Mr. Ma from Daxizhuang suffered from damage progressing into consumptive disease (Xu Lao), presenting with choking cough, aversion to cold, and night sweats. He had a history of blood loss. His pulse was fine and rapid, and his tongue coating was yellow. This indicates lung Qi injury; it is not a trivial condition.

Second visit: The previous case record is listed above. The current pulse remains fine and rapid. Cough produces purulent sputum mixed with blood. In summary, lung Qi injury with aversion to cold belongs to severe consumptive disease.

Four doses.
Commentary by Shao Jie: The Lung governs the skin and hair. When the Lung is injured, it loses its protective function. Heat damages Yuan Qi; when Qi is damaged, it cannot generate fluids or consolidate body fluids, leading to choking cough, aversion to cold, and night sweats. However, consumptive disease progressing to blood loss is indeed an extremely severe condition. Based on these symptoms, Huang Qi Jian Zhong Tang should be used urgently to strengthen Middle Qi, thereby increasing food intake and fluid production, eventually replenishing blood and essence to restore the deficiency of True Yin. Since this patient's Lung Qi is injured, the initial prescription consists entirely of herbs that clear the Lung and generate fluids. It is supplemented with Dong Chong Xia Cao, which treats pulmonary consumption, and Yun Mu Shi, which benefits the Lung meridian; this is indeed an excellent formula for treating pulmonary consumption. According to Dai's Bai Ji Pi Pa Wan (using one liang of Bai Ji, five qian each of Pi Pa Ye and Ou Jie, ground into fine powder; separately stir-fried E Jiao with Ge Fen, dissolved in Sheng Di juice over heat, then mixed with the previous powder to form pills the size of longan fruits, one pill per dose), it is a specialized formula for treating hemoptysis due to lung blood loss. This consultation seems to follow Dai's intent in prescribing treatment, achieving similar results through different methods.
Mr. Wang from Xiafangqiao contracted illness due to summer-heat and dampness injuring Qi. Symptoms include cough and tidal fever. The clear orifices are already obscured. The right pulse is fine and rapid, while the left pulse is forceful. The tongue surface is bright red and glossy, with white debris in the center. Body fluids are severely damaged, having developed into consumptive disease. The situation is difficult to manage. Clearing and descending, nourishing the Stomach, and resolving phlegm are appropriate. (September 30th.)

Two doses.
Commentary by Shao Jie: Summer-heat and dampness enter via the mouth and nose, damaging the Qi level. Treatment should employ pungent-cool and slightly bitter methods to clear the Upper Jiao Qi level for recovery. However, due to prolonged neglect, the fluids of the Lung and Stomach have been depleted, resulting in consumptive disease. Coupled with the obscuration of the clear orifices, the condition is indeed critical. This prescription肃肺 (clears the Lung) and nourishes fluids to resolve phlegm; the therapeutic method is highly appropriate.
Mrs. Ma from Shufu suffers from consumptive cough and tidal fever. Her pulse is choppy; the left pulse is fine and rapid. The tongue coating is white with a red center. Menstruation is obstructed, and her physique is frail. This is not a trivial condition.

Second visit: Cough persists. Frailty and tidal fever continue. The pulse is deficient and fine, with the right side being wiry. The tongue coating is slightly yellow. There is epigastric fullness and diarrhea. Ultimately, this is a critical condition. Clearing Qi and harmonizing the Middle is indicated, awaiting the restoration of Zheng Qi (June 9th).

Commentary by Shao Jie: Both the Chong and Ren meridians are damaged, causing the failure of Yin and Yang Qi to interact. The fluids of the Five Zang organs are consumed, prolonging into consumptive weakness. The initial prescription calms the Chong vessel and invigorates blood, while clearing the Lung and nourishing the Stomach, addressing both aspects simultaneously, but failed to produce effects. The second visit revealed diarrhea, indicating Spleen Qi deficiency. Although herbs supporting the Spleen and regulating Qi were added to the Lung-Stomach nourishing formula, it remains difficult to cure.
Mr. Fan from Anchang has persistent night sweats. All six pulses are deficient and fine. The tongue is white with little moisture. Cough and shortness of breath are present, along with epigastric pain. Prevention of hemorrhage is necessary. (February 19th.)

Third visit: Previous medication was effective. Cough and night sweats have significantly reduced. The pulse is deficient and fine. The tongue coating is slippery and white. Physical strength is weak, and the waist and ankles are unsteady. It is advisable to continue modifying the previous method. (March 19th.)

Commentary by Shao Jie: This condition arises because Kidney fluids are deficient and cannot ascend to nourish the Heart, causing Yang to fail to anchor. In spring, when Liver Wood dominates, Liver Yang transforms into Wind, attacking the Lung and causing complications such as hemoptysis and night sweats. The initial prescription, combining Yin-nourishing and Yang-anchoring herbs with Liver-calming and Lung-clearing agents, effectively prevented hemorrhage and settled cough, proving to be an excellent formula and yielding results. The subsequent prescription protected Yang, eliminating night sweats. The final prescription addressed physical weakness and unstable waist/ankles by adding Kidney-tonifying and Heart-nourishing herbs. The progression was orderly and worthy of emulation.
