Both smallpox (Xiaozhen) and measles (Mazhen) are caused by fetal toxin (Tai Du), which is essentially fire pathogen (Huo Xie). Smallpox belongs to the Shaoyang Ministerial Fire (Shaoyang Xiang Huo), where Yang energy is typically abundant, resulting in lesions that are large and swollen. Measles belongs to the Shaoyin Sovereign Fire (Shaoyin Jun Huo), where Yin energy is typically deficient, resulting in lesions that are small and densely distributed. When Heart Fire (Xin Huo) is excessive, it affects the Lung; therefore, treating measles should focus on the Lung. Coughing indicates fire flaring up in the Lung; clear nasal discharge occurs because fire scorches Metal (Lung), causing fluids to flow freely; tearing from the eyes signifies Lung heat transferring to the Liver; scratching the eyebrows, eyes, nose, and face are all signs of Lung heat.
Spring warmth, summer heat, autumn dryness, and winter cold are the dominant Qi of the four seasons. If winter should be cold but turns warm instead, Yang Qi leaks out violently early, and the Fire command arrives prematurely. Humans contracting this Qi will inevitably develop sores or rashes in the following spring. Those who have not yet manifested smallpox or measles will definitely be triggered by this infection. Although termed "fetal toxin," none arise without the influence of epidemic pestilence Qi (Tian Xing Li Qi); thus, infections spread simultaneously with similar symptoms. Once measles eruption begins, one should administer Tiandai Xuanhua Wan (Heavenly Substitution Decoction for Dispersion) to preemptively resolve the toxin, ensuring the toxin clears without becoming severe.
Revised Tiandai Xuanhua Wan (based on Han Feixia's Wuwen Dan, made into pills using decoctions of Cyperi Rhizoma, Perillae Folium, and Rheum Palmatum, coated with Cinnabar and Realgar, and overlaid with gold foil).
Indications: Preemptively resolves epidemic pestilence and contagious diseases with similar symptoms; treats toxic fire in smallpox and measles.
Composition: Raw Glycyrrhizae Radix et Rhizoma (Jun herb in years of Jia/Ji Earth), Scutellariae Radix (Jun herb in years of Yi/Geng Metal), Phellodendri Cortex (Jun herb in years of Bing/Xin Water), Gardeniae Fructus (Jun herb in years of Ding/Ren Wood), Coptidis Rhizoma (Jun herb in years of Wu/Gui Fire), Forsythiae Fructus (Zuo assistant), Sophorae Tonkinensis Radix (Zuo assistant), Arctii Fructus (Zuo assistant).
The first five herbs serve as the Jun (Monarch) herb based on the elemental correspondence of the year; the remaining four serve as Chen (Minister) herbs. The Jun herb dosage is doubled; the Chen herb dosage is halved; the Zuo herb dosage is half again of the Chen herb. Processed on the Winter Solstice day into powder, mixed into a paste using decoction of Cimicifugae Rhizoma boiled in snow water collected in the twelfth lunar month, formed into pills the size of a longan seed, coated with processed Cinnabar. Take one pill per dose, washed down with decoction of Bambusae Folium.
Early fever in measles resembles Cold Damage (Shanghan), but measles presents with red cheeks, cough, sneezing, clear nasal discharge, tearing, yawning, drowsiness, vomiting/diarrhea, or scratching of the face and eyes. Shengma Gegen Tang (Cimicifugae and Puerariae Decoction) is indicated. Do not treat as Cold Damage and do not recklessly use sweating or purging methods. Sweating increases heat, leading to epistaxis, hemoptysis, mouth ulcers, sore throat, red painful eyes, irritability, and urinary/fecal retention. Purging empties the interior, leading to incontinence or dysentery. The Classics state: "First observe the seasonal Qi, do not violate the Harmony of Heaven." This refers to such cases.
It is well known that measles prefers coolness while smallpox prefers warmth. However, during the initial eruption of measles, mild warmth facilitates emergence. Thus, at the onset, if the eruption is thorough, the toxin resolves. For smallpox, the toxin resolves only after progressing through budding, flowering, and fruiting stages; however, excessive warmth during the fruiting stage causes ulceration and non-healing, hence post-smallpox care also requires coolness. Therefore, treatment for both must avoid extreme heat or cold; appropriate warmth and coolness allow Yin and Yang to harmonize naturally.
The primary concern with measles is whether the eruption can emerge. If fully erupted, the toxin resolves. During the febrile phase, adjust medication according to seasonal temperature: for severe cold, use Guizhi Gegen Tang; for intense heat, use Shengma Gegen Tang combined with Renshen Baihu Tang; for moderate weather, use Jingfang Baidu San; for epidemic conditions, use Renshen Baidu San. Externally, apply Hushu Jiu (Coriander Wine) by rubbing the body with ramie cloth dipped in wine to hasten eruption. If eruption fails despite treatment, accompanied by abdominal distension/pain, wheezing, coma, or delirium, it is a fatal sign.
