dispelling summer-heat but must focus on tonifying Qi. However, since it is due to Yin deficiency leading to invasion by Yang pathogenic factors, one might think tonifying Yin is primary. It is unknown that when Yang pathogenic factors enter the Spleen, they rely on Yin qi. Tonifying Yin makes Yin qi prosperous, which becomes favorable for Yang pathogenic factors; Yang combines with Yin. There is fear that if Yin is weak, it cannot match Yang, but if one aids Yin qi, regardless of whether Yin can repel Yang, the Yang pathogen will dwell there for a long time and cause changes. Only by tonifying Yang qi, making it robust, and adding flavors that disperse summer-heat, will the pathogenic Yang dare not oppose the healthy Yang, retreating without battle. Use the formula:
Ren Shen (Ginseng Radix, 5 qian) Fu Shen (5 qian) Bai Zhu (5 qian) Xiang Ru (2 qian) Bai Bian Dou (2 qian) Chen Pi (Citri Reticulatae Pericarpium, 5 fen)
Gan Cao (1 qian) Decoct in water and take orally. After one dose, qi returns; after two doses, summer-heat fully dissipates.
This formula is named San Shu Hui Yang Tang (Disperse Summer-heat and Restore Yang Decoction). Ren Shen, Fu Ling, Bai Zhu, and Bai Bian Dou in the formula are all herbs that strengthen the Spleen and tonify Qi to restore Yang. Xiang Ru is used to disperse summer-heat. Why such disparity in dosage and weight? It is unknown that Yin deficiency here refers to Spleen Yin deficiency. Although the Spleen belongs to Yin, it requires Yang-tonifying herbs to be effective. Moreover, with very strong Yang pathogens, large dosages are needed to counteract them. If small amounts are used, there is a risk of defeat that cannot be contained. Even if victory is achieved, after summer-heat recedes, Yuan Qi (Primal Qi) may not recover rapidly. Rather than tonifying Yang after summer-heat leaves, is it not more beneficial to use larger amounts during the peak of the pathogen, so that Zheng Qi (Healthy Qi) remains intact and the pathogen is removed quickly?
For this condition, Jia Wei Si Jun Tang (Modified Four Gentlemen Decoction) is also effective.
Ren Shen Bai Zhu (each 5 qian) Gan Cao Xiang Ru (each 1 qian) Fu Ling (3 qian) Pao Jiang (Prepared Zingiberis Rhizoma, 3 fen) Decoct in water and take orally. Two doses lead to recovery.
In heatstroke, Qi fails to ascend and descend, resulting in Huo Luan (Cholera-like syndrome with vomiting and diarrhea), opisthotonos (Angle Bow Reverse Stretching), alternating chills and fever, and chest/heart distress. People think it is internal heat from summer-heat, but who knows it is a disorder of Yin and Yang? When the Yin and Yang Qi of the human body are harmonious, pathogens cannot interfere. If Yin and Yang cannot interact, pathogens take advantage of the deficiency to enter. When pathogens enter the Zang-Fu organs, they aid the strong and do not aid the weak. Seeing strong Yin, they aid Yin; seeing strong Yang, they aid Yang. In summer, people often have Yin deficiency and Yang excess. Pathogens enter due to Yin deficiency, bullying the weakness of Yin. However, seeing strong Yang, they aid Yang rather than Yin. Seeing the pathogen aiding Yang, Yin becomes jealous of Yang's prosperity and fights. Yang dislikes the pathogen siding with Yang and tries to blame the pathogen on Yin, but Yin refuses. Thus, Yin and Yang become chaotic, Qi does not communicate, ascending fails above, descending fails below. Huo Luan vomiting and diarrhea block the middle, opisthotonos troubles the exterior. Yin not interacting with Yang causes cold; Yang not interacting with Yin causes heat. The interior of the heart and chest becomes a battlefield, how can one not feel distressed? Therefore, the treatment method should harmonize Yin and Yang Qi, slightly assist with anti-summer-heat agents, and regulate slowly, without abrupt suppression. Use the formula He He Yin Yang Tang (Harmonize Yin and Yang Decoction):
Ren Shen (1 qian) Bai Zhu (2 qian) Fu Ling (5 qian) Xiang Ru (1 qian) Huo Xiang (Patchouliae Herba, 1 qian) Su Ye (Perillae Folium, 1 qian)
Nong Po (Magnoliae Officinalis Cortex, 5 fen) Chen Pi (3 fen) Zhi Qiao (Aurantii Fructus Immaturus, 3 fen) Sha Ren (Amomi Fructus, 1 grain) Tian Hua Fen (Trichosanthis Radix, 1 qian) Decoct in water, let cool, and take slowly.
