痘不治法∶初出涌壮者不治,出如蚕种者不治;随出随没者不治;如蚊虫咬者不治;气血相失者不治;
倒出者不治;饮水如促鼻者不治。与肺气不能疏理也。

官桂救表;当贯脓而不贯脓者,倍参、 、当归,煎热,临服入人乳汁、好酒同参温服,此贯脓之巧法也;

How to diagnose smallpox in children? When the cheeks are red and the eyelids are swollen and red; accompanied by yawning, sneezing, and signs of fright or terror, with ears and fingers as cold as ice; if three days pass after symptoms appear without visible rash, the herbs for ascending and diffusing cannot be delayed; Baidu Gegen Tang (Decoction for Overcoming Poison and Kudzu Root) can be selected, which is most suitable for resolving heat and inducing sweating to release the exterior; cold-natured agents must be used with caution, as disturbing the Zang-Fu organs may lead to disaster and danger.
For children suffering from smallpox with deficiency-cold of the Zang-Fu organs, the following manifestations occur (cold-natured herbs are contraindicated): aversion to cold, alternating chills and fever, spontaneous sweating with aversion to wind, cold extremities, cyanotic or pale complexion, cyanotic eyes, greenish-white stool; clean body, lethargy and desire to sleep, clear and profuse urination and defecation, vomiting and diarrhea without thirst; drowsiness, cold breath from mouth and nose, inability to eat, undigested milk and food; abdominal distension, weak voice, cold calves, listless spirit; reluctance to speak, vomiting milk and passing clear loose stools, floating-thin-deficient pulse (indicating exterior deficiency), deep-thin-slow pulse (indicating interior deficiency).
For children suffering from smallpox with excess-heat of the Zang-Fu organs, the following manifestations occur (warm-natured herbs are contraindicated): red face, constipation, high fever, dry hair and skin; purple lips, dark yellow urine, extremely hot hands and feet, palpitations and delirious speech; yellow eyes, hot foul breath, vomiting and dysentery with thirst, manic crying; nasal congestion, runny clear nose, dark yellow stool, abdominal distension and anorexia; headache, generalized body pain, irritability with phlegm obstruction, chest and diaphragm fullness and stuffiness; choking cough, dry throat, cough with wheezing, bleeding from upper and lower orifices; floating-rapid-excess-large pulse (exterior syndrome), deep-excess-rapid pulse (interior syndrome).
Smallpox has ten stages: fever, initial eruption, complete eruption, rising and swelling, pus formation, full pus, drying up, scabbing, scab formation, and recovery of vitality.
Treatment during the initial fever stage:
Jiawei Baidu San (Modified Decoction for Overcoming Poison): Take this medicine immediately at the onset of fever for miraculous effects.
Herbs: Chaihu (Bupleuri Radix), Qianhu (Peucedani Radix), Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Fangfeng (Saposhnikoviae Radix), Jingjie (Schizonepetae Spica), Bohe (Menthae Haplocalycis Herba), Zhike (Aurantii Fructus Immaturus), Jiegeng (Platycodonis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Tianma (Gastrodiae Rhizoma), Digupi (Lycii Cortex).
This is an ancient formula, excluding Renshen (Ginseng Radix et Rhizoma) and Fuling (Poria), fearing that early tonification might aid fire. It is advisable to add Zicao (Arnebiae Radix), Chantui (Cicadae Periostracum), Ziye (Perillae Folium), Mahuang (Ephedrae Herba), Jiangcan (Bombyx Batryticatus), and Congbai gen (Allii Fistulosi Bulbus cum Radice) to resolve heat. If there is diarrhea, add Zhuling (Polyporus) and Zexie (Alismatis Rhizoma), remove Zicao, decoct in water and take hot; sweating is ideal.
Shengma Gegen Tang (Decoction of Cimicifugae Rhizoma and Puerariae Radix): At the beginning of fever, before distinguishing between measles, smallpox, cold damage, or food stagnation, use this formula to dissipate the pathogen to avoid errors.
