[Commentary by Shao Xinpu]
Warming and tonifying Yang Qi (Yang Energy). Nourishing Yin to transform Ying (Nutritive) Qi. Searching out pathogens entering the collaterals. Using moving drugs to intercept [malaria]. Harmonizing with both acrid and sour flavors. Simultaneously tonifying Ying and Wei (Defensive) Qi. And methods such as nourishing Stomach Yin after Yang-type malaria (malaria presenting with heat signs), and regulating Spleen Yang after Yin-type malaria (malaria presenting with cold signs). These methods are already fully prepared. Collecting various schools of thought, integrating them without rigidity. As the saying goes, using medicine is like using soldiers; Mr. Ye deserves the reputation of a skilled physician. (Commentary by Shao Xinpu)
Xu Lingtai's Commentary: This is merely talk of copying others blindly.
Another Commentary: The ancient sages believed that for every disease, there must be a principal formula. For example, Xiao Chai Hu Tang (Minor Bupleurum Decoction) is the principal formula for malaria. Although the manifestations of malaria differ, one generally adds or subtracts from this formula. If there are other concurrent symptoms, it is also acceptable not to use the original formula. There is indeed logic in cases where malaria can be cured without using Chai Hu (Bupleuri Radix). However, to claim that Chai Hu must absolutely never be used for malaria is to spread erroneous doctrines. I previously heard that this old master prohibited the use of Chai Hu in treating malaria. Those who believe rumors passed down as secret methods have mutually warned against its use. I believe this is the fabrication of ignorant people; this old master would certainly not go to such lengths. Now, reading these medical cases, none of the formulas use Chai Hu, so I know this statement is true. Then this old master's deviation from orthodox teachings is truly surprising. Chai Hu Tang is the principal formula for the Shaoyang (Lesser Yang) channel. For symptoms of alternating chills and fever, this formula is indispensable. Zhang Zhongjing used Chai Hu most frequently. The Shanghan Lun (Treatise on Cold Damage) states: "Regarding the Chai Hu patterns of cold damage, if even one symptom is present, it suffices; it is not necessary for all to be present." He revered Chai Hu to such an extent. Yet this old master deliberately opposes the sage, uniquely avoiding Chai Hu. It would be like prohibiting Guizhi (Cinnamomi Ramulus) for Taiyang (Greater Yang) patterns, or prohibiting Gegen (Puerariae Lobatae Radix) for Yangming (Yang Brightness) patterns. This must be the words of an ignorant fool; how could an experienced famous doctor make such remarks? Truly a strange event in the world.
Patient Zhou: Due to damp-heat prevalent in the late summer and dietary irregularity. As stated, excessive dampness leads to the five types of diarrhea. First, use Weiling Tang (Gastrointestinal Regulation Decoction) to separate and drain Yin and Yang. (Etiology: Summerheat, Dampness, Heat) Remove Gancao (Licorice Root) from Weiling Tang.
Patient Wen: In late summer, excess dampness causes diarrhea, with borborygmus and scanty urination. The Yang of the Fu organs fails to govern the separation of clear and turbid. First, guide out dampness and harmonize the Middle Jiao. Use Weiling Tang.
Again: In previous years, injury to the Yin aspect affected the Yang position, often resulting in abdominal fullness and loose stools. From late summer into autumn, dysentery often occurred. This is due to accumulated weakness in the Qi aspect of the Middle Jiao, making it easy for the Qi of water and grain to gather into dampness. Or, foul pathogens enter via the mouth and nose, causing disharmony between the Spleen and Stomach. Therefore, care in regulation during summer and autumn is most important. Prescribed formulas for summer and autumn application for reference. When warm weather steams, Southern regions are most prone to various ailments such as summerheat, sha qi (toxic heat),痞胀 (stagnant distension). Before illness strikes, if there is suspicion or worry, immediately drink aromatic agents that rectify Qi, preventing pathogen entry is the primary principle.
Huoxiang Geng (Aucklandiae Caulis) Bai Kou Ren (Amomi Fructus Rotundus) Juhong (Citri Reticulatae Pericarpium Rubrum) Jiegeng (Platycodonis Radix) Xingren (Armeniacae Semen Amarum) Yujin (Curcumae Longae Rhizoma) Jiangxiang (Dalbergiae Odoriferae Lignum) Houpo (Magnoliae Officinalis Cortex)
After the Summer Solstice, heat prevails and dampness steams, injuring Qi and causing spiritual fatigue. Use Dongyuan Yiqi Tang (Dongyuan's Qi-Supplementing Decoction). If thirst occurs at the anus after sweat subsides, combine with Shengmai San (Generate Pulse Powder) to astringe fluids.
Patient Mou: Autumnal summerheat and foul turbidity are inhaled. Symptoms include alternating chills and fever resembling malaria, coughing with phlegm above, and profuse diarrhea below. The Three Jiao spreads the pathogen, urine is short and red. Propose aromatic methods to repel foulness, separating and draining dampness.
