When the Qi of the Middle Jiao (Middle Burner) is insufficient, urination and defecation become abnormal, and the intestines suffer from painful borborygmus (rumbling sounds). When the Qi of the Lower Jiao (Lower Burner) is insufficient, it leads to flaccidity and coldness of the limbs, along with a sensation of emptiness in the heart. Treatment involves tonifying the point located below the lateral malleolus and retaining the needle.
When the Qi of the Upper Jiao (Upper Burner) is insufficient, the head feels empty and unsatisfied, the ears suffer from painful tinnitus, and the head tilts painfully due to weakness. Treatment involves tonifying the point located below the lateral malleolus and retaining the needle.
For throat obstruction (Hou Bi) resulting in inability to speak, select points on the Foot Yangming Meridian; if speech is possible, select points on the Hand Yangming Meridian. If rebellious Qi rushes to the throat causing inability to speak, accompanied by cold hands and feet and constipation, select points on the Foot Shaoyin Meridian. For dryness in the pharynx and heat in the mouth feeling like sticky glue, select points on the Foot Shaoyin Meridian. For throat obstruction with tongue retraction, dry mouth, vexation, heart pain, and pain along the medial aspect of the arm extending to the inability to raise the hand to the head, select the point at the corner of the nail of the fourth finger (ring finger), approximately the width of a leek leaf from the tip (Guanchong, SI 1).
For toothache without aversion to cold drinks, select points on the Foot Yangming Meridian; for toothache with aversion to cold drinks, select points on the Hand Yangming Meridian. For dental caries pain, puncture the Hand Yangming Meridian; if ineffective, puncture the vessels entering the teeth, which will provide immediate relief.
For persistent epistaxis with heavy blood flow, select points on the Foot Taiyang Meridian; for dark or profuse bleeding, select points on the Hand Taiyang Meridian. If ineffective, puncture below the wrist bone (Wangu, SI 4); if still ineffective, bleed at Xi Zhong (Weizhong, BL 40).
For internal heat with panting, select the blood collaterals in the Foot Shaoyang Meridian. For chest fullness with panting, select the point at the tip of the great toe, approximately the width of a leek leaf from the nail (Yinbai, SP 1). For cold patterns, retain the needle; for heat patterns, withdraw quickly. Stop needling when the Qi descends.
For irregular emotions (anger and joy) with loss of appetite and diminishing voice, puncture the Foot Taiyin Meridian. For anger with excessive talking, puncture the Foot Shaoyang Meridian.
Puncture the Hand Yangming Meridian. If pain is severe, puncture the arterial pulse at the curve of the Foot Yangming Meridian (e.g., Zusanli, ST 36); seeing blood indicates immediate cessation. If ineffective, press the Renying point (ST 9) along the meridian for immediate relief.
For neck pain preventing flexion and extension, puncture the Foot Taiyang Meridian; for inability to turn the head left and right, puncture the Hand Taiyang Meridian.
For inability to lift the thigh and leg, take the point laterally in the joint space (Huantiao, GB 30), using a Yuanli needle; large needles are contraindicated. Deficiency of the Yangming Meridian leads to relaxation of the Zongjin (major tendons) and lack of tension in the Dai Mai (Girdle Vessel), resulting in foot flaccidity and inability to use the legs. Tonify the Ying-Spring points and unblock the Shu-Stream points.
For acute bloody diarrhea, select Ququan (LR 8).
Pain in the lower abdomen that prevents urination and defecation is termed Shan (Hernia). This results from cold invasion. Puncture the area between the lower abdomen and the thighs, and between the lumbar and iliac bones. Perform multiple punctures until the area becomes warm, then the disease will resolve. For sudden severe pain of Heart Shan, select points on the Foot Taiyin and Foot Jueyin meridians, thoroughly puncturing to remove blood from the collateral vessels.