Herbs: Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Radix), Baishao (Paeoniae Radix Alba) (each 3 qian), Gancao (Glycyrrhizae Radix et Rhizoma) (1 qian).
Crush one dose, add three slices of Shengjiang (Zingiberis Rhizoma Recens), decoct in water and take hot. In cold months, add Ziyehuangqin (Perillae Folium) 8 fen; for cold limbs, add Guizhi (Cinnamomi Ramulus) 1.5 qian; for lower back pain, knowing it is smallpox, add Guizhi 1.5 qian; if the epidemic qi is fierce and fever is severe, indicating abundant toxic qi, add Niubangzi (Arctii Fructus) 1.5 qian; taking only the previous Baidu San is particularly excellent.
Treatment during the initial eruption stage:
Shengong San (Divine Merit Powder): Treats excessive toxic qi during eruption, where blood is red and widespread without clear boundaries, resembling silkworm eggs, or for various hemorrhages, vomiting, and diarrhea within seven days; take this for detoxification.
Herbs: Huangqi (Astragali Radix), Renshen (Ginseng Radix et Rhizoma), Baishao (Paeoniae Radix Alba), Zicao (Arnebiae Radix), Shengdi (Rehmanniae Radix Preparata), Honghua (Carthami Flos), Niubangzi (Arctii Fructus) (equal parts), Qianhu (Peucedani Radix), Gancao (Glycyrrhizae Radix et Rhizoma) (halved).
Crush, decoct in water, take at any time. If heat is severe, add Huanglian (Coptidis Rhizoma) and Huangqin (Scutellariae Radix) each 1 qian; if not subsided, add Dahuang (Rhei Radix et Rhizoma) ground into the mixture. For convulsions, add one Chantui (Cicadae Periostracum) with wings and feet removed. If pustules are pale black, indicating cold invasion, add Guanzi Gui (Cinnamomi Cortex) 1 qian.
Huisui Jiu (Coriander Wine): At the initial eruption of smallpox, spray this wine around the bed walls and give it to the patient. Caretakers should also drink it to ward off filth and toxicity; apply pure rouge to the corners of the eyes to prevent toxin entering the eyes.
Take a bunch of Huisui (Coriandri Fructus), size of a copper coin, finely cut, mix well with hot wine and administer. Soon, the whole body feels unblocked. After one or two hours, dip paper in sesame oil and shine on areas without rash; dozens of pearl-like bright spots will emerge. If Huisui is unavailable, grinding Huisui Zi (Coriandri Fructus) works too.
Treatment when eruptions are complete:
Baoyuan Tang (Preserve Origin Decoction)
Herbs: Renshen (Ginseng Radix et Rhizoma) (2 qian), Huangqi (Astragali Radix) (young ones, 2 qian), Gancao (Glycyrrhizae Radix et Rhizoma) (1 qian).
Crush one dose, add one slice of Shengjiang (Zingiberis Rhizoma Recens), decoct in water and take.
If there are cold symptoms before the fourth day with dark color, use Baoyuan Tang plus Guanzi Gui (Cinnamomi Cortex). If after the fifth day there is interior cold, use Fuzi Lizhong Tang; if mild, only Baoyuan Tang plus Guanzi Gui is suitable.
Abdominal pain indicates excessive toxin; treat with Shengong San.
Persistent facial redness with indistinct boundaries: double the Qianhu in Shengong San.
Vomiting indicates excessive toxin rising with fire; treat with Shengong San.
Diarrhea indicates excessive fire rushing out; stop with Shengong San. Use Shengma (Cimicifugae Rhizoma) to lift and raise; do not use astringents. Only by resolving the toxin will it stop naturally.
Thirst: Use Honghua Zi Tang (Red Flower Seed Decoction) plus Niubangzi (Arctii Fructus); even if smoke rises from the mouth, it will resolve. For thirst, Ren Shen Mai Men Dong Tang (Ginseng and Ophiopogon Decoction) is also acceptable. Do not use Jujube Decoction.
