Malaria of the Foot-Taiyang (Bladder) Channel causes lower back pain and a heavy head, with cold sensation arising from the back. It presents with chills preceding fever, and after the fever subsides, sweating occurs but is difficult to stop. Treatment involves needling Weizhong (BL-40) until bleeding occurs.
Malaria of the Foot-Shaoyang (Gallbladder) Channel causes the body to feel flaccid and weak. The chills and fever are not severe. The patient dislikes seeing people; upon seeing others, they feel fearful and apprehensive. There is abundant heat and profuse sweating. Treatment involves needling the Foot-Shaoyang (Gallbladder) channel.
Malaria of the Foot-Yangming (Stomach) Channel causes initial chills, with shivering and severe coldness. After a long period, fever ensues. Upon the departure of heat, sweating occurs. The patient desires sunlight, moonlight, or fire warmth to feel comfortable. Treatment involves needling the acupoints on the dorsum of the foot of the Foot-Yangming (Stomach) channel.
Malaria of the Foot-Taiyin (Spleen) Channel causes dissatisfaction and frequent sighing. There is loss of appetite, frequent alternating chills and fever with sweating. Upon onset, vomiting often occurs; once vomiting ceases, the condition alleviates, and then treatment should be applied.
Malaria of the Foot-Shaoyin (Kidney) Channel causes severe vomiting, frequent chills and fever, with more heat than cold. The patient wishes to stay indoors with doors and windows closed. This type of malaria is difficult to cure.
Malaria of the Foot-Jueyin (Liver) Channel causes lower back pain and fullness in the lower abdomen. Urination is difficult, resembling strangury (Lóng), but it is not true strangury; rather, there is a frequent urge to urinate without passage, due to Qi deficiency. The abdomen feels depressed and uncomfortable. Treatment involves needling the Foot-Jueyin (Liver) channel.
Lung Malaria causes coldness in the heart/chest. When the cold is severe, fever follows. During the intervals of fever, the patient is prone to fright and hallucinations, as if seeing things. Treatment involves needling the Hand-Taiyin (Lung) and Hand-Yangming (Large Intestine) channels.
Heart Malaria causes mental restlessness. The patient strongly desires cold water. However, chills are predominant while fever is mild. Treatment involves needling the Hand-Shaoyin (Heart) channel.
Liver Malaria causes the complexion to appear pale green/cyan, with frequent sighing. The patient’s appearance resembles that of a corpse. Treatment involves needling the Foot-Jueyin (Liver) channel until bleeding occurs.
Spleen Malaria causes chills and abdominal pain. When fever arises, borborygmus (intestinal rumbling) occurs in the intestines; after the rumbling stops, sweating ensues. Treatment involves needling the Foot-Taiyin (Spleen) channel.
Kidney Malaria causes shivering, pain in the lower back and spine, difficulty turning over, and difficult defecation. The eyes appear dim/blurred, and the hands and feet are cold. Treatment involves needling the Foot-Taiyang (Bladder) and Foot-Shaoyin (Kidney) channels.
Stomach Malaria causes hunger before the attack begins. The patient feels hungry but cannot eat. After eating, there is a feeling of distension and fullness, with an enlarged abdomen. Treatment involves causing bleeding at the transverse vessels of the Foot-Yangming (Stomach) and Foot-Taiyin (Spleen) channels.
When malaria fever is just rising, needle the arterial pulse on the dorsum of the foot, open the aperture, and release blood; the chill/fever will cease immediately. When malaria chills are about to begin, needle the Hand-Yangming (Large Intestine), Hand-Taiyin (Lung), Foot-Yangming (Stomach), and Foot-Taiyin (Spleen) channels.
If the malaria pulse is full, large, and urgent, needle the Back-Shu points. Use a medium-sized needle, one point beside each of the Five Shu points of the Back-Shu, adjusting the amount of bloodletting according to the patient's fatness or thinness. If the malaria pulse is small, solid, and urgent, moxa the Foot-Shaoyin (Kidney) channel on the leg, and needle the Jing-Well points of the fingers/toes. If the malaria pulse is full, large, and urgent, needle the Back-Shu points, using the Five Shu points, one for each Back-Shu point, adjusting according to the blood flow. If the malaria pulse is slow, large, and deficient, use herbal medicine; acupuncture is not suitable.
Generally, when treating malaria, treatment should begin approximately the time it takes to eat a meal before the onset. If this timing is missed, the opportunity is lost. For various malarial conditions where the pulse is imperceptible, bleed the ten finger webs; once the blood flows, the disease will certainly resolve. First, observe any red spots on the body resembling adzuki beans and treat them all by pricking.
The twelve types of malaria have different onset times. By observing the clinical manifestations, one can determine which channel is affected. Needle approximately the time it takes to eat a meal before the onset. One needle application reduces the severity; two applications bring relief; three applications result in cure. If ineffective, bleed the two vessels under the tongue. If still ineffective, bleed the full meridians at the Xi-Cleft points. Also, needle the areas below the neck along the spine; this will certainly cure the disease. The two vessels under the tongue refer to Lianquan (CV-23).
When treating malaria, first ask where the pain started initially and needle that area first. If headache and heaviness start first, bleed the vertex, temples, and between the eyebrows. If neck and back pain start first, needle those areas. If lower back and back pain start first, bleed Weizhong (BL-40). If arm pain starts first, needle the interdigital spaces of the Hand-Shaoyin (Heart) and Hand-Yangming (Large Intestine) channels. If leg and shin soreness/pain start first, bleed the interdigital spaces of the Foot-Yangming (Stomach) channel.
Wind Malaria presents with sweating and aversion to wind upon onset. Bleed the Back-Shu points of the Three Yang channels.
Fu-Sui Pain (Pain of the Dorsal Marrow) presents with severe soreness and pain that cannot be pressed. Name: Fu-Sui Pain. Needle Juegu (GB-39) with a round-sharp needle until bleeding occurs; the pain will stop immediately.
For slight body pain, needle Zhiyin (BL-67) and the Jing-Well points of all Yin channels without bleeding. Treat every other day.
For malaria without thirst, occurring every other day, needle the Foot-Taiyang (Bladder) channel. For malaria with thirst, occurring every other day, also needle the Foot-Taiyang (Bladder) channel.
For Warm Malaria without sweating, apply the "Fifty-Nine Acupoints" needling method.