Huoxiang (Agastaches Herba) Houpo (Magnoliae Officinalis Cortex) Guangpi (Citri Reticulatae Pericarpium) Fuling (Poria) Gancao (Glycyrrhizae Radix et Rhizoma) Zhuling (Polyporus)
Zexie (Alismatis Rhizoma) Mugua (Chaenomelis Fructus) Huashi (Talcum) Tanxiang Zhi (Santalum Album Juice)
Again: Medication took slightly longer to act, confirming that the mentioned foul turbidity was not imaginary. The penile cyst is due to severe damp-heat sinking downward into the lower Jiao, differing from descriptions of genital diseases in formularies. Propose methods of Liu Hejian.
Houpo (Magnoliae Officinalis Cortex) Xingren (Armeniacae Semen Amarum) Huashi (Talcum) Hanshuishi (Calcitum) Shigao (Gypsum Fibrosum) Zhuling (Polyporus) Zexie (Alismatis Rhizoma) Siguye (Fici Folium)
Patient Mou: Chronic Yin malaria has long injured the body, leading to consumptive disease. Cannot lie face down, spine is stiff, pulse sinks and weak, dorsum of foot is edematous. This is dysfunction of the Du Mai (Governing Vessel), gradually leading to kyphosis and disability. Recently, summer-dampness invaded internally causing diarrhea. First, separate and drain dampness and harmonize the Middle Jiao.
Houpo (Magnoliae Officinalis Cortex) Huoxiang (Agastaches Herba) Guangpi (Citri Reticulatae Pericarpium) Fuling (Poria) Zexie (Alismatis Rhizoma) Mugua (Chaenomelis Fructus) Chao Bian Dou (Stir-fried Dolichos Lablab Seed) Chao Zha Rou (Stir-fried Hawthorn Meat) Chao Sharen (Stir-fried Amomi Fructus)
Patient Cai (21): Shortness of breath requiring frequent pauses indicates deficiency. Recently, abdominal discomfort and diarrhea due to summerheat injury. First, clear summerheat and harmonize the Spleen to prevent progression to dysentery.
Houpo (Magnoliae Officinalis Cortex) Guangpi (Citri Reticulatae Pericarpium) Zhi Cao (Prepared Licorice) Fuling (Poria) Zexie (Alismatis Rhizoma) Chao Bian Dou (Stir-fried Dolichos Lablab Seed) Maiya (Hordei Fructus Germinatus) Mugua (Chaenomelis Fructus) Chao Zha Rou (Stir-fried Hawthorn Meat) Sharen (Amomi Fructus)
Again: Xiangsha Yigong San (Aromatic-Amomum Yi-Gong Powder).
Patient Ye (57): Usually overworked and fatigued, fire from the five zang emotions ignites easily. Above, nasal passages are blocked; below, hemorrhoids and intestinal bleeding occur. Spring and winter warm pathogens represent the outward expansion of Yang Qi, allowing pathogens to hide inside when deficiency exists. The hidden pathogens cannot be resolved simply by dispersing acute exterior conditions. Moreover, the constitution involves internal injury from fatigue. The illness has lasted ninety days. The dorsum of the foot swells daily, bowel movements occur five to six times daily, stool is sticky, yellow-red-purple and stagnant, urination is difficult, only passing slightly upon defecation. The kidney gate pivot mechanism is abolished, and the two Intestines (Fu organs) fail in their duties. Water and grain ingested are not managed by the Spleen and Stomach, brewing dampness that sinks downward, transforming into putrefied forms. Just as Earth Qi peaks entering summer, when the Spleen and Stomach govern Qi, internal damp-heat flourishes due to daily injury to the Spleen and Kidney. Unclear physicians, making mistakes again and again, will inevitably lead to abdominal fullness. The left pulse being slow and choppy reflects long-term Yin-Yang depletion, which is reasonable. But why is the right pulse wiry and large? Is it due to excess physical stagnation? As Zhongjing said: "Wiry indicates decreased Stomach Qi, large indicates disease progression." This is also due to the gradual relaxation of Yangming meridians and edema spreading upward from below. Adhering to middle-jiao treatment hinders food intake and causes stifling fullness. The main idea is to promote movement in the middle and separation/drainage in the lower. Only when urination becomes natural and Fu organ opening/closing functions normally is there a turning point. If delayed for another month, when Yin generates at the Summer Solstice, it will be hard to reverse. Si Ling San (Four-Poria Decoction) plus Jiaomu (Spermacoce Seed), Houpo, Yizhi, Guangpi white part.
Again: After five days of separation-drainage methods, diarrhea reduced and urination通畅, but foot edema remains. Know that the Spleen and Stomach have been trapped for a long time, damp-heat stagnates turbidity, unable to circulate. Ingested food transforms into dampness through steaming; excess dampness forms the five diarrheas. To remove dampness, one must promote urination. However, excessive diuresis concerns the Lower Jiao for a sixty-six-year-old. During summer, when the Spleen and Stomach command, the Spleen should be tonified to become healthy, and the Stomach should be dispersed to become clear. Supporting righteous Qi and driving out damp-heat is the method of combining elimination and tonification. Take Zisheng Wan (Nourishing Life Pill) at night, sent down with stir-fried rice soup.