Severe thirst: Take real yellow earth soaked in boiling water, cover the bowl, let sit briefly, then pour into a cup. Add a little sugar and drink; it stops immediately.
Unstopping sweating with cool body indicates blood overflowing with qi. Use Danggui (Angelicae Sinensis Radix) (5 qian), Huangqi (Astragali Radix) (3 qian), Suanzaoren (Ziziphi Spinosae Semen) (1 qian). Crush one dose, decoct in water and take warm; it stops immediately.
Phlegm: Use Baifuzi (Aconiti Kusnezoffii Radix) ground in water; effective. Do not use Erchen Tang (Two Aged Things Decoction), as it dries the Yangming channel, leaving solitary yang without yin, unable to transform.
Cough: Decoct Xingren (Pruni Armeniaci Semen Amarum) in water, grind Baifuzi (Aconiti Kusnezoffii Radix) and take.
Depressed crown within three days is not deficiency but extreme fire turning to yin descent, like branches drooping at noon. Use Jiwei Shengong San (Nine-Ingredient Divine Merit Powder) to reduce fire.
Pale and dull color on days 3-4 without moisture: Use Shengong San to invigorate blood and reduce fire, making it bright and full.
If treatment is missed and detoxification is unknown, on days 5-6, shine a lamp on it; if vital qi is not destroyed but toxin is excessive causing heat with red dry boundaries, treat with Shengong San. Observe changes and treat accordingly; perhaps one or two can be saved. Beyond this, divine merit cannot be applied.
Recognizing Smallpox: Those with qi respected and blood distinct survive. This means fire settles before eruption, qi follows its command, tips are sharp and white, roots are red and not scattered, like a pearl placed on rouge. White tips show rising qi respected; red roots show blood adhering securely, thus survival. Those with toxin invading yang positions die. This means erupting with fire, qi losing command, tips red-purple, boundaries indistinct, like blood or pig liver, extreme momentum without return, thus death. If covered by cinnabar and rouge, vital qi remains, treatable.
Incurable Smallpox: Erupting violently and strongly is incurable; erupting like silkworm eggs is incurable; appearing and disappearing immediately is incurable; resembling mosquito bites is incurable; loss of qi and blood harmony is incurable; inverted eruption is incurable; drinking water like obstructed nose breathing is incurable, due to lung qi failing to disperse.
Generally, before seven days is interior excess; do not administer warm-dry agents as they aid toxin. After eight days is interior deficiency; do not administer cold-cool agents as they destroy vital qi.
Treatment during the rising and swelling stage:
If rising fails when it should, use Chuanshanjia (Manis Squama) fried to pearls, ground 2 qian, taken with warm wine.
If blood weakness prevents rising with thin roots, use Baoyuan Tang plus Dingxiang (Caryophylli Flos) (3 grains), Rougui (Cinnamomi Cortex) (1 qian), Danggui (Angelicae Sinensis Radix) (2 qian), Chuanxiong (Ligustici Chuanxiong Rhizoma) (1 qian), decoct in water and take.
For non-swelling, gray-white, depressed crowns, indicating qi-blood deficiency and deficiency-cold, use Tuoli San (Support Interior Powder) plus Dingxiang (Caryophylli Flos).
For purple-red non-swelling, indicating excessive fire and blood heat, use Neituo San (Interior Support Powder) removing Rougui (Cinnamomi Cortex), adding Zicao (Arnebiae Radix) and Honghua (Carthami Flos); if heat is excessive, add Huangqin (Scutellariae Radix); if purple-black and sunken, mix this powder with Chuanshanjia (Manis Squama) fried to pearls, 5 fen, taken together.
Neituo San (Interior Support Powder): Treats qi-blood deficiency or wind-filth-toxin intrusion causing toxin sinking inward, lying dormant without erupting, or erupting unevenly. Use this to invigorate blood, harmonize qi, supplement deficiency, support interior toxin, ensuring complete eruption and easy healing.