After one dose, Yin and Yang harmonize; after two doses, all symptoms resolve; a third dose is unnecessary.
This formula separates the clear and turbid aspects of Yin and Yang, connects the floating and sinking of upper and lower, and harmonizes during times of rebellion, achieving miraculous results. It assists Healthy Qi without increasing Fire, expels Pathogens without damaging Qi, and turns events into non-events.
For this condition, Jia Jian Liu Jun Tang (Modified Six Gentlemen Decoction) is also effective.
Ren Shen Fu Ling Bai Shao (Paeoniae Radix Alba, each 3 qian) Bai Zhu (1 liang) Xiang Ru (1 qian) Sha Ren (1 grain) Chen Pi (5 fen) Ban Xia (Pinelliae Rhizoma, 1 qian)
Decoct in water and take orally. One dose brings stability.
Contracting summer-heat Qi, abdominal pain occurs, desire to vomit but unable, desire to defecate but unable; this is named Gan Huo Luan (Dry Cholera). When pathogens enter the Stomach, vomiting expels pathogens upwards; when pathogens enter the abdomen, defecation directs pathogens downwards. If pathogens go up, they do not enter the middle; if pathogens go down, they do not stay inside. Now, without vomiting or defecation, pathogens neither go up nor down, firmly residing in the Middle Jiao. It is like rebels revolting; although the four borders are peaceful, sudden bloodshed occurs at the city gate, and corpses lie in the interior. At this time, unless a general who disregards his life rallies loyal and brave troops,冒着 arrows and stones to seize the gate and quell the disaster, how can one pacify the rebels in an instant and settle the trouble in a moment? The treatment method urgently requires:
Ren Shen (1 liang) Gua Di (Momordicae Fructus, 7 pieces) Decoct in a large bowl of water. Drinking it induces vomiting, and recovery follows.
This formula is named Ren Shen Gua Di San (Ginseng and Momordica Powder). For such diseases, the pulse must be deep and hidden; without vomiting, death ensues. Ancients knew to use Gua Di to induce vomiting but dared not add Ren Shen. The cause of this condition is deficiency of Stomach Qi, allowing summer-heat pathogens to enter. Now, inducing heavy vomiting would further damage the Stomach; without Ren Shen, Stomach Qi cannot be stabilized during vomiting. Moreover, since Stomach Qi is inherently weak and summer-heat pathogens are obstructed, even using Gua Di to induce vomiting, the Qi is timid and cannot send upwards. Often, there is a desire to vomit but inability to do so. Even if vomiting is triggered, the amount is small, so how can pathogens exit quickly? Only by using Ren Shen up to one liang, Yang Qi becomes vigorous, possessing the strength to expel pathogens and surge upwards. With the assistance of Gua Di, how could one not vomit heavily? Pathogens scatter rapidly due to vomiting, and Healthy Qi remains undamaged. It is like internal chaos settled and people living in peace; why disturb the ramparts of the suburbs?
For this condition, Shen Lu Tang (Ginseng Stem Decoction) is also good.
Ren Shen Lu (Ginseng Radix Caulis, 2 liang) Decoct in boiling water (one bowl), mix with well water (one bowl), add a little salt, and drink evenly. Sweep the throat with a goose feather to induce vomiting. Vomiting brings relief. If vomiting does not occur but intake is tolerated, recovery also ensues.
