Pulse Diagnosis: At the beginning of pregnancy, the Cun (inch) pulse is faint and fine, with a rate of five beats per cycle (approx. 50-60 bpm), both hands show a balanced pulse, and prolonged pressing yields an indistinct sensation. By the third month of pregnancy, Yin pulses beat upon Yang positions; Qi declines while Blood flourishes, making the pulse appearance appropriate; Liver Qi becomes stagnant while Lung Qi weakens; the Heart pulse is slippery and surging; the Chi (cubit) pulse is slippery and slightly scattered, becoming stronger with prolonged pressing; alternatively, the Guan (bar) pulse may be large and slippery, with frequent intermittent pauses (Dai pulse); if the pulse is surging yet slow, the fetus is likely damaged. By the fourth month, fetal sex can be distinguished: right-sided pulses indicate a female, left-sided a male; pulses may be floating or deep, rapid, large, and solid; if both sides are vigorous, twins may be present; further examination of menstruation history helps confirm Yin-Yang balance. As long as the pulse is rapid but not scattered, pregnancy is confirmed by the fifth month; excessively tight or loose pulses indicate risks of edema or bleeding. In the sixth and seventh months, the fetus prefers a robust growth phase; a deep, slow, and choppy pulse warns of threatened miscarriage; a wiry pulse with chills and fever suggests the need to warm the uterus. By the eighth month, a wiry and solid pulse is normal, whereas a deep and thin pulse is unfavorable; a slightly tight pulse at the Shao Yin position indicates that in twins, one may be compromised; physical exertion or fright may cause fetal blood to become unstable; chest oppression and pain with cyanosis indicate fatal outcomes. A chaotic pulse at term is actually auspicious.
If menstruation has ceased for three months but the Chi pulse remains strong, it confirms pregnancy.
Yan Tai San (Fetal Verification Powder): Crush Chuanxiong Rhizome (Chuanxiong) into powder. Take one qian (approx. 3-5g) on an empty stomach, mixed with decoction of Artemisia argyi Leaves (Ai Ye). If slight movement is felt in the abdomen, pregnancy is confirmed. If no movement occurs after one day, it is amenorrhea, not pregnancy.
Ai Cu Tang (Mugwort Vinegar Decoction): Use good vinegar to extract juice from fresh Mugwort (Ai Ye). Drink half a cup. Severe abdominal pain indicates pregnancy; absence of pain indicates no pregnancy.
The principle of An Tai (calming the fetus) involves nourishing Blood, strengthening the Spleen, and clearing Heat.
For those with pre-existing Heat:
Angelica Sinensis Radix (Danggui, 2 qian), Ligusticum Chuanxiong Rhizoma (Chuanxiong, 1.5 qian), Rehmanniae Radix Preparata (Shengdi, 1 qian), Leonuri Herba (Yimucao, 1 qian), Atractylodis Macrocephalae Rhizoma (Baizhu, 2 qian), Scutellariae Baicalensis Radix (Tiaoqin/Huangqin, 1 qian), Amomi Fructus Rotundus (Sharen, 8 fen), Cyperi Rhizoma (Xiangfu, stir-fried with child's urine, 1 qian), Perillae Caulis (Sugeng, 1 qian), Coptidis Rhizoma (Huanglian, stir-fried, 8 fen), Glycyrrhizae Radix et Rhizoma (Gancao, 3 fen).
Decoct these ingredients as one dose with three slices of Zingiberis Recens Rhizoma (Shengjiang), and take warm.

Angelica Sinensis Radix (Danggui), Ligusticum Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishao), Scutellariae Baicalensis Radix (Tiaoqin/Huangqin) (each 1 liang), Atractylodis Macrocephalae Rhizoma (Baizhu, de-rhizome, 5 qian).
Grind into powder, bind with wine paste into pills the size of Sophora japonica seeds. Take 50 pills per dose with tea or water, on an empty stomach, three times daily. This formula nourishes Blood and clears Heat. It is suitable for thin individuals with Blood deficiency and Heat, restless fetus, or habitual miscarriage, to clear the root cause and prevent disaster.

Qianjin Bao Tai Wan (Thousand Gold Fetal Preservation Pills): For women who miscarry after two months of conception. Although Qi and Blood are deficient, the injury lies in the Chong and Ren vessels. The Chong vessel connects to the Stomach Meridian of Foot-Yangming, which supplies the fetus. At this stage, one must regulate diet, abstain from sexual desire, and avoid anger to prevent miscarriage. Taking this pill can preserve the fetus.

Fo Shou San (Buddha's Hand Powder): Treats pregnancy at five to seven months where physical impact injures the fetus, or fetal death in utero, with lochia discharge, unceasing pain, and jaw locking approaching collapse. Use this medicine to test: if the fetus is undamaged, pain stops and mother and child are safe; if the fetus is damaged, it will be expelled immediately.

