Stroke is divided into true stroke and pseudo-stroke. True stroke refers to sudden collapse at onset, caused by weak constitution and disharmony of Ying (Nutritive) and Wei (Defensive) Qi, or emotional disturbances such as anger, joy, worry, overthinking, sadness, fear, and shock, or injury due to excessive alcohol and sexual activity and physical labor. This leads to the depletion of Zhen Qi (True Qi), laxity of Cou Li (skin pores and interstitial spaces), allowing Wind pathogen to enter through deficiency, resulting in the disease. There are differences among invasion of Fu organs, Zang organs, Blood vessels, Qi deficiency, and Blood deficiency, thus treatment methods vary accordingly. Generally, invasion of Fu organs is treatable, while invasion of Zang organs is difficult to cure and may be incurable.
Symptoms such as open mouth with hand relaxation, closed eyes with urinary incontinence, foaming from the mouth, fixed gaze, snoring-like phlegm sounds in the throat, muscle wasting with bone and tendon pain, straightened hair, head shaking and upward tossing, face red like makeup, sweat dropping like pearls, and wheezing with phlegm indicate an incurable condition. If movement is restricted accompanied by tendon pain, it is due to lack of blood nourishing the tendons, termed "Tendon Withering," which is also incurable.
In cases of sudden collapse, unconsciousness, and locked jaw, this is Stroke Phlegm Fainting. (See subsequent formula.) First, use Tong Guan San (Passing-Through Powder) blown into the nose, followed by emesis; if still unconscious after vomiting, urgently moxa Baihui (GV20), Renzhong (GV26), Jiache (ST6), and Hegu (LI4); then administer Dao Tan Tang (Phlegm-Guiding Decoction) or She Sheng Yin (Life-Preserving Drink). Once slightly conscious, if Qi is not yet smooth and phlegm not fully cleared, only then should Huo Xiang Zheng Qi San (Agastache Powder to Rectify the Qi) be taken with added Nan Xing (Arisaema Cum Bile), Mu Xiang (Vladimiria Root), Fang Feng (Saposhnikovia Root), and Dang Gui (Angelica Sinensis) for one or two doses. Then, based on specific symptoms, select appropriate formulas for treatment, avoiding rigid adherence to a single method.
Tong Guan San: Treats Stroke Phlegm Fainting, coma, sudden collapse, unconsciousness approaching death.
Ingredients: Ya Zao (Gleditsia Sinensis Fruit, peeled and string removed, 1 liang), Sheng Ban Xia (Pinellia Rhizoma, raw), Li Lu (Veratrum Nigrum Rhizome) (each 5 qian), Xi Xin (Asari Radix et Rhizoma), Ku Shen (Sophorae Flavescentis Radix) (each 2 qian).

Secret Recipe: (Same indications as above.) Use Ba Dou (Croton Seed), shelled, pounded in paper to remove oil, discard beans. Make paper twists and insert into the nose, or add Ya Zao powder for better effect. Alternatively, burn the aforementioned paper twist and inhale the smoke.
Hui Sheng Dan: Transmitted by Wang Changdengting of Hailing, treats Stroke Phlegm Fainting and unconsciousness.
Ingredients: Cong Guan Li Lu (Li Lu inside Scallion Tube, 2 liang), boiled in one bucket of river water to make juice; Qing Meng Shi (Lapis Glaucophanum, 2 liang). Heat until red hot, throw into the juice. Repeat several times, filter cleanly. Take juice from ten male pig gallbladders, stir into the previous juice, then cook into paste using double-boiling method. When warm, add Pian Nao (Borneolum Syntheticum, 1.5 qian), store in porcelain jar sealed with yellow wax. Each dose: one pill size of soybean, dissolved in fresh water. Blow into nostrils (left for males, right for females), phlegm will naturally vomit. If jaw is tight and cannot vomit, pry mouth open to release phlegm, then administer other medicines.

Another method: Dan Fan (Chalcanthite, 1 fen), ground, mixed with warm yellow wine, taken until phlegm is vomited. Another method: Chen Sha (Cinnabar) and Bai Fan (Alumen) in equal parts, filled into pig gallbladder during Dog Days, dried in ventilated place.
