Mr. Lou of Anchang: Yang fails to interact with Yin, resulting in insomnia (inability to fall asleep). The pulse is rapid and forceful on the right side; the tip of the tongue is red; accompanied by cough, rebellious Qi, and deficient Stomach Qi. Decoct in clear water, four doses.
Commentary by Jie: The Stomach is the sea of the six fu organs; its Qi should descend smoothly. Here, Yangming Qi rebels upward and attacks the Lung, causing cough and insomnia. Therefore, treatment focuses on descending Lung Qi, nourishing Stomach Qi, and calming the Shen.
Mr. Shen of Yuzhuang: Liver Yang is unsettled; night sleep is slightly improved. The pulse is still wiry on the right, and fine-rapid on the left; there is harsh cough; tongue color remains as before. It is appropriate to soften the Liver and Kidney to harmonize Yang. Commentary by Jie: Liver Yin is deficient, and Yang fails to submerge. The Liver stores the Hun (Ethereal Soul), and the Lung stores the Po (Corporeal Soul). Now that Hun and Po are unsettled, one cannot sleep peacefully at night. Liver Qi rebels and overacts on the Lung, causing harsh cough. Thus, treatment primarily focuses on submerging Yang, nourishing Yin, softening the Liver, and clearing the Lung.
Mr. Huang: Harsh cough, palpitations, inability to sleep at night. Pulse is deficient and fine. Stomach Qi is dull with nausea. Suitable for warming the Gallbladder, harmonizing the Stomach, clearing the Lung, and calming the Shen.

Three doses.
Commentary by Jie: Luo Dongyi stated: The Gallbladder is the official of uprightness, a pure and quiet organ. It prefers tranquility and abhors disturbance; it prefers gentleness and dislikes stagnation. This is because the Wood organ, due to long-term stagnation, fails to achieve tranquility. When Phlegm-Qi rebels upward, it is due to damp-heat from the Earth element counter-attacking, preventing the Wood Qi from flowing freely. Therefore, such cases must first clear heat and resolve obstruction in the Sanjiao (Triple Burner). Observing this, this condition is purely due to Shaoyang failing to receive gentle warmth, leading to various symptoms arising together. This prescription's method deeply aligns with Luo's principles.
Mr. Xiang of Anchang: Heart feels fearful and anxious as if hanging in the air; night sleep is uneasy. Pulse is deficient and fine; the Left Guan position is fine and forceful. Tongue is red; occasionally speaks hastily. Tentatively suitable for Buxin Dan (Heart-Nourishing Pill) modified. (December 29th, Xinhai year.)

* Junci Medulla (Dengxin) - 7 strands
Four doses.
Commentary by Jie: Heart Blood is insufficient, and the Shen (Spirit) is restless, leading to feelings of fear and anxiety as if hanging, and insomnia. Thus, treatment focuses on tonifying Heart Shen. Using Shengdi to nourish Yin fluids, Suanzaoren to tonify the Heart, Fushen and Yuanzhi to calm the Shen, Chihu (Dendrobium) to moisten fluids, and Ganju to nourish the Liver. Also using Puzhisu (Akebia Fruit) to secure the Kidney, and Dengxin as a guide. This formula is indeed a specific agent for treating insomnia due to Heart deficiency.
Mr. Wang of Anchang: Vertigo occurs frequently, palpitations, little sleep. Pulse is forceful. Tongue color is transparent. Physical strength is weak with edema on the dorsum of the foot. Suitable for softening the Liver and Kidney to calm the Shen. (April)

Second Visit: Vertigo has decreased, night sleep is slightly better, sweating during sleep. Pulse is deficient, physical weakness persists. Still appropriate to follow previous methods with modifications. (February 23rd)

* Mori Fructus (Sangshenzi) - 3 qian
Decoct in clear water, eight doses.
Commentary by Jie: Liver Yin is depleted and fails to store the Hun, causing vertigo and little sleep. Heart Shen is unsettled, causing palpitations and weakness. Additionally, damp-heat stagnates in the Lower Jiao, causing edema on the dorsum of the foot. Therefore, within the context of nourishing Liver and Kidney, Muli and Zexie are added to dispel dampness. Since the medication addresses both aspects, efficacy is naturally achieved. At the second visit, nocturnal sweating indicates that Yin fluids are still not secured and are leaking outward. However, since the edema has resolved, treatment now focuses solely on softening the Liver, tonifying the Kidney, and calming the Shen.
Mr. Shen of Xizhuang: Stomach Qi remains dull; unable to sleep at night. Pulse is soggy and fine. Tongue coating is yellow and slippery; dampness has not yet cleared. Suitable for Wendan Tang (Warm the Gallbladder Decoction) modified. (January 10th)

* Tectonae Herba (Tongcao) - 1.5 qian

Decoct in clear water, three doses.
Commentary by Jie: The Gallbladder is a pure and quiet organ. Here, Phlegm-dampness has not cleared, and residual heat in the chest and diaphragm has not subsided, causing insomnia. Stomach appetite remains intact. Treatment uses Wendan Tang modified, simultaneously clearing damp-heat and nourishing the Stomach.
Mr. Xu of Anchang: Damp-heat has reduced slightly, stomach appetite has increased somewhat. Pulse is wiry-soggy on the right, deficient-fine on the left. Night sleep is slightly better, but defecation is difficult. Suitable for harmonizing the Stomach and calming the Shen. (June 6th, Yisi 20th day.)

* Salviae Miltiorrhizae Radix et Rhizoma (Danshen) - 3 qian
* Hordeum Vulgare Germinatum (Guya) - 4 qian
Five doses.
Commentary by Jie: Damp-heat is retreating, but Kidney fluids have been consumed and fail to ascend to nourish the Heart, hence insomnia. Stomach fluids have not recovered, hence constipation. Thus, treatment focuses on harmonizing the Stomach and calming the Shen, tonifying the Heart and nourishing the Kidney, with slight assistance from diuretics to leach out dampness. Once Stomach Qi is harmonized, sleep will naturally become peaceful.
