Mr. Fei, Right side: Ying deficiency with Liver hyperactivity. Liver Qi stagnation transforms into fire; Spleen deficiency generates dampness; dampness stagnates and transforms into heat. Damp-heat and stagnant fire flow into the Dai Mai (Girdle Vessel), which loses its restraining power, resulting in internal heat, red urine, soreness of the lower back, and leukorrhea; damp-heat presses downward on the Large Intestine, causing a sensation of bearing down at the anus. Stagnant fire should be cleared, but clearing fire must be assisted by nourishing Ying; accumulated dampness should be leached out, but leaching dampness must be combined with supporting the Earth element (Spleen).
Prescription: Bai Guishen (Radix Angelicae Sinensis) 2 qian, Chi Fuling (Poria with Red Core) 3 qian, Nong Duzhong (Cortex Eucommiae) 2 qian, Liu Yi San (Six-to-One Powder) [wrapped] 3 qian, Da Baishao (Radix Paeoniae Lactiflorae) 2 qian, Huai Shanyao (Rhizoma Dioscoreae) 3 qian, Wutougu (Cuttlefish Bone) 3 qian, Chao Tiao Qin (Flos Scutellariae, stir-fried) 1.5 qian, Hei Shanzhi (Fructus Gardeniae, charred) 1.5 qian, Huangbai Tan (Cortex Phellodendri, charred) 0.8 fen, Sheng Baizhu (Rhizoma Atractylodis Macrocephalae) 1.5 qian, Qiji Geng (Stem of Eleocharis) 1.5 qian.
Mr. Wu, Right side: Deficiency of the Three Yin meridians (Liver, Spleen, Kidney), with damp-heat pouring downward. Frequent leukorrhea, vaginal prolapse with a bearing-down sensation, loose stools, tenesmus. The strategy is to tonify Qi and raise clear Yang, nourish Yin and transform dampness.
Prescription: Sheng Huangqi (Radix Astragali) 3 qian, Huangbai Tan (Cortex Phellodendri, charred) 0.8 fen, Xiao Shengdi (Radix Rehmanniae) 3 qian, Chuan Shengma (Rhizoma Cimicifugae) 0.3 qian, Mi Zhi Zhike (Pericarpium Citri Reticulatae, honey-fried) 1 qian, Wutougu (Cuttlefish Bone) 3 qian, Fen Danpi (Cortex Moutan) 1 qian, Jing Huaimi (Flos Sophorae) [wrapped] 3 qian, Sheng Gancao (Radix Glycyrrhizae) 0.8 fen, Ku Jiegeng (Radix Platycodonis) 1 qian, Fu Zexie (Tuberm Alismatis) 1.5 qian, Weixi Wan (Weixi Pill) [wrapped] 3 qian.
Mr. Huang, Right side: Deficiency of Ying and Blood, with Hyperactive Liver Fire, carrying damp-heat to disturb the Dai Mai (Girdle Vessel). Leukorrhea with blood and white discharge, dizziness, soreness of the lower back. The treatment is to nourish the Liver, transform dampness, and bind the Dai Mai.
Prescription: Bai Guishen (Radix Angelicae Sinensis) 2 qian, Yun Fuling (Poria) 3 qian, Nong Duzhong (Cortex Eucommiae) 2 qian, Xian Ou (Rhizoma Nelumbinis, sliced) 2 liang, Sheng Yiren (Semen Coicis) 4 qian, Wutougu (Cuttlefish Bone) 3 qian, Sheng Baishao (Radix Paeoniae Lactiflorae) 2 qian, Nen Baiwei (Herba Cynanchi Atrati) 1.5 qian, Chuan Duanrou (Radix Dipsaci) 2 qian, Huangbai Tan (Cortex Phellodendri, charred) 0.8 fen, Fen Danpi (Cortex Moutan) 1.5 qian, Fu Zexie (Tuberm Alismatis) 1.5 qian, Sheng Baizhu (Rhizoma Atractylodis Macrocephalae) 3 qian, Zhenling Dan (Zhenling Elixir) [wrapped] 3 qian.
Follow-up visit: The leukorrhea with blood and white discharge has decreased. Menstruation comes early, indicating deficiency of Ying-Yin, excess Liver Fire, and disharmony of the Chong and Ren vessels. Again, the strategy is to nourish Blood and soften the Liver, thereby regulating the Extraordinary Vessels.
From the previous prescription, remove Baiwei, add Zhi Biejia (Carapax Trionycis, honey-fried) 3 qian.