Extreme summer-heat causes madness: climbing high and shouting, discarding clothes and running, jumping into water upon seeing it. People think it is invasion by summer-heat toxin, but who knows it is assisted by Stomach Fire? Summer-heat entering the Zang-Fu organs mostly attacks the Heart, not the Stomach. Summer-heat and Heart both belong to Fire. The Stomach is the son of the Heart. Seeing the Heart attacked by summer-heat pathogens, the Stomach emits Fire from within Earth to defend. The Stomach is a mansion of abundant Qi and Blood. If Fire does not erupt, fine; but once it erupts, its fierce power is often irresistible. Fearing the strength of Stomach Fire, summer-heat pathogens retreat into the Heart. The Heart prefers cold and dislikes heat, fearing direct invasion by summer-heat, feeling insecure. Stomach Fire rages, summer-heat pathogens invade the Heart directly and cannot exit. Thus, Fire is unleashed, burning outside the Heart. How can the Heart withstand the pressure of two Fires? It must flee downstairs. Once the Heart Monarch exits, Shen (Spirit) has no foundation, flying away with the blazing fire. Climbing high and shouting is Fire rising upwards calling for help; discarding clothes and running is hating clothes for adding heat; jumping into water is liking Water to overcome Fire. At this time, the Heart lacks fluid nourishment, and sweat escapes profusely from the skin, losing Yang. This is a disease of exhaustion of both Yin and Yang, a critical and urgent state. If Fire is not greatly purged, how can the prairie fire be extinguished? Use the formula San Sheng Tang (Three Sages Decoction):
Ren Shen (3 liang) Shi Gao (Gypsum Fibrosum, 3 liang) Xuan Shen (Scrophulariae Radix, 3 liang) Decoct several bowls of water, administer orally. After one dose, madness settles; after two doses, Spirit calms. Do not use a third dose. Additionally, use Huan Tu Tang (Slow Plan Decoction):
Xuan Shen (2 liang) Ren Shen (1 liang) Mai Dong (Ophiopogonis Radix, 1 liang) Qing Hao (1 liang) Decoct in water and take orally. After two doses, summer-heat and heat are both resolved.
San Sheng Tang uses Shi Gao up to 3 liang, and Ren Shen, Xuan Shen each up to 3 liang, which may seem somewhat domineering. However, extreme fire cannot be extinguished by a cup of water. Without heavy usage, Kidney Water will be scorched dry instantly. Although Shi Gao is used in large quantities, the amount of Ren Shen is equal, truly sufficient to control the majesty of Bai Hu Tang (White Tiger Decoction). Thus, it purges Stomach Fire without damaging Stomach Qi. Xuan Shen moistens and generates Water; when Water is generated, Fire is easier to extinguish. Huan Tu Tang does not use Shi Gao because Stomach Fire has already been greatly purged; what remains is just lingering smoke and dying flames, appearing and disappearing. Why use heavy Yin wind and rain to wash it again? Thus, Mai Dong and Qing Hao are changed to both benefit Yin and extinguish Fire, achieving the effect of gentle rain. Someone asks: Madness caused by summer-heat seems suitable for relieving summer-heat, yet San Sheng Tang only purges Fire and ignores summer-heat; how can it work? It is unknown that summer-heat is also Fire; purging Fire is purging summer-heat. If Xiang Ru and Huo Xiang (herbs clearing summer-heat) were added to purging Fire, Shi Gao would want to descend, while Xiang Ru and Huo Xiang would want to disperse externally, interfering with Shi Gao's action, preventing direct purging of Fire.
For this condition, San Qing Tang (Three Clears Decoction) is also miraculous.
Xuan Shen (4 liang) Shi Gao (1 liang) Qing Hao (1 liang) Decoct in water and take orally. One dose brings peace.
Heatstroke naturally involves much sweating. Now, there is heavy sweating like rain, continuous and unstoppable. People think it is sweating leading to Yang loss, a fatal symptom, but who knows it is sweating leading to Yin loss, a fatal symptom? Summer-heat damages the Heart, and sweat leaks outwards, yet the Heart itself does not sweat. How can there be sweat? This sweat is generated in the Kidney, not the Heart. The fluids of the Heart are generated by the Kidney. Is Heart sweat not from the Kidney? Although sweating leads to Yang loss, it is due to Yang excess, not Yin deficiency. It is merely that Yin cannot control Yang, so Yang becomes excessive; Yin cannot secure Yang, so Yang escapes. Yin and Yang are mutually rooted. If Yin cannot secure Yang, but Yang can cling to Yin, Yang might return to Yin. Unfortunately, Yang goes out and does not return. Yin roots in Yang; seeing Yang leave and not stay, Yin follows and exits together, exhausting the essence in the Kidney, turning entirely into sweat and leaking heavily. Consider how much fluid is in the Heart; how can it produce rain-like sweat? Clearly, it is Kidney sweat, not Heart sweat. Since sweat is from the Kidney and not the Heart, loss is also Yin loss, not Yang loss. Then, should one let Yin loss lead to death? There is still a method to save life. Use the formula Jiu Wang Sheng Yin Dan (Save Life and Generate Yin Pill):
Ren Shen (2 liang) Shu Di (Rehmanniae Radix Preparata, 4 liang) Shan Zhu Yu (Corni Fructus, 2 liang) Bei Wu Wei (Schisandrae Fructus, 5 qian) Fu Shen (1 liang) Bai Shao (1 liang)
Decoct in water and take orally.