Guizhi Gegen Tang: Treats difficult eruption due to severe cold.
Ingredients: Cinnamomi Ramulus, Puerariae Radix (Powdered), Paeoniae Radix Rubra, Cimicifugae Rhizoma, Saposhnikoviae Radix, Honey-fried Glycyrrhizae Radix.
Add three slices of Zingiberis Rhizoma Recens and one qian of Fermented Soybean. Decoct in water and take orally.
Shengma Gegen Tang combined with Renshen Baihu Tang: Treats toxin blocked by heat during hot summer months.
Ingredients: Cimicifugae Rhizoma, Puerariae Radix (Powdered), Paeoniae Radix Alba, Honey-fried Glycyrrhizae Radix, Anemarrhenae Rhizoma, Gypsum Fibrosum (Calcined), Selected Ginseng.
Add a pinch of Oryza Sativa. Decoct in water and take orally.
Jingfang Baidu San: Treats moderate weather conditions.
Ingredients: Selected Ginseng, Bupleuri Radix, Ligustici Chuanxiong Rhizoma, Platycodonis Radix, Schizonepetae Spica, Poria, Citri Reticulatae Pericarpium (Aged), Peucedani Radix, Notopterygii Rhizoma et Radix, Angelicae Dahuricae Radix, Saposhnikoviae Radix, Honey-fried Glycyrrhizae Radix.
Add five leaves of Menthae Haplocalycis Herba. Decoct in water, take hot.
Renshen Baidu San: Treats measles during epidemic outbreaks.
Ingredients: Official Selected Ginseng, Notopterygii Rhizoma et Radix, Angelicae Dahuricae Radix, Peucedani Radix, Bupleuri Radix, Ligustici Chuanxiong Rhizoma, Poria, Citri Reticulatae Pericarpium (Aged), Platycodonis Radix, Honey-fried Glycyrrhizae Radix.
Add three slices of Zingiberis Rhizoma Recens. Decoct in water and take orally.
Hushu Jiu: Treats failure of measles eruption.
Ingredients: Four liang of Coriandri Fructus, chopped. Boil two cups of good wine in a pot, add coriander, cover tightly to prevent vapor loss (do not boil further). Dip ramie cloth in the wine and rub the entire body to facilitate eruption.
If fever persists for six to seven days without eruption, indicating thick skin and closed pores, or external wind-cold invasion, or latent toxin manifesting as vomiting/diarrhea, urgently use exterior-releasing formulas. Use Mahuang Tang removing Pruni Semen, adding Mylabris and Cimicifugae Rhizoma. Externally apply Hushu Jiu. If constipation persists, indicating severe internal toxin retention, use Liangge San adding Arctii Fructus. Failure to erupt thereafter is fatal.
Mahuang Tang: Treats measles failing to erupt after 6-7 days due to wind-cold obstruction.
Ingredients: Ephedrae Herba (Cleaned), Gypsum Fibrosum (Calcined), Mylabris, Cimicifugae Rhizoma, Honey-fried Glycyrrhizae Radix.
Add three cun of Allii Fistulosi Bulbus. Decoct in water and take orally.
Liangge San: Treats deep-seated measles toxin with interior blockage preventing eruption.
Ingredients: Rheum Palmatum, Natrii Sulfas (White), Forsythiae Fructus, Gardeniae Fructus (Charred), Menthae Haplocalycis Herba, Phragmitis Rhizoma, Glycyrrhizae Radix et Rhizoma (Tip).
Decoct, strain, and mix with three spoons of raw honey before taking.
During early measles fever, persistent coughing, rapid breathing, facial/eyelid edema, and restlessness indicate internal steaming of fire-toxin scorching the lungs. Ganjie Tang (Licorice and Platycodon Decoction) added with Gypsum Fibrosum, Anemarrhenae Rhizoma, and Arctii Fructus is indicated.
Ganjie Tang added with Gypsum Fibrosum, Anemarrhenae Rhizoma, and Arctii Fructus: Treats stomach fire flaming into the Lung, cough, facial edema, and failed eruption.
Ingredients: Raw Glycyrrhizae Radix et Rhizoma, Platycodonis Radix, Gypsum Fibrosum (Calcined), Anemarrhenae Rhizoma, Arctii Fructus.
Add five fresh leaves of Menthae Haplocalycis Herba. Decoct in water and take orally.
Spontaneous sweating or epistaxis during measles fever need not be stopped; these are mechanisms of toxin dispersal. However, excessive sweating requires Qingre (Clearing Heat) with Renshen Baihu Tang combined with Huanglian Jiedu Tang. Severe epistaxis requires cooling with Yuannshen Dihuang Tang.
Renshen Baihu Tang combined with Huanglian Jiedu Tang: Treats excessive sweating weakening defensive Qi.
Ingredients: Official Selected Ginseng, Anemarrhenae Rhizoma, Gypsum Fibrosum (Calcined), Raw Glycyrrhizae Radix et Rhizoma, Coptidis Rhizoma (Ya Lian), Phellodendri Cortex, Scutellariae Radix, Gardeniae Fructus (Charred).