Case 1: A pregnant woman had vaginal bleeding. After taking cooling-blood medicines, bleeding worsened, accompanied by poor appetite and fatigue. This was Spleen Qi deficiency failing to control blood. I used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment Qi Decoction) and she recovered. Later, due to anger, she experienced chills and fever with continued bleeding, indicating Liver Fire flaring up boiling the blood. I used Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) to stop bleeding, and Bu Zhong Yi Qi Tang to stabilize the fetus.
Case 2: A pregnant woman had vaginal bleeding, fever, thirst, poor appetite, and fatigue. This belonged to Spleen Qi deficiency overacted by Liver Fire. I used Si Jun Zi Tang (Four Gentlemen Decoction) plus Chai Hu (Bupleuri Radix) and Shan Zhi (Gardeniae Fructus); bleeding stopped. Upon recurrence due to anger, I used Liu Jun Zi Tang (Six Gentlemen Decoction) plus Chai Hu, Shan Zhi, and Sheng Ma (Cimicifugae Rhizoma), and she stabilized.
Case 3: A pregnant woman at six months presented with fatigue, reluctance to eat, sallow complexion, afternoon fever, and poor fetal growth due to overwork and excessive desire. This was insufficient Spleen Qi. I used Ba Zhen Tang (Eight Treasures Decoction) with doubled doses of Renshen (Ginseng), Baizhu (Atractylodes), and Fuling (Poria) for over thirty doses. Her Spleen and Stomach strengthened, and the fetus became stable and grew.
Case 4: A pregnant woman suffered miscarriage, followed by unconsciousness and frequent phlegm expectoration. She self-medicated with Blood-nourishing and Phlegm-resolving agents, leading to stupor, spontaneous sweating, convulsions, and copious phlegm. Others diagnosed Stroke Wind and wanted to use wind-dispelling and phlegm-resolving drugs. I stated this was caused by Spleen Qi deficiency and Cold. I prescribed Shi Quan Da Bu Tang (Comprehensive Great Tonifying Decoction) plus Pao Jiang (Prepared Ginger) for over twenty doses, and she recovered soon.
Case 5: A pregnant woman vomited blood due to anger, with distending pain in the hypochondrium and scanty painful urination. Anger caused Blood to accumulate above and exit with Fire. I used Xiao Chai Hu Tang (Minor Bupleurum Decoction) combined with Si Wu Tang (Four Substances Decoction) for four doses to stop bleeding; then regulated with Liu Jun Zi Tang (Six Gentlemen Decoction) and An Tai Yin (Calming Fetus Drink) for stabilization.
Case 6: A woman had amenorrhea for eight months with gradual abdominal enlargement, sallow or bluish complexion. Medicines for pregnancy symptoms failed. On diagnosis, I noted: bluish face, choppy pulse, alternating chills and fever—indicating Liver Meridian Blood disease. This was a syndrome of Spleen and Liver injury due to depression and anger, not pregnancy. Disbelieving, she continued using pregnancy verification powders without effect. I used Jia Wei Gui Pi Tang (Modified Restore the Spleen Decoction) and Xiao Yao San (Free and Easy Wanderer Powder) for over twenty doses each; symptoms improved slightly. Desiring quick results, she took Tong Jing Wan (Menstruation-Promoting Pills) once, resulting in bleeding, stupor, spontaneous sweating, aversion to cold, cold limbs, vomiting, and anorexia. I used Renshen (Ginseng) and Pao Jiang (Prepared Ginger) for two doses to gradually improve, then Shi Quan Da Bu Tang (Comprehensive Great Tonifying Decoction) for over fifty doses to stabilize her. (All above cases relate to tonification.)
Case 7: Madam Chong, wife of Minister Liu, suffered severe morning sickness during pregnancy: incessant vomiting, inability to eat, mental irritability, dizziness, and vertigo. Various physicians tried harmonizing Spleen and Stomach formulas like Er Chen Tang (Two-Chen Decoction), Huo Xiang Zheng Qi San (Agastache Regulating Qi Powder), and Bao Sheng Tang (Protect Life Decoction) without success. I diagnosed a slightly rapid Left pulse and rapid Qi Kou pulse, indicating Blood deficiency with excess Qi and Fire. Without nourishing Blood, Fire would not descend; without descending Fire, vomiting would not cease. I used Fu Ling Bu Xin Tang (Poria Nourishing Heart Decoction) plus Jiang Zhi (Ginger Juice), Chao Huang Lian (Stir-fried Coptis), and Zhu Ru (Bambusa Textilis Caulis in Mucilage) for two doses, resulting in complete recovery.
Case 8: A woman always miscarried at three months of pregnancy but refused medication. I advised her to cook soup with a four-to-five-year-old hen, add red millet and yellow millet, make porridge, and eat it. After a few times, the fetus stabilized, and she delivered a healthy boy at full term.