Herbs: Huangqi (Astragali Radix) (honey-fried), Renshen (Ginseng Radix et Rhizoma), Danggui (Angelicae Sinensis Radix) (each 2 qian), Chuanxiong (Ligustici Chuanxiong Rhizoma), Fangfeng (Saposhnikoviae Radix), Jiegeng (Platycodonis Radix), Baizhi (Angelicae Dahuricae Radix), Houpo (Magnoliae Officinalis Cortex) (fried with ginger juice), Gancao (Glycyrrhizae Radix et Rhizoma) (each 1 qian), Muxiang (Aucklandiae Radix), Guanzi Gui (Cinnamomi Cortex) (each 3 fen).
For red-purple-black sinking due to heat-toxin, remove Gui (Cinnamon), add Zicao (Arnebiae Radix), Honghua (Carthami Flos), Huangqin (Scutellariae Radix); for pale-white/gray-black sinking due to deficiency-cold, add Dingxiang (Caryophylli Flos) to save interior, Guanzi Gui (Cinnamomi Cortex) to save exterior; when suppuration should occur but doesn't, double Renshen (Ginseng Radix et Rhizoma), Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), decoct hot, add human milk and good wine before taking warm, a clever method for suppuration; for diarrhea, add Dingxiang (Caryophylli Flos), Ganjiang (Zingiberis Rhizoma), Roukouren (Myristicae Semen).
Treatment during the pus formation stage:
If pus does not form by day eight, roots are deficient and blood scant. Initially, toxin carried blood recklessly, depleting blood. Now unable to form pus, urgently supplement with Gurong Tang (Deer Antler and Angelica Decoction).
Gurong Tang (Deer Antler and Angelica Decoction)
Herbs: Lurong (Cervi Cornu Degelatinatum) (prepared with butter, 1 liang), Danggui (Angelicae Sinensis Radix) (5 qian). Crush, decoct in wine and take.
For deficiency-cold smallpox, on days 8-9 with shiny white appearance like water blisters, depressed crown, white root, itching collapse, and chills.
Huiyang Jiu (Return Yang Wine)
Herbs: Lurong (Cervi Cornu Degelatinatum) (prepared with butter and baked), Dafuzi (Aconiti Lateralis Radix Preparata) (wrapped in flour, baked, navel and skin removed), tender Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix) (washed with wine).
Crush, decoct in good wine and take. If accompanied by phlegm-cough, add Niudan Nanting (Arisaematis Cum Bile Rhizoma).
Gray/black sinking with vomiting of white foam indicates exterior deficiency; use Muxiang San (Aucklandia Powder).
Severe vomiting: Muxiang San plus Bai Doukou (Amomi Fructus Rotundus). This is standard treatment. If detoxification was not done, use another method.
Chills, teeth grinding, itching collapse, and diarrhea indicate interior deficiency; use Yigong San (Extraordinary Merit Powder). Severe diarrhea: Yigong San plus Roudoukou (Myristicae Semen).
Dry retching indicates stomach deficiency with interior heat; use Yigong San (Extraordinary Merit Powder).
Muxiang San (Aucklandia Powder)
Herbs: Muxiang (Aucklandiae Radix) (ground), Qianhu (Peucedani Radix), Huangqi (Astragali Radix), Baifuling (Poria Cocos) (peeled), Baizhu (Atractylodis Macrocephalae Rhizoma), Houpo (Magnoliae Officinalis Cortex) (fried with ginger juice), Hezi (Terminaliae Chebulae Fructus) (baked, flesh taken), Chenpi (Citri Reticulatae Pericarpium) (each 1 qian), Rougui (Cinnamomi Cortex) (8 fen), Renshen (Ginseng Radix et Rhizoma), Dingxiang (Caryophylli Flos) (male, 5 grains).
