Teeth and teeth are of the same category but have different names. "Chi" refers to the inner bed (alveolar process/gums), while "Ya" refers to the outer plate (crown). The inner part is capable of chewing, whereas the outer part has no action. Being capable of chewing implies constant labor, while having no action implies constant rest. Constant labor makes it prone to injury, while constant rest ensures health. Therefore, pain mostly occurs in the inner part and rarely in the outer part. Regarding flavors (sour/pungent), qi intensity (thick/thin), texture (hard/brittle), and nature (cold/warm), during a single chewing motion, toxins from food and drink seep into the interdental spaces. If small remnants of food get stuck in between and cannot be removed immediately, and if one picks their teeth in a drafty place, even causing bleeding, how long before this becomes a pathogenic factor? Thus, toothache is mostly caused by Wind. When Wind enters internally, it causes immediate swelling and pain extending to the cheek, making chewing difficult. This is what people find most distressing. Besides Wind patterns, there are Fire and Worm categories. Wind is acquired externally, Fire arises from internal injury, and Worms are generated by Fire. Why say so? Teeth are the sprouts of bones and the surplus of Kidney Qi. The flesh at the tooth roots, deep within the interdental spaces, belongs to the Foot-Yangming Stomach Meridian. Now, those suffering from tooth disease, is it truly the teeth themselves that hurt? Teeth are harder than bones; they are neither blood nor sinew, but the most stubborn and wood-natured substance. How can they hurt? The site of pain is actually in the flesh deep within the interdental spaces. Because Yangming has Fire, heat steams the Stomach; when the Stomach meridian receives heat, it communicates upward to the teeth. Thus, the pain is accompanied by foul odor difficult to approach, deep red root flesh, fluid leaking from interdental spaces, and a salty taste. This is named "Yaxuan" (Gum Disease/Pyorrhea), often leading to rotting and ulceration. This results from Stomach Fire. Sometimes there is itching or pain, or unbearable severe pain that suddenly stops, leaving one feeling fine. This does not belong to Wind or Fire. It is worms boring through. Where do these worms come from? There must be some food residue left at the tooth roots, refined by Fire, and formed using Blood and Qi. If they bite the teeth, the teeth break; if they bite the flesh, the flesh hurts. If they do not bite but cause slight pain, the flesh itches. This is how worm pain differs from Wind and Fire. If there is no pain but the teeth become scorched, dry, and fall out, this is not Stomach Fire, but weak Kidney Qi failing to secure the roots. Therefore, elderly people's teeth are often loose and sparse, while young and strong people do not have this condition. Observing this allows for appropriate treatment.