In this formula, Shu Di, Shan Zhu Yu, and Wu Wei Zi are all substances that fill essence and replenish water. Fu Shen calms the Heart, Bai Shao gathers the Hun (Ethereal Soul), and Ren Shen restores Yang; this is known to people. Since Yin has escaped externally, how else to irrigate depleted Yin but by filling essence and marrow? Since Yang has escaped externally, how else to attract scattered Yang but by tonifying Guan Yuan (CV4)? Shan Zhu Yu and Wu Wei Zi, while tonifying Yin, are also astringents. Yin benefits from tonification, Water generates, giving the Kidney a root; sweat benefits from tonification, fluids turn, leaving the Heart unharmed. With Fu Shen to calm it and Bai Shao to gather it, Yang returns and Yin reverses, having a swift divine opportunity.
For this condition, Ren Shen Qing Hao Tang (Ginseng and Artemisia Decoction) is also miraculous.
Ren Shen (2 liang) Sheng Di (Rehmanniae Radix, 1 liang) Mai Dong (1 liang) Qing Hao (5 qian) Bei Wu Wei Zi (1 qian) Decoct in water and take orally. If sweating stops, life is saved; otherwise, there is no rescue.
Extreme summer-heat causes delusions and hallucinations, speaking wildly as if seeing ghosts, yet the person remains quiet without irritability, and thirst is not severe. This is extreme Cold battling, Cold drawing out the Shen, resembling madness, but it is not madness caused by extreme Heat. Heatstroke is clearly a Heat syndrome; how can Heat transform into Cold and present symptoms resembling madness? Generally, this person has inherent Yin deficiency and weak Yang. Summer-heat pathogens take advantage of dual deficiency of Yin and Yang, entering from the Lung to the Heart. With insufficient Heart Qi, Shen frequently escapes to the Kidney. The Yin-Cold Qi in the Kidney rises, so summer-heat exits the Heart and lingers in the Lung Meridian. Having retreated outside the Heart, the Heart Monarch fears invasion by summer-heat and relies on the Mother of Liver Wood to calm the Shen. When Shen enters the Liver, Liver Hun becomes restless, wandering outside the body, causing delusions of ghosts and strange speech. With Hun wandering outside, Shen resides in the Hun chamber, enjoying tranquility. Moreover, Liver Wood has no excessive Fire, and Kidney Yin-Cold Qi presses on the Heart Monarch, helping to prevent summer-heat invasion, instilling fearlessness, thus no irritability arises. Only the Lung Qi alone suffers from summer-heat; Fire punishes Metal causing thirst. However, seeing the Lung Mother punished, Kidney Yin-Cold Qi rushes up to save the Lung, so although thirsty, it is not severe. So, what is the treatment? Disperse summer-heat in the Lung, tonify Earth Qi of the Spleen and Stomach. When Earth Qi prospers, Lung Qi also prospers. Prosperous Lung Qi can resist pathogens; with dispersing herbs, summer-heat is hard to retain. Summer-heat disperses, Hun returns, Shen returns; this is inevitable. Use the formula Hu Jin Tang (Protect Metal Decoction):
Mai Dong (1 liang) Ren Shen (3 qian) Bai He (Lilii Bulbus, 5 qian) Fu Ling (3 qian) Zi Wan (Asteris Radix, 1 qian) Xiang Ru (1 qian)
Gan Cao (1 qian) Decoct in water and take orally.
Two doses lead to recovery.
This formula only tonifies Qi of the Lung, Spleen, and Stomach. It does not save the Heart to benefit Cold, does not assist the Kidney to purge Fire, does not tonify the Liver to drive out the Soul. Yet Hun returns to the Liver, and Shen returns to the Heart spontaneously, because the pathogen is controlled. Why drive the pathogen too severely? Zheng Qi is not greatly deficient, why tonify Zheng Qi too much? One should not treat downwards because the pathogen is above, nor treat heavily below where Zheng Qi is light.
