There are diseases that clear physicians can treat, folk practitioners can treat, but great physicians cannot treat; these are wind-bi syndromes. "Bi" means obstruction, referring to pain caused by the blockage of meridians and collaterals due to combined cold-dampness pathogens. The *Neijing* (Inner Canon) therefore distinguishes between wind predominance, cold predominance, and dampness predominance, and mentions walking bi, painful bi, and fixed bi. Upon examining the pulse, if it is floating, tight, and wiry, the pathology primarily归结s to wind pathogen, originating in the Liver Meridian and affecting the limbs. If the pathogen strikes the bones, extension is possible but flexion is not; if it strikes the tendons, flexion is possible but extension is not; if it strikes the blood, circulation becomes stagnant and sluggish. The treatment method involves: using Qianghuo (Notopterygii Rhizoma et Radix) and Fangfeng (Saposhnikoviae Radix) to disperse wind; Zisu (Perillae Folium) and Qingpi (Citri Reticulatae Pericarpium Viride) to move stagnation; Wujiapi (Acanthopanacis Cortex) and Huangbai (Phellodendri Cortex) to strengthen bones; Yimi (Coicis Semen) and Mugua (Chaenomelis Fructus) to relax tendons; Cangzhu (Atractylodis Rhizoma) and Fangji (Stephaniae Tetrandrae Radix) to dry dampness; Songjie (Pinus Massoniana Node) and Qiagen (Solanum Melongena Root) to scatter cold; Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma) to tonify qi; Shengdi (Rehmanniae Radix) and Danggui (Angelicae Sinensis Radix) to invigorate blood. Complex symptoms require complex prescriptions (Empirical Formula: Qianghuo, Fangfeng, Shigao (Gypsum Fibrosum), Ce Baiye (Platycladi Cacumen), Huangsongjie, Yimi, Mugua, Danggui, Zhigancao (Glycyrrhizae Radix et Rhizoma Preparata), Shengdihuang). If stagnation transforms into heat, regardless of other factors, one must vigorously clear heat; if closure accumulates cold with a slow or absent pulse, one must re-warm the meridians. The so-called "clear physician" is one expelled from the official registry, annotating alchemical texts, integrating the hearts of Confucianism, Buddhism, and Daoism into one, unifying the principles of Heaven, Earth, and Humanity, investigating the Dao in famous mountains, facing the wall for nine years, traveling ten thousand li. Regarding this syndrome, they externally illuminate the excessive evils of the Five Movements and Six Qi, and internally penetrate the hyperactivity and harm of the Zang-Fu organs. Using wind-dispersing herbs as sovereign agents, some have been cured using several jin; using Dahuang (Rhei Radix et Rhizoma) to purge heat, some have been cured using several jin; using Fuzi (Aconiti Lateralis Radix Praeparata) to warm meridians, some have been cured using several jin. Great physicians gape in astonishment, while folk practitioners are captivated. How dare folk practitioners contend with clear physicians? This empirical formula has been used by one person for a lifetime, by two generations, by three generations; it is a rare treasure, not acquired overnight. Gathering herbs in deep mountain tiger dens ("If you do not enter the tiger's den, how can you get the tiger's cub?" - Ban Chao, *Book of Han*), in the Cansong regions ("The road of Cansong is rugged and hard to travel" - Li Bai), sparing no effort in climbing; teaching children in quiet rooms ("Suddenly ink spills on the desk, smearing books like an old crow" - Lu Tong), pondering over worm-trail calligraphy ("Walking like spring worms, writing like autumn snakes" - Emperor Taizong of Tang on Xiao Zi Yun), investing much deliberation. Buying rice and salt is indeed a family heirloom; "pillowing on the stream and rinsing stones" ("Pillowing on the stream to wash the ears, rinsing stones to sharpen the teeth" - Sun Chu, Jin Dynasty), hoping to become a treasure waiting to be hired. Imagine him whispering: "My grandfather did this and it worked repeatedly; my father did this and it worked repeatedly; I do this and it works repeatedly." This volume of knowledge is not gained from theoretical understanding, nor from pharmacological properties, but from empirical experience. Thus, one knows that seedlings growing in rocky valleys are not without cause; all such plants as Yunu (resembling kudzu, creeping with nodes, called Dragon Tail or Tiger Kudzu in Jiangdong, with fine leaves and red stems), Kuigu (hook-shaped, also known as King Melon, fruit like melon, red and bitter), Jitou (chicken head) and Yajiao (duck foot) (mentioned in *Records of Buddhist Monasteries* in Luoyang) are nothing but agents to dispel wind, dry dampness, and scatter cold. Obtained by skilled hands, they happen to match this syndrome perfectly and align with the theories of clear physicians. Hence, great physicians are surprised by folk practitioners, and clear physicians respect them. As for the "great physicians" praised by the world, I know them now. It is not that their medicine is grand, but that their tonification is grand. Why is tonification grand? Because the dosage of herbs is large, making the physician appear grand. When going out, they wear light clothes, ride fat horses, whip along the main roads, appearing elegant; when diagnosing pulses, they concentrate, close their eyes, sit motionless all day, appearing diligent; when prescribing, they bite pens, dip brushes, ponder for half an hour, appearing focused. Yet, reading their prescriptions, they use Huangqi (Astragali Radix), Baizhu, Fuzi, Ganjiang (Zingiberis Rhizoma). Who knows that heat becomes fiercer with tonification, and cold-dampness becomes more congealed? Tangled and prolonged, leading to untreatable conditions, is this not lamentable? Peeking into their depths, this is due to their habitual adherence. Aggressively imposing, they master shortcuts in medicine; proudly showing off, they open strange prescriptions at sea. They never dreamt of what Spleen-Stomach is, or what Mingmen (Gate of Life) is. Opening their mouths, they either say Spleen-Stomach earth is failing, or Mingmen fire is declining. Of thousands of herbal medicines, they summarize less than ten; of thousands of ancient formulas, they calculate only two commonly used ones. Regardless of internal or external injury, they generally apply tonification; regardless of yin or yang syndromes, they always apply warming. *Lingshu* and *Suwen* are dismissed with a stroke; *Tangye* (written by Yi Yin) and *Nanjing* are said to be too difficult to learn in a century. Books from Han, Tang, Song, and Yuan dynasties, with their many elaborations; theories of Zhang, Zhu, Liu, and Li, are merely rhetorical flourishes. Medical books contain ten thousand words, yet only three are remembered; human hearts have seven apertures, yet none are opened. Like summer insects talking about ice (knowing only cold, not heat); like spring frogs sitting in wells (knowing only void/ruins, not solid/stones). Alas, heroes and generals suffer unjustly; talented scholars and beauties die prematurely. At midnight, when roosters crow, there is no thought of turning back; throughout life, all tracks are mere tongue-tying prescriptions (unable to eat or speak). Great physicians avoid clear physicians; folk practitioners feel ashamed to associate with great physicians. Alas! Clear physicians are not born every generation, folk practitioners dare not use them, and the toxic legacy of great physicians is widespread!
