Loushuang (Sinus/Fistula), one must first take tonifying medicines to generate Qi and Blood, using Ren Shen (Ginseng), Bai Zhu (Atractylodes Macrocephala Rhizome), [missing character, likely Fu Ling or Gan Cao], Chuan Xiong (Sichuan Lovage Rhizome), and Dang Gui (Angelica Sinensis) as the main ingredients, taken in large doses. Externally, use Fu Zi (Aconite Lateralis Radix Preparata) powder, mixed with saliva to form cakes, about the thickness of a coin, and apply moxibustion. If the sinus is large, use a large moxa cone; if small, use a small cone. Moxa until it feels slightly warm, do not cause pain. If it dries, re-grind into powder, make a cake, and moxa again. If the patient feels fatigued, stop and resume the next day, continuing until the flesh levels out, which indicates efficacy. Some also use Fu Zi slices for moxibustion, still applying tonifying medicated plasters, which is particularly excellent. Zhilou (Hemorrhoidal Fistula), cool the Large Intestine, widen the Large Intestine Qi mechanism. Use Zhi Qiao (Aurantii Fructus) removing the pith, inserting Ba Dou (Crotonis Fructus), binding with iron wire, boiling thoroughly, removing Ba Dou, and using the Zhi Qiao for medicine. For pills, mash thoroughly; for decoctions, use dried material to widen the bowel. To astringe orifices, use Chi Shi Zhi (Halloysitum Rubrum), Bai Shi Zhi (Halloysitum Album), Ku Fan (Alumen), Huang Dan (Minium), and Bing Pian (Borneolum Syntheticum). For fistulous tracts, externally plug with Tong Zi Xiao Bian (Child's Urine)-soaked Lu Gan Shi (Calamina) and Mu Li Fen (Ostreae Shell).
Formula: Huang Lian San (Coptis Powder). Originally suffering from Zhilou, a mass appeared beside the anus, with thick skin, swelling, pain, and pus formation, discharging from the hemorrhoidal pore. Treat as Food Stagnation descending. Huang Lian (Coptidis Rhizoma), A Wei (Ferulae Resina), Shen Qu (Massa Medicata Fermentata), Shan Zha (Crataegi Fructus), Tao Ren (Persicae Semen), Lian Qiao (Forsythiae Fructus), Huai Jiao (Gleditsiae Sinensis Fructus), Xi Jiao (Rhinocerotis Cornu) (equal parts). Grind all into fine powder. Take a small amount, place on the palm, lick occasionally, and swallow the saliva. If symptoms resolve by two-thirds, stop taking further doses.
[Appendix] Loushuang (Sinus/Fistula) is the ulceration and leakage of various Wei syndromes (Wasting-Thirst/Atrophy Syndromes). There are Ye Lang Wei (Wild Wolf Atrophy), Shu Wei (Mouse Atrophy), Lou Wei (Mole Atrophy), Lou Gu (Mole), Feng Lou (Bee Fistula), Pi Fu Lou (Ant Fistula), Si Cao Lou (Grub Fistula), Fu Qu Lou (Fly Larva Fistula), etc., all of this category. Analyzed in detail, there are thirty-six types, with different names. Generally, external injury from the Four Qi (Four Climatic Factors), internal distress from the Seven Emotions, irregular diet, and infection/toxicity from insects and animals, all transform into Chuang (Sores/Ulcers). Once the perforation is deep, pus does not drain completely, cold combines with Wind pathogen, thus dripping continuously to form Loushuang. However, some Loushuang are recent, some are long-standing. Recent ones appear pale red, slightly swollen, or with small nodules; long-standing ones appear dry and white above, black and rotting inside, with parasitic worms and foul odor.
Attached Formulas: Zhu Shen Wan (Pig Kidney Pill). Used for Loushuang where foul fluid exits from the Large Intestine. Hei Qian Niu (Pharbitidis Semen) ground to fine powder 2.5 qian, placed in a pig kidney, tied with thread, wrapped in green bamboo leaves, slow-cooked until ripe. Chew and take with warm wine on an empty stomach. Ru Xiang Wan (Frankincense Pill). Treats Cold Loushuang. Ru Xiang (Boswelliae Resina) 2.5 qian, Mu Li Fen (Ostreae Concha) 1.25 qian. Grind to powder, bind with snow-ice paste into pills the size of hemp seeds. Take 30 pills each time, with ginger tea on an empty stomach. Sheng Di Huang Gao (Rehmannia Paste). General treatment for Loushuang. Lu Feng Fang (Vespa Nest) roasted, Wu Bei Zi (Gallae Chinensis), Mu Xiang (Aucklandiae Radix) 3 qian, Ru Xiang (Boswelliae Resina) 1 qian, Qing Fen (Calomelas) 1 zi. Grind to powder, take a handful of fresh Sheng Di Huang (Rehmanniae Radix), mash finely, mix into paste. Spread on raw silk and apply. She Tui San (Snake Slough Powder). Treats Loushuang with unceasing blood and water discharge. She Pi (Serpentis Skin) roasted until charred, Wu Bei Zi (Gallae Chinensis), Long Gu (Fossilia Ossis Mastodi et Hirte) 2.5 qian each, Xu Duan (Dipsaci Radix) 5 qian. Grind to powder. Add a little She Xiang (Moschus), mix with saliva for topical application. Fumigation Formula for Loushuang: Ai Ye (Artemisiae Argyi Folium), Wu Bei Zi (Gallae Chinensis), Bai Jiao Xiang (Dammar), Ku Lian Gen (Meliae Cortex) (equal parts). Chop and burn as incense. Place in a long barrel, sit over the fumigation site. Washing Formula for Loushuang: Treats foul matter in fistulous tracts; always keep wind away and wash clean. Wash with decoction of Lu Feng Fang (Vespa Nest) and Bai Zhi (Angelicae Dahuricae Radix), or Da Fu Pi (Arecae Pericarpium) and Ku Shen (Sophorae Flavescentis Radix). After washing, wait for water to drain and wipe dry. First, use Shi Liu Pi (Punicae Granati Cortex) grown facing east, dried and powdered, sprinkle dry to kill parasites, then after a while, apply medicine. Ji Wei Formula (For Chronic Atrophy): Jiu Kong Feng Fang (Nine-Holed Vespa Nest) roasted yellow. Grind to powder, mix with Du Zhi (Suillii Grease) from December, rub onto the area. When juices absorb, add Long Gu (Fossilia Ossis Mastodi et Hirte) and Jiang Xiang Jie (Aquilariae Lignum Resinatum) powder with a little Ru Xiang (Boswelliae Resina) to harden the sore. Loushuang, whether on legs or feet, initially stems from accumulated Heat, but becomes Cold over time. Break Fu Zi (Aconite Lateralis Radix Preparata) into two pieces, soak in human saliva until saturated, form into a piece, place on the fistula, and apply moxibustion. Another Formula: Chuan Xiong (Ligustici Chuanxiong Rhizoma) 0.5 liang, Xi Xin (Asari Radix et Rhizoma), Bai Zhi Shao (Angelicae Dahuricae Radix tip) 1.5 qian. Grind to powder. Take as decoction daily. If disease is below, take before meals; if above, take after meals. Depending on the sore size, find Huang Ma Gen (Hemp Root) from the previous year, scrape off skin, twist into rope, insert into hole until resistance is met, apply plaster externally.