For this condition, Ren Shen Mai Dong Tang (Ginseng and Ophiopogon Decoction) is also excellent.
Ren Shen (2 liang) Mai Dong (3 liang) Decoct in water and take orally.
Summer-heat causes hematemesis pouring like a basin, purely purple-black in color, asthma and distension, inability to lie down, thirst for drinking but discomfort afterwards. People think it is extreme summer-heat moving Blood, but who knows it is extreme Kidney Heat causing vomiting of blood? Clearly, it is heatstroke moving blood to vomit, yet attributed to Kidney Heat? Because summer-fire induces Kidney Fire. The Fire in the Kidney is Dragon-Thunder Fire. Dragon-Thunder originally hides underground. In summer, the underground is very cold, Dragon-Thunder cannot hide downwards and leaks upwards. Its anger激ates, forming fierce thunderbolts, lightning splitting the sky, heavy rain pouring; this is common. The human body also has Dragon-Thunder Fire, hiding in the Kidney, its Qi responding to Heaven's Dragon-Thunder. Summer-heat is also Heaven's Dragon-Thunder Fire. Extreme summer-heat causes Dragon-Thunder to emerge from the earth, a clear proof of similar energies attracting each other. If Dragon-Thunder Fire in the body does not move, summer-heat cannot attract it. If Kidney Water is originally deficient, Kidney Fire is ready to move; encountering Heaven's Dragon Fire, similar energies resonate, how can it not vigorously rise? Having risen vigorously, the two Fires clash, their momentum becoming fiercer, rushing upwards, carrying all blood in the Stomach to vomit heavily. Stomach blood should be red, but turning purple-black shows its Dragon-Thunder Qi. Wherever Dragon-Thunder strikes, trees turn purple-black; how much more so for viscera passed through? The Stomach Qi passed through is greatly damaged. Qi damage causes rebellion, Qi rebellion causes asthma. With damaged Stomach Qi, how can new blood be generated quickly to nourish the Stomach? This is why the chest and hypochondrium become distended. The Stomach is the Gate of the Kidney. If the Gate is not closed, lacking opening and closing authority at night, how can one lie down? Blood loss dries fluids, fluid dryness causes thirst. Insufficient internal water seeks external water to save from burning, but drinking water brings no comfort because Dragon-Thunder Fire is Yin Fire, not Yang Fire. The treatment should greatly tonify Water in the Kidney to restrain Dragon-Thunder Fire, not greatly purge Dragon-Thunder Fire to damage Stomach Qi. Use the formula Pei Lin Gao (Abundant Rain Paste):
Xuan Shen (2 liang) Ren Shen (1 liang) Sheng Di (2 liang) Mai Dong (2 liang) Niu Xi (Achyranthis Bidentatae Radix, 5 qian) Jing Jie (Schizonepetae Herba, stir-fried black, 3 qian)
Decoct in water and take orally. One dose stops bleeding, two doses resolve asthma and distension, three doses stop thirst, four doses achieve full recovery. After recovery, continuing to take Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) is acceptable.
This formula greatly tonifies Kidney Water. When Water is sufficient, Dragon-Thunder Fire naturally returns to its residence in the Kidney. Once Fire is safe in the Kidney residence, blood stops at the Stomach Gate. Why use Huang Bo (Phellodendri Cortex) and Zhi Mu to purge Fire, or Xiang Ru and Huo Xiang to disperse summer-heat? Moreover, purging Fire makes Fire more intense, inevitably damaging Stomach Earth; dispersing summer-heat makes summer-heat hard to retreat, inevitably consuming Lung Metal. Ultimately, blood cannot stop, Fire cannot be extinguished, leading to death.
For this condition, Dan Hao Tang (Moutan and Artemisia Decoction) is also miraculous.
Dan Pi (Moutan Cortex, 3 liang) Jing Jie (3 qian) Qing Hao (1 liang) Decoct in water and take orally.
Summer-heat Qi causes cold feet, hot upper body, irritability, and vomiting upon drinking. People think it is a syndrome of Lower Cold and Upper Heat, blocked by summer-heat isolating Yin and Yang, but who knows it is because summer-heat disperses and Kidney Fire cannot descend? The Dragon-Thunder Fire in the human body, because