(Morning) Renshen (Ginseng Radix et Rhizoma) Guangpi (Citri Reticulatae Pericarpium) Fangji (Stephaniae Tetrandrae Radix) Houpo (Magnoliae Officinalis Cortex) Fuling (Poria) Sheng Zhu (Raw Atractylodis Macrocephalae Rhizoma) Zexie (Alismatis Rhizoma) Shenqu (Massa Medicata Fermentata) Huanglian (Coptidis Rhizoma) Wuzhu Yu (Evodiae Fructus)
Patient Zhu: Carelessness in diet and mouth led to internal damp-heat, recurrence of diarrhea. Excess dampness forms the five diarrheas; Qi stagnation causes abdominal distension. (Etiology: Damp-Heat)
Renshen (Ginseng Radix et Rhizoma) Mao Zhu (Atractylodis Rhizoma) Chuan Lian (Coptidis Rhizoma) Huang Qin (Scutellariae Radix) Bai Shao (Paeoniae Radix Alba) Guangpi (Citri Reticulatae Pericarpium) Fuling (Poria) Zexie (Alismatis Rhizoma) Zha Rou (Crataegi Fructus)
Patient Chen: Pulse is slow and large, abdominal pain and diarrhea, scanty urination. This is dampness generated internally from water and grain,郁蒸 (stagnating and steaming) in the intestines and stomach, causing confusion of clear and turbid. If not cleared and separated, it will extend into accumulation, sticky dysentery. Propose Qin Shao Tang (Scutellaria and Peony Decoction).
Dan Huang Qin (Light Scutellariae Radix) Sheng Bai Shao (Raw Paeoniae Radix Alba) Guangpi (Citri Reticulatae Pericarpium) Houpo (Magnoliae Officinalis Cortex) Huoxiang (Agastaches Herba) Fuling (Poria) Zhuling (Polyporus) Zexie (Alismatis Rhizoma)
Patient Zhang: Pulse is slow and choppy, abdominal fullness, painful diarrhea that is unsatisfactory. Qi stagnation for a long time, dampness congeals in the intestines. Use Danxi Xiao Wen Zhong Wan (Danxi's Minor Warming Center Pill).
Zhen Sha (Magnetum) Xiao Chuan Lian (Minor Coptidis Rhizoma) Cang Zhu (Atractylodis Rhizoma) Bai Zhu (Atractylodis Macrocephalae Rhizoma) Xiang Fu (Cyperi Rhizoma) Ban Xia (Pinelliae Tuber) Guangpi (Citri Reticulatae Pericarpium) Qing Pi (Citri Reticulatae Pericarpium Viride) Shenqu (Massa Medicata Fermentata) pill with malt paste.
Patient Cheng: Palpating the Liver area, the pulse is uniquely large, while Spleen and Stomach pulses are slow and weak. Normally eats little but spirit is good. Innate vitality is abundant. Currently watery diarrhea, lower abdomen full. The lower abdomen is the location of the Jueyin Liver. Due to failure to distinguish Yin and Yang, turbidity occupies the lower, causing Liver loss of free flow. Should use Wu Ling San (Five-Ingredient Poria Pill) to guide water and drain dampness, imitating the ancients' method of "opening branch rivers urgently."
Patient Huang (9 years old): Chronic diarrhea accompanied by sores indicates dampness prevailing with heat stagnation. Bitter-cold herbs must be assisted by wind-dispersing drugs, aligning with Dongyuan's principle: "The Spleen benefits from ascending to be healthy; the Stomach benefits from descending to be harmonious."
Renshen (Ginseng Radix et Rhizoma) Chuan Lian (Coptidis Rhizoma) Huang Bo (Phellodendri Cortex) Guangpi (Citri Reticulatae Pericarpium) Zhi Cao (Prepared Licorice) Sheng Bai Zhu (Raw Atractylodis Macrocephalae Rhizoma) Qiang Huo (Notopterygii Rhizoma et Radix) Fang Feng (Saposhnikoviae Radix) Sheng Ma (Ephedrae Herba - *Note: Context implies Cimicifugae Rhizoma based on 'ascending', usually Sheng Ma is Cimicifugae, text says Sheng Ma but lists Ephedrae in herb list earlier? Standard TCM Sheng Ma is Cimicifugae. I will translate as Cimicifugae Rhizoma*) Chai Hu (Bupleuri Radix) Shenqu (Massa Medicata Fermentata) Mai Ya (Hordei Fructus Germinatus)
Patient Zhu (34): Thin and pointed shape, Wood-Fire constitution. Since last year, diarrhea, ineffective treatments with Spleen/Stomach drugs. This is inherent deficiency of Yin Water, dampness from alcohol/food sinking, Yin weakness failing to contain. Should use bitter flavor to firm Yin, light-draining to overcome dampness.