Add a pinch of Oryza Sativa. Decoct in water, take hot.
Yuannshen Dihuang Tang: Treats severe epistaxis to protect Yin.
Ingredients: Scrophulariae Radix, Rehmanniae Radix Preparata, Moutan Cortex, Gardeniae Fructus (Charred), Cimicifugae Rhizoma, Paeoniae Radix Alba, Lygodii Spora, Raw Glycyrrhizae Radix et Rhizoma, Imperatae Rhizoma (fresh bundle, heartless stalk removed) as guide. Decoct in water, take hot.
Measles fever with vomiting/diarrhea is purely a heat syndrome, not cold. Toxin in the Upper Jiao causes vomiting; Lower Jiao causes diarrhea; Middle Jiao causes both. For pure diarrhea, use Huangqi Tang; for vomiting with diarrhea, Huangqi Tang added with Pinelliae Rhizoma; for tenesmus, Huanglian Jiedu Tang combined with Tianshui San.
Huangqi Tang: Treats measles fever with spontaneous diarrhea.
Ingredients: Scutellariae Radix (Withered), Paeoniae Radix Alba, Honey-fried Glycyrrhizae Radix.
Add one large red date. Decoct in water, take hot.
Huangqi Tang added with Pinelliae Rhizoma: Treats measles fever with diarrhea and vomiting.
Same as above plus Pinelliae Rhizoma and Zingiberis Rhizoma Recens.
Huanglian Jiedu Tang combined with Tianshui San: Treats measles diarrhea with tenesmus.
Ingredients: Coptidis Rhizoma (Ya Lian), Phellodendri Cortex, Scutellariae Radix (Withered), Gardeniae Fructus (Charred), Talcum, Honey-fried Glycyrrhizae Radix.
Concentrated decoction, take boiling hot on an empty stomach.
Sore throat in measles/smallpox is common due to fire-toxin steaming. Do not confuse with Hou Bi (sore throat/pharyngitis) requiring needle drainage. Measles sore throat is dry/painful; treat with Ganjie Tang or Shi Niu Zi Tang (Arctii Fructus Decoction) taken slowly.
Ganjie Tang: Treats measles sore throat and difficulty swallowing.
Ingredients: Raw Glycyrrhizae Radix et Rhizoma (Jun), Platycodonis Radix (Chen), Arctii Fructus (Shi).
Add ten stems of Junci Medulla. Decoct in water and take orally.
Shi Niu Zi Tang: Same indications, for slightly more severe cases.
Ingredients: Stir-fried Arctii Fructus, Cimicifugae Rhizoma, Belamcandae Rhizoma (Fresh), Raw Glycyrrhizae Radix et Rhizoma.
Add Junci Medella as guide. Decoct in water, take hot.
Note: Measles sore throat originates from Lung/Stomach, not Shaoyin/Shaoyang Hou Bi. Avoid cold/astringent herbs and blowing powders containing Borneolum Syntheticum or Calculus Bovis. In severe ulcerative cases, use Yuxiao Wuyou San or Sishen San.
Yuxiao Wuyou San: Treats strangling wind, swollen throat, voice loss, phlegm obstruction, mouth ulcers, alcoholism, infant milk accumulation, bone fragments stuck, tongue swelling, etc.
Ingredients: Borax, Calcined Hydrotalcite, Natrii Sulfas (Basal), Indigo Naturalis, Menthae Haplocalycis Herba, Lygodii Spora, Coptidis Rhizoma, Millettiae Speciosae Radix, Scrophulariae Radix, Poria, Talcum, Schizonepetae Spica, Sophorae Tonkinensis Radix, Amomi Fructus Rotundus (with shell), Raw Glycyrrhizae Radix et Rhizoma.
Grind to fine powder. Take half qian, sprinkle on tongue, swallow with clear water. Eliminates pathogens, dispels evil, prevents plague, relieves thirst.
Sishen San: Treats heat-toxin swellings and epidemics.
Ingredients: Rheum Palmatum, Hydrotalcite, Stir-fried Arctii Fructus (1 liang each), Natrii Sulfas (5 qian).
Grind to powder. For severe swelling, mix with fresh water and apply. For throat blockage preventing swallowing, make pills with raw honey and suck slowly.
Note: For scarlet fever (Lanhou Dansha), add Borax and Indigo Naturalis (5 qian each) for better effect.
Thirst and preference for drinking in measles indicate pure fire-toxin scorching Lung and Stomach with hyperactive Heart Fire. Early fever/thirst: Shengma Gegen Tang added with Trichosanthis Radix and Ophiopogonis Radix. Severe thirst: Renshen Baihu Tang combined with Huanglian Jiedu Tang.
Smallpox eruptions are preferred in 3-4 batches (even distribution); measles eruptions are preferred in one surge (complete eruption). Thorough eruption lightens the condition.
