Sini Tang (Four Reversal Decoction): Treats exterior heat of Shanghan (Cold Damage) that has not entered the interior, where premature use of cold-natured purgatives causes the exterior heat to persist and sink into the interior as cold, resulting in incessant diarrhea. For spontaneous diarrhea caused by exterior heat and interior cold, urgently warm the interior; once the diarrhea stops, the condition is resolved.
It also treats Shaoyin disease (Disease of the Lesser Yin), characterized by a deep pulse, diarrhea, cold extremities, irritability, thirst, and vomiting.
Gancao (Licorice Root, Glycyrrhizae Radix et Rhizoma) (1 qian, honey-fried), Ganjiang (Dried Ginger, Zingiberis Rhizoma) (1 fen), Fuzi (Prepared Aconite Root, Aconiti Lateralis Radix Praeparata) (half piece, raw, skin and umbilical scar removed. Aconite weighing half liang is optimal; smaller ones have weak efficacy, larger ones have malignant properties and are not suitable for ancient prescriptions. This is not merely to prefer larger pieces, but to acknowledge the standards of the ancients).
Crush the above herbs to the size of hemp seeds. Add two zhan of water, decoct until one zhan remains, strain the juice, and take warm.
If heat accumulation is extreme, causing cold extremities, this formula is contraindicated; purgation should be used instead.
Fuling Banxia Tang (Poria and Pinellia Decoction): Treats all types of vomiting in Shanghan and miscellaneous diseases, or those accompanied by wheezing/cough, pain, epigastric fullness/stuffiness, and headache.
Fuling (Poria, Poria) (1 fen, peeled), Banxia (Pinellia Tuber, Pinelliae Rhizoma) (1 qian), Shengjiang (Fresh Ginger, Zingiberis Rhizoma Recens) (1 fen, juice extracted).
Crush the above herbs to the size of hemp seeds. Add one zhan of water, decoct until four parts remain, strain the juice, add ginger juice, and take warm at any time.
Another prescription adds Huangqin (Scutellaria Root, Scutellariae Radix) (1 fen, remove decay) and Hongpi (Red Peel, likely Citri Reticulatae Pericarpium Rubrum) (1 fen, remove pith) to treat wind-phlegm.
The Suwen states: Accumulation (Jiju), retained fluid (Liuyin), epigastric stuffiness/oppression (Pige), central fullness (Zhongman), damp accumulation (Shiji), cholera with vomiting and diarrhea (Huoluan), and hard masses with abdominal fullness (Xia) all stem from Taiyin damp-earth, which is the root of qi accumulation in the spleen and stomach. (Accumulation [Ji] does not disperse; Gathering [Ju] does not transform; Retention [Liu] does not move; Fluid [Yin] stagnates; Stuffiness [Pi] is blocked; Oppression [Ge] is obstructed; Central fullness [Zhongman] is damp accumulation; Cholera vomiting/diarrhea is retention/gathering; Zheng is a sign; Xia is false.) These diseases are governed by the laws of flourishing and declining yin-yang changes in the five zang and six fu organs. Excess causes harm; control brings balance; extremes reverse. For example, water dissipates when dry but does not dissolve when wet, becoming accumulated fluid. The human body's spirit and essence, along with Ying-Wei (Nutritive and Defensive), blood, qi, and body fluids, circulate freely. If the couli (pores/skin texture) are closed and dense, Pi (stuffiness) forms. If the intestines and stomach are severed, and transmission/transformation fails, stagnation occurs. Earth governs the physical form; abdominal fullness in the center is called Zhongman. Due to loss of regulation in transmission, severe cases result in cholera vomiting and diarrhea. Zheng refers to hardness in the abdomen, palpable upon pressure. Water is naturally soft and pliant; if it becomes hard like earth, it indicates excess causing harm and loss of control. Xia refers to hardness in the middle that gathers and scatters irregularly, so the disease has not yet reached a critical stage.
The classics state: Blood stasis leads to internal coagulation, forming Xia. Heat transferring from the Small Intestine to the Large Intestine forms Xia. Heat transferring from the Large Intestine to the Small Intestine involves two heats clashing, causing blood extravasation and forming Fu Xia (Hidden Xia). Blood涩 (astringency/stasis) impedes menstruation, leading to Xia. "Fu" is synonymous with "Xia", and "Xia" is synonymous with "Shan" (Hernia), due to transcription errors. The world transmits these as cold diseases, but Xia can also be hot. Or, yang qi is constrained,郁热 (stagnant heat) obstructs, causing hardness that does not resolve; the world mistakenly calls this Cold Xia. Or, there is hard stuffiness, abdominal fullness, and acute pain. (Cold contracts sinews, so acute pain indicates cold.) Extreme cold causes blood coagulation and stasis, which may paradoxically involve Earth transformation controls, resulting in hard stuffiness and abdominal fullness. Alternatively, heat constrained internally causes abdominal fullness, hardness, and unbearable pain; these may all be heat. Misjudged! Misjudged! Why not differentiate by pulse? All diseases have yin-yang and cold-heat aspects; they must be carefully analyzed.
The five zang and six fu organs have accumulations in all four seasons. The accumulation of the Heart is named Fuliang (Hidden Beam), located above the navel, as thick as an arm, extending up to the heart, horizontal under the heart, resembling a roof beam, hence Fuliang. The accumulation of the Liver is named Benqi (Rushing Qi), located under the left hypochondrium, covering like a cup, with head and feet. If unhealed for long, it causes malaria. The accumulation of the Spleen is named Piqi (Stuffiness Qi), in the epigastrium, covering large like a cup. If unhealed for long, it causes inability to move limbs, jaundice, and food failing to nourish muscles. The accumulation of the Lung is named Xiben (Breath Rushing), knotted under the right hypochondrium, covering large like a cup. If unhealed for long, it causes chills and fever, wheezing/cough, and发展为 lung abscess. The accumulation of the Kidney is named Ben (Rushing), in the lower abdomen, extending up to the epigastrium, moving like running, indefinite in location. If unhealed for long, it causes wheezing/reversal, bone萎 (atrophy/weakness), shortness of breath, and fatigue. These are the accumulations of the five zang. Studying this principle thoroughly reveals its complexities.
Transmission to the organ it overcomes (Ke) leads to death; transmission to the organ it does not overcome leads to recoverability. For example, Lung disease transmitting to Liver, Liver to Spleen, Spleen to Kidney, Kidney to Heart, Heart to Lung—all transmit to what they overcome. When the qi of the five zang is deficient and invaded by external/internal pathogens, it stays and stagnates, forming accumulations. The pulse is deep, fine, and faint.
